04 · Why everyone starts at 2.5mg

Why everyone starts on 2.5mg

Everyone starts Mounjaro on 2.5mg once a week. This dose is about letting your body adjust, not about weight loss itself.1 After at least 4 weeks the dose increases to 5mg. It can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) to a maximum of 15mg weekly, depending on response and tolerance. Many people settle at 5mg, 10mg or 15mg, and higher is not automatically better.1

05 · The first weeks

What to expect in the first weeks

The 2.5mg weeks are an adjustment phase, and experiences differ widely. Some people notice appetite change in week one, others little until a higher dose, and both are normal. Early loss includes water weight, so the trend matters more than any single weigh-in. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.

Weight loss on Mounjaro is gradual by design, and most measurable progress builds from 5mg onward. In the SURMOUNT-1 trial of 2,539 adults, people lost on average 15% of body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, against 3.1% with lifestyle changes alone. Individual results vary.2

06 · Using the pen

How to use the 2.5mg pen

Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food. Licensed sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site each dose. Each KwikPen contains four weekly doses.1

If you miss a dose, take it within 4 days (96 hours) of the scheduled time. After more than 4 days, skip it and take the next on your usual day, never two doses within 3 days of each other.13 Keep pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, then it must be thrown away. Do not use a pen that has been above 30°C or frozen.1

The MHRA has seized thousands of fake weight-loss pens in the UK and issued alerts after falsified Mounjaro pens were identified. Some contained insulin rather than tirzepatide and caused hospitalisations. Only buy from a pharmacy you can verify on the GPhC register, never from social media, beauty salons or unverified websites.7

08 · NHS or private

NHS or private prescription

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.4 Full rollout is expected to take years, so many people who meet the licensed criteria choose regulated private treatment. Licensed use is for adults with a BMI of 30 or above, or 27 to 30 with a weight-related condition, alongside a reduced-calorie diet and more activity.1

How often can I take 2.5mg Mounjaro?
Mounjaro is a once-weekly medicine, taken on the same day each week. A 2.5mg pen holds four weekly doses. After at least 4 weeks the dose normally steps up to 5mg. Do not take it more often or stretch it to fortnightly, as both are unlicensed.
Can diabetics get Mounjaro on the NHS?
Mounjaro is licensed in the UK for type 2 diabetes as well as weight management, and it is available on the NHS with access phased in over several years. Ask your GP or diabetes team whether you qualify. If you take insulin or sulfonylureas, doses may need adjusting to avoid low blood sugar, so tell your prescriber.

What should I avoid while taking Mounjaro?
Never use a frozen, cloudy or expired pen, and buy only from a pharmacy on the GPhC register. Tell your prescriber about new symptoms or medicines. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.
What drugs should you not take with Mounjaro?
Tell your prescriber about every medicine you take. Mounjaro can reduce absorption of oral contraceptives because stomach emptying slows. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Insulin and sulfonylureas may need dose adjustments because of low blood sugar risk. Your prescriber reviews your medicines before deciding whether it is suitable.
Can I eat bananas while taking Mounjaro?
No food is banned on Mounjaro, including bananas. Mounjaro is used alongside a reduced-calorie diet and increased physical activity. Ask your prescriber for dietary advice tailored to you.
When is the best time of day to take Mounjaro?
There is no best time of day. Mounjaro works the same morning or evening, with or without food. What matters is the same day each week. Many people choose a weekend morning so early side effects fall on days off. You can change your day if at least 3 days have passed since your last dose.
