Where 12.5mg sits on the Mounjaro dose ladder

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose KwikPen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

Everyone starts on 2.5mg once a week. That starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg. It can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 That makes 12.5mg the fifth strength on the ladder and the step just below the maximum licensed dose.

Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace.1 Some people reach 12.5mg and stay there because it suits them; others stop at a lower step. This matters for price comparison because you only need a 12.5mg pen if your prescriber has taken you there. How tirzepatide works

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 This dual action distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic).

How to compare Mounjaro prices fairly

A fair comparison matches dose, pack size, date and inclusions. Many comparison tables mix first-order promotional prices with standard prices, or list different doses side by side, which makes the gaps look bigger or smaller than they are. Before trusting any chart, check five things.

  • Same dose. A 12.5mg pen must be compared with another 12.5mg pen, not with a starting pen.
  • Same pack. Our prices are per pen, which is four weeks of weekly doses.

Treat a very low price with caution. Genuine UK stock is supplied through Lilly's official distribution to licensed pharmacies, so a price far below the rest of the range deserves questions about who is supplying the pen. A fair comparison also never involves a seller offering to supply without a consultation or prescription. If a price looks too good to be true, check the seller on the GPhC register before you pay anything.

06 · Planning the cost

Planning what reaching 12.5mg costs over time

You do not start on 12.5mg, so the real cost question is what the whole route costs. Everyone starts on 2.5mg, moves to 5mg after at least 4 weeks, and then steps up by 2.5mg at a minimum of 4-week intervals.1 The minimum time to reach 12.5mg is therefore set by the licence, and your own pace may be slower if your prescriber holds a dose longer.

At standard prices, before discounts, if you step up after 4 weeks on each of 2.5mg, 5mg, 7.5mg and 10mg, your first four pens cost £959.96. Many people settle at 5mg, 10mg or 15mg, so a plan that assumes 12.5mg may overstate or understate your own costs.1 Treat any total as a planning tool, and revisit it at each monthly review, where the prescribing team looks at how you are doing before any dose change.

07 · Is 12.5mg right

Is 12.5mg right for you, and what do trials show?

Whether you need 12.5mg is a clinical decision, and it starts with whether Mounjaro is suitable at all. Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity. A prescriber will assess your BMI, medical history and current medicines before deciding whether it is suitable for you.1

What the main trial reports

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages; individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 As a worked example only, for someone starting at 100 kg, 20.9% is about 21 kg.

Pace, review and stopping rules

Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend, not individual weigh-ins. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.11

First weeks: what is normal

Experiences differ widely in the first weeks. Some people notice appetite change and early loss in week one, others feel little until a higher dose, and both are normal. Early loss includes water weight. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1

Mounjaro is a prescription-only medicine, supplied only after an online consultation reviewed by a GPhC-registered prescriber. Individual results vary.

08 · NHS versus private

NHS or private: how Mounjaro access really works

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

What this means for price

On the NHS in England, a prescription normally carries the standard charge, currently £9.90 per item unless you are exempt, though the right route for you depends on whether your GP service offers it to your group. If you are eligible and are offered NHS treatment, that is a legitimate and sensible option to explore with your GP first.

Choosing between the two

The honest picture is that the NHS route depends on where you sit in the phased rollout. The private route depends on a clinical consultation and on paying the price. Neither route changes the medicine: Mounjaro is tirzepatide, supplied as the same KwikPen. A regulated private pharmacy must still review your BMI, medical history and current medicines before supplying.

  • Choose to ask your GP first if you are in a high-need group that the phased rollout is reaching, and you are happy to wait for local services.
  • Consider private treatment if you meet the licensed criteria and want to start sooner, accepting the cost.
  • Either way, use only a pharmacy you can verify on the GPhC register.

Diabetes and the NHS

Mounjaro is also licensed for type 2 diabetes in the UK, and NICE has separate guidance for that use.8 Whether it is offered on the NHS for diabetes is decided by your diabetes team or GP based on that guidance, so ask them directly. The NHS also provides its own overview of the medicine for both conditions.5

Avoiding fake pens when you compare prices

The biggest risk in price shopping is a counterfeit pen. The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified. Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.7 Only buy from a pharmacy you can verify on the GPhC register, never from social media, beauty salons or unverified websites. Genuine treatment always involves a prescription and a clinical consultation.12

What a genuine pen looks like

Mounjaro is manufactured by Eli Lilly. Genuine UK stock is supplied through Lilly's official distribution to licensed pharmacies, and every pen carries a batch number and expiry date that can be checked.7 A genuine UK pen comes from a registered pharmacy, arrives cold-chain packaged, and the solution inside is clear and colourless.13

Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, has been above 30°C, or has been out of the fridge for more than 30 days in use. If supply or packaging ever seems wrong, check the pharmacy on the GPhC register and contact the pharmacy before injecting.13

A checklist before you buy

Cost-saving supplements marketed under nicknames such as a "poor man's Ozempic" are a different matter. They are low-cost supplements with no clinical oversight, and their dosing is not standardised, so they are not a like-for-like alternative to a licensed medicine and we do not treat them as one.

11 · Using your pen

Using, storing and travelling with your 12.5mg pen

Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food. Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose. Each KwikPen contains four weekly doses.1 Needles, a sharps bin and injection guidance come with every order.

Day, time and missed doses

There is no best time of day. Mounjaro works the same morning or evening, with or without food, and what matters is the same day each week. Many people choose a weekend morning so early side effects fall on days off.1

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other. You can change your injection day if at least 3 days (72 hours) have passed since your last dose.1

Storage

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, which is useful for short trips, after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1 Unopened pens should be kept in the fridge and keep until the expiry date printed on the box.13

Travelling

You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements; for hot climates use an insulated pouch. Check destination rules for travelling with prescription injectables before you fly.1

If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit, because figures circulating online often come from other countries' labels.13 Storage matters for value as well as safety: a pen that has been frozen or left out cannot be used, and its cost is wasted.

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
