Can you do weight training on Mounjaro?
Yes. Mounjaro (tirzepatide) is a prescription-only medicine, supplied after an online consultation reviewed by a GPhC-registered prescriber. It is used alongside a reduced-calorie diet and increased physical activity.1 Lifting weights counts as physical activity, so there is nothing in the product's purpose that conflicts with it.
Tirzepatide activates the GIP and GLP-1 receptors, which helps regulate appetite and slows how quickly the stomach empties.1 Most people feel fuller sooner and less hungry between meals. For someone lifting, that matters in practice: eating enough to train well can take planning.

03 · Muscle and weight loss
Mounjaro and muscle
Many people ask whether they will lose muscle on Mounjaro. Some of any rapid weight loss comes from lean tissue, and protecting muscle protects your metabolism, strength and appearance. The two levers are adequate protein — aim to include a protein source at every meal, which is harder when appetite is low — and resistance exercise two or more times a week.
The trial evidence shows overall body-weight change. In SURMOUNT-1, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2
Those figures describe total weight, not the split between fat and lean tissue.
Losing weight while eating less is the setting in which many people choose to keep lifting. That is a sensible reason to train. It is not a promise about any particular outcome.
04 · Fuelling your sessions
Eating enough to train on lower appetite
The main practical challenge is appetite, not the gym. Tirzepatide helps regulate appetite and slows how quickly the stomach empties, so most people feel fuller sooner.1 That supports a reduced-calorie diet, but it can also make a pre-workout meal feel unappealing.
A few non-clinical habits tend to help people who lift:
- Plan the day's meals in advance, so you are not relying on hunger to prompt you to eat.
- Choose smaller, easily digested options before training if a large meal sits heavily.
- Pay attention to protein at each meal.
- Drink fluids regularly through the day.
If you struggle to eat adequately, speak to your prescriber. If you feel faint when exercising, get medical help straight away. You can also report any suspected side effect through the Yellow Card scheme.19
05 · Building a routine
Building a sensible routine
A good routine is one you can repeat. Choose a small number of sessions you can realistically sustain, focus on controlled technique, and increase the load gradually.
Compound movements such as squats, rows and presses are a common foundation in general strength training. If you have joint problems, a heart condition or have been inactive for a long time, get advice before starting. A qualified trainer or physiotherapist can tailor a plan.

Dose increases and how you feel in the gym
Mounjaro comes as a once-weekly KwikPen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Your prescriber steps the dose up over time, and our team provides ongoing monthly reviews, dose titration and side-effect support.
It is reasonable to treat each increase as a moment to check in with yourself. Keep sessions manageable, notice how appetite and energy change, and avoid chasing personal bests until you know how you feel at the new strength. This is common-sense pacing, not a clinical rule.
The most common side effects, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort, and side effects are most likely when starting treatment or moving up a dose.13 If a new symptom appears during exercise, discuss it with your prescriber instead of working through it.
07 · Breaks and stopping
Breaks from the gym and stopping treatment
Missing sessions happens, through illness, travel or work. A short break is a normal part of any training life, and the usual advice is simply to return gradually.
If you ever stop Mounjaro, tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system.1 There is no way to speed this up, and nothing needs doing while it clears. Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average.14 Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.

08 · Access and cost
Getting treatment if you train
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).4 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

09 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
