Medical weight loss shakes are over-the-counter drinks sold to help people cut calories, and the word "medical" is mostly a marketing label rather than a licence. They are foods or food supplements. They are not prescription medicines, and they have not been through the licensing process that a medicine such as tirzepatide has.

In practice you will meet two types. Meal-replacement shakes are designed to stand in for a whole meal and usually add vitamins and minerals. Protein shakes (including ready-to-drink bottles and powders) are designed to raise your protein intake, and generally do not replace a full meal's nutrition. Bars sold alongside them follow the same split.

The distinction matters because the labels overlap. A tub that says "diet protein" is not the same as a nutritionally complete meal replacement, so read what the product claims to be before relying on it for a meal.

Using shakes alongside Mounjaro

Shakes can sit alongside Mounjaro, and the main reason people use them is protein. Tirzepatide activates the GIP and GLP-1 receptors, which helps regulate appetite and slows how quickly the stomach empties, so most people feel fuller sooner and less hungry between meals.1 That is helpful for a reduced-calorie diet, but it can make it hard to eat enough protein.

Because large portions sit heavily when the stomach empties more slowly, a smaller liquid option can feel easier on some days.

Mounjaro is a once-weekly prescription injection. A shake cannot replicate its action, and a shake is never a substitute for prescribed treatment.

05 · Protein and muscle

Why protein matters when appetite is low

Some of any rapid weight loss comes from lean tissue, and protecting muscle protects your metabolism, strength and appearance. The two levers are adequate protein and resistance exercise two or more times a week. Aim to include a protein source at every meal, which is harder when appetite is low. This matters more, not less, as the dose rises and appetite falls.

A protein shake is one way to close a gap on days when solid food is unappealing, but real meals remain the foundation. No food is banned on Mounjaro, though many people feel worse after very fatty, fried or very sugary food. Smaller, slower meals tend to work better.

Treatment is licensed alongside a reduced-calorie diet, so the aim is a sustainable deficit, not a perfect food list.

06 · Hydration and supplements

Fluids, vitamins and what shakes do not replace

Drinking enough matters more on Mounjaro than usual: reduced appetite means less fluid from food, and vomiting or diarrhoea can dehydrate you quickly. Keep water within reach through the day. A shake counts as a drink but should not be your only fluid. If you cannot keep fluids down, contact your clinical team — dehydration needs medical help straight away.

No supplement is required with Mounjaro, but eating much less can leave gaps, so a standard daily multivitamin is a reasonable safety net while losing weight. Discuss anything beyond that with a pharmacist. Supplements sold as "natural GLP-1 boosters" are not equivalents and are not substitutes for prescribed treatment. Oral supplements, like other oral products, may be absorbed more slowly when you start treatment.

Meal-replacement shakes often contain added vitamins, which can overlap with a multivitamin. Check the label and ask a pharmacist if you are unsure.

07 · Choosing a shake

How to choose a shake sensibly

Choose by label, not by marketing. Look at whether the product is a complete meal replacement or a protein top-up, how much protein it provides, which sweeteners it uses, and whether you tolerate it. Anyone wanting to avoid artificial sweeteners should read the ingredients list, because sweetener use varies widely between brands.

Watch the extras. High-calorie add-ins, sweetened milk drinks and large servings can undo a calorie deficit, in the same way that high-calorie coffee drinks are an easy place for a deficit to leak.1 Avoid products that promise dramatic weight-loss claims or specific results; no shake can offer that.

Shakes are not suitable for everyone. If you have a kidney condition, take regular medicines, or are older and losing weight unintentionally, speak to a pharmacist or GP before relying on them. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. Alcohol is worth a note too: there is no direct interaction with Mounjaro, but alcohol adds calories that work against your goals.

08 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis) — stop using Mounjaro and get medical help straight away; pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems) — get medical help straight away; or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing — get medical help straight away. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

09 · NHS and clinical routes

NHS and private routes beyond shakes

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
