How tirzepatide works in the body

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1.1 Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1

For a lifter, each of those effects has a practical consequence. Feeling full sooner means big meals become hard to finish. Slower stomach emptying means food sits longer, which can affect how you feel before and after a session. Improved insulin response is relevant to blood sugar control, which is why the medicine is also licensed for type 2 diabetes. None of these effects is about muscle tissue itself.

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. It comes as a pre-filled multi-dose pen, the KwikPen, in strengths from 2.5mg to 15mg: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Treatment starts at the lowest strength and your prescriber adjusts it over time.

The key point for physique athletes is what the mechanism does not include. It does not direct calories into muscle, it does not signal your body to hold on to lean tissue, and it does not replace the need for protein, training volume and recovery. It simply lowers the drive to eat. Whether that helps or hurts depends entirely on what you are trying to do and on how much you can still eat comfortably while taking it.

04 · What trials show

What the trial evidence does and does not show

The headline weight-loss evidence comes from SURMOUNT-1, and it is about total body weight, not body composition. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.2 Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 As a worked example only: for someone starting at 100 kg, 20.9% is about 21 kg.

Read those numbers carefully if you lift. They describe how much the scale moved. They do not tell you what that weight was made of, and the participants were adults with excess weight, not trained athletes aiming for low body fat.

It is also worth noting the difference between the licensed purpose and your goal. A trial that shows large average weight loss in people with excess weight is relevant to a similar person. It does not show that a lean, trained person will respond the same way, or safely. 05 · The muscle question

The muscle question: what you can and cannot control

The SURMOUNT-1 figures report total body weight, not body composition.2

What you can control is the part the treatment itself is built around: it works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 For a lifter, increased physical activity naturally means continuing resistance training.

Food is the second lever, and it is where the medicine bites. When appetite drops, meals can shrink without you noticing. Your prescriber or a registered dietitian can set a plan that suits your medical history.

Recovery is the third lever. Sleep, rest days and sensible training volume matter more when you are eating less. 06 · Cutting versus bulking

Cutting, bulking and why the goal matters

Cutting and bulking pull in opposite directions, and tirzepatide only fits the direction of eating less. The medicine helps people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 That is a cutting-style effect by definition.

During a bulk, the challenge is the reverse: getting enough food in. Strong appetite suppression makes that harder, and slower stomach emptying means meals feel heavy for longer. Using an appetite-suppressing injection during a phase where you are trying to eat more works against your goal. If you are bulking, the better question is whether you need the medicine at all.

During a cut, the picture is more tempting and more risky. The appeal is obvious, yet the licensed use is for adults who need to manage their weight, with diet and increased physical activity as the foundation.2 Using it to push a lean physique leaner is not that purpose.

There is also a middle path many people face: a lifter who has gained more weight than they want and wants to lose it steadily before returning to productive training. That is a weight-management goal, and it is the kind of conversation a prescriber can have with you. In that situation the goal is health first, with your training habits as an asset rather than the target.

Whichever phase you are in, avoid improvising. Do not alter the dose, stretch or space out doses, or combine products on your own initiative. Dose changes belong to the prescribing team. 07 · Safety and exercise

Like all medicines, Mounjaro can cause side effects, and most are digestive. The most common, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort. Side effects are most likely when starting treatment or moving up a dose, and for most people they ease within days to a few weeks as the body adjusts.13 Read the patient information leaflet before you start, and again when your dose changes. For athletes, a few sensible habits follow from how the medicine works. Because appetite drops, make sure you are eating and drinking enough to train safely. If you feel faint, get medical help straight away; if you feel unusually exhausted or unwell, stop the session and tell your prescriber.

If you are taking other medicines for diabetes, such as insulin or sulfonylureas, tell your prescriber. Doses of those medicines may need adjusting because of the risk of low blood sugar. That is an adjustment conversation, not a ban, but it must happen under clinical supervision, not through guesswork.

NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

09 · Stopping and planning

Stopping, clearance and planning around events

If you stop tirzepatide, it does not leave your system immediately. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system.1 There is no way to speed this up, and nothing needs doing while it clears.

That timing is relevant to anyone with a date in mind: a holiday, a photo shoot, a show. Because the medicine lingers, appetite effects do not switch off the moment you stop injecting, and they do not switch on instantly when you start.

Equally, do not try to manage the pen or dose to fit an event. Dose decisions stay with your prescriber at your monthly review.

Storage is straightforward. Unopened pens go in the fridge, 2–8°C, until first use, and they keep until the printed expiry date. Once in use, a KwikPen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Do not use a pen that has been above 30°C or has been frozen.13 If you travel to competitions or training camps, plan ahead for keeping the pen cool, and check the patient information leaflet or ask the pharmacy for anything not covered there.

Mounjaro manages weight; it does not cure the biology behind it. 10 · Serious side effects
