For the 2.5mg starter pen specifically, the cheapest and dearest providers in our index run from £143.96 to £179.99. For the same dose, the smallest and largest gaps between providers are £27 and £41. Boots' Mounjaro dose prices run from £179.00 to £339.00. All figures are per month, meaning one four-week pen, and our index was checked 10 October 2026.

Why do prices differ at all? Eli Lilly raised the UK list price of Mounjaro effective September 2025, and pharmacy sell prices vary because of confidential discounts between Lilly and each pharmacy.10 That means two pharmacies can pay different amounts for the same pen and pass on different prices. It also means prices move, so a comparison article from last year, or a search snippet quoting a figure, may be out of date.

When you compare, follow a short checklist. First, compare the same dose, since a 2.5mg pen and a 10mg pen are not priced the same. Second, look at the price after any introductory offer, because many providers discount the first order. Third, check whether the quoted price includes the consultation, delivery and clinical follow-up. Fourth, confirm the provider is on the GPhC register, which takes a minute. Stage two is every later order. After your first order, an automatic £25 reorder discount applies to every future order (code DOSE25). Every later order is £25 off the standard price of the dose you are on. So the ongoing monthly cost at any dose is that dose's standard price minus £25. The standard prices are £169.99 for 2.5mg, £218.99 for 5mg, £279.99 for 7.5mg, £290.99 for 10mg, £319.99 for 12.5mg and £329.99 for 15mg, each for one four-week pen. Per week, those work out at £42.50, £54.75, £70.00, £72.75, £80.00 and £82.50.

Your dose steps up over the first months, with at least 4 weeks on each dose, depending on how you respond. If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96 at standard prices. Once you settle on the dose your prescriber considers right for you, cost depends on that dose: £279.99 per month at 7.5mg, £290.99 at 10mg and £319.99 at 12.5mg.

All of these totals are at standard prices, before discounts. A new customer's first order is £40 less, and every later order is £25 less. So your actual spend will be lower than the standard-price totals. The reorder discount matters most over a year, because it is applied to every order after the first rather than just once.

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages and individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them. For someone starting at 100 kg, 15% is about 15 kg.

Only buy from a pharmacy you can verify on the GPhC register, never from social media, beauty salons or unverified websites.12 Genuine treatment always involves a prescription and a clinical consultation.

Where does genuine stock come from? Mounjaro is manufactured by Eli Lilly, and genuine UK stock is supplied through Lilly's official distribution to licensed pharmacies. How do you check a pen is genuine and still good? A genuine UK pen comes from a registered pharmacy with a batch number and expiry date, arrives cold-chain packaged, and the solution inside is clear and colourless.13 Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, has been above 30°C, or has been out of the fridge for more than 30 days in use. If supply or packaging ever seems wrong, check the pharmacy on the GPhC register and contact the pharmacy before injecting. For unopened pens, store them in the fridge until first use; they keep until the printed expiry date.

09 · NHS, diabetes and private cost

NHS access versus paying privately

Mounjaro is available on the NHS, but access is being phased in over several years, so the cheapest route for some people is NHS treatment if they qualify. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition.4 The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.6 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

What does that mean in practice? If you fall into a highest-need group, ask your GP about NHS access first. NHS prescriptions in England carry the standard prescription charge of £9.90 per item where it applies, though exemptions exist. If you do not yet qualify, or you do not want to wait, private prescribing is the alternative. Neither route is better in every case: it depends on your circumstances and timing.

A prescriber will assess your BMI, medical history and current medicines before deciding whether it is suitable for you. 10 · Cheaper alternatives examined

Cheaper alternatives and "poor man's Ozempic"

There is no like-for-like cheaper replacement for Mounjaro, but there are other licensed medicines and a lot of marketing for products that are not. Knowing the difference stops you spending on something that does not work.

Start with the other licensed GLP-1 medicines. Wegovy (semaglutide) is the main comparator. Its pens are 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once weekly, and titration steps up after at least four weeks, reaching the 2.4mg maintenance dose from week 17.18 Tirzepatide differs because it activates two gut-hormone receptors, GIP and GLP-1, rather than one.1 Your first Wegovy pen with us is £99.99 as a first-order price, and you can compare it with our weight loss treatments. Which is right for you is a clinical decision for your prescriber, and cost should be only one part of it. Next, tablets. There is no tablet form of Mounjaro, as tirzepatide is injection-only in the UK; Wegovy tablets and Foundayo are licensed GLP-1 tablets, and both are prescription-only.

Then there is the phrase "poor man's Ozempic" and the idea of mimicking Ozempic naturally. It is usually applied to supplements such as berberine, and sellers market these as "nature's Ozempic". These products are low-cost supplements with no clinical oversight, available cheaply without assessment, and their dosing is not standardised. They are not licensed weight-loss medicines and are not an equivalent to a GLP-1 medicine. Dramatic weight-loss claims made for them should be treated with caution.

Is there a best weight loss pill? There is no single answer, and anyone giving one is oversimplifying. The right medicine depends on your BMI, health conditions, other medicines and preferences, which is exactly what the prescriber review is for.
