03 · Why GLP-1 supply gets tight

Why GLP-1 medicines sometimes run short

Several things can tighten supply without a formal national shortage:

  • Sudden demand spikes when news stories, price changes or celebrity coverage send many people to order at once.
  • Dose-specific gaps, where one strength is scarce while others are plentiful. Mounjaro comes in six strengths, so a gap in one is not a gap in all.
  • Wholesale and distribution limits, which control how much stock reaches each pharmacy even when the manufacturer is producing normally.
  • Cold-chain handling, because pens must be stored at 2–8°C until first use, so stock cannot simply be held loosely anywhere.

None of these tells you the medicine is permanently unavailable. They tell you that supply is a moving target and that a good provider plans around it. Once a pen is in use, it can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away.13

How to check whether a shortage claim is real

Treat a shortage claim as unproven until it comes from an official or regulated source. The internet is full of alarming posts, and the loudest ones are often the least reliable. A few habits help you sort real information from noise.

  • Look for who is speaking. A regulator, the manufacturer or a registered pharmacy is more reliable than a forum thread or video title in capital letters.
  • Separate local from national. A friend who could not find a pen at one shop has described a local gap, not a national shortage.
  • Ask what exactly is short. One strength, one wholesaler or all supply? The answers differ enormously.
  • Check the date. Old stories resurface constantly and look current. If a post gives no clear date, do not rely on it.
  • Go to the supplier. The most useful evidence is whether you can order your strength now. Ours is shown live as in stock on every page.

That change in behaviour can look like a supply problem, because demand shifts quickly even if the amount of medicine has not changed.

06 · NHS and private supply

NHS and private supply: do they differ?

They draw on the same medicine but reach patients through different routes, so your experience of availability can differ. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46

Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting. If you are already on an NHS prescription and your pharmacy cannot fill it, speak to your GP or prescriber rather than trying to fix it yourself.

What can I take instead of Mounjaro?

The only sensible alternative is another licensed medicine chosen with a prescriber, not a supplement or an unregulated product. For most people considering a swap, that means Wegovy (semaglutide), a once-weekly injection like Mounjaro. Liraglutide (Saxenda, and products such as Erly) is a different option. It is supplied at 6mg/ml and is a daily injection, unlike once-weekly Mounjaro and Wegovy.

The two main medicines work differently. Tirzepatide activates two gut-hormone receptors, GIP and GLP-1, which distinguishes it from single-action medicines such as semaglutide.1 A prescriber can weigh your history, tolerance and goals against each option.

Remember that any starting point matters. Your prescriber decides when to switch and which dose to start on, and Wegovy's licensed dose ladder starts at 0.25mg, so ask before you act.

08 · Staying on track safely

Staying on track if supply is disrupted

The safest response to a supply wobble is to plan early and keep your prescriber informed.

If a gap does mean you miss treatment, it helps to know that tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 What that means for restarting your dose depends on how long the gap was, so ask your prescriber rather than guessing.
