Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties and change how the body responds to insulin.1

How common diarrhoea is on Mounjaro

The most common effects, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort.1 The weight-management trial figures show how the rate changes with strength.

Diarrhoea was reported by 18.7% of people on 5mg, 20.8% on 10mg and 22.5% on 15mg, compared with 7.8% on placebo.1 So it rises modestly with higher strengths, but it is far from universal, and plenty of people have none at all.

Nausea, vomiting and diarrhoea were more common while the dose was being increased and became less frequent over time.1 Very few people leave treatment because of these effects: 1.9% of people on 5mg, 3.3% on 10mg and 4.3% on 15mg stopped treatment permanently because of gastrointestinal side effects, compared with 0.5% on placebo.1

Speak to your clinical team rather than pushing through severe symptoms.1 If you have stopped altogether, remember the medicine takes about four to five weeks to clear your system, and nothing needs doing while it clears.1

There is no way to switch it off instantly, but practical steps help most people. Eat smaller meals slowly, stop at the first feeling of fullness, favour plain lower-fat foods while adjusting, stay well hydrated and avoid lying down straight after eating.13

  • Keep fluids steady. Little and often works better than large amounts at once when stools are loose.

Very watery or sudden, urgent stools are an intense form of the same effect, and they deserve closer attention because of fluid loss.

Frequent watery stools raise the risk of dehydration. If you cannot keep fluids down, contact your clinical team — dehydration needs medical help straight away.13 Tell them how often you are going and how long it has lasted so they can advise on your dose.

Constipation is a known effect of the medicine,1 so if you swing between hard stools and sudden loose ones, mention that pattern to your prescriber or pharmacist.

When diarrhoea needs medical attention

Most diarrhoea on Mounjaro is a nuisance, not an emergency, but some symptoms need urgent attention. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis). Also seek immediate medical help for pain in the upper right abdomen, fever or yellowing of skin or eyes (possible gallbladder problems), or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.1

If you cannot keep fluids down, contact your clinical team — dehydration needs medical help straight away. If you also take insulin or a sulfonylurea, watch for low blood sugar (shakiness, sweating, confusion), because your prescriber may need to adjust those medicines.1
