For most adults who meet the licensed criteria, a prescribed weight loss medicine is a reasonable, well-studied option, provided a clinician has checked your history first. Mounjaro is a once-weekly injectable prescription medicine licensed in the UK for weight management in adults. It is used alongside a reduced-calorie diet and increased physical activity, not instead of them.1

The risks fall into three groups. The first is common, mild and temporary: digestive upset as your body adjusts. The second is uncommon but serious, such as pancreatitis or gallbladder problems. The third is avoidable: buying from sellers who offer to supply without a consultation or prescription, or combining the medicine with conditions that make it unsuitable.

First decide whether you meet the criteria. Mounjaro is licensed for adults, and NICE recommends it on the NHS for adults with a BMI of at least 35 plus at least one weight-related condition. 03 · Common side effects

Common side effects and what to expect

The most frequent effects are digestive: nausea, vomiting, diarrhoea, constipation and stomach upset. They tend to appear when you start treatment or step up a dose, and for most people they ease as the body adjusts. Dizziness is also a common effect, and feeling tired is very common.13

These effects follow from how the medicine works. Tirzepatide acts on gut-hormone receptors that regulate appetite and slow how quickly the stomach empties, so food stays in the stomach longer and you feel fuller sooner.1 That is helpful for appetite, and it also explains the fullness, nausea and bowel changes.

Report any side effect through the MHRA Yellow Card scheme, and tell your prescriber about anything new or persistent.19

04 · Serious risks and red flags

Serious risks: when to seek help

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.13

If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.13

Do not wait to see whether severe abdominal pain passes. If you have severe, persistent stomach pain, stop using Mounjaro and get medical help straight away.

05 · Who should not take it

Who should not take Mounjaro

Mounjaro is not suitable for everyone. You should not take it if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive.13 Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.

Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems. Always share your full medical history and medicine list during consultation.13

This is the main safeguard in a regulated service. A genuine consultation is a clinical review, not a scoring script. Anyone willing to supply without one is skipping the step that protects people with these conditions.

06 · Thyroid, kidneys, mood

Thyroid, kidneys, liver and mood

Thyroid. In rodent studies tirzepatide caused thyroid C-cell tumours; it is not known whether this applies to humans, and no increase has been shown in human trials. As a precaution, tell your prescriber if you or a close relative have had medullary thyroid cancer or the inherited condition MEN 2 — they will weigh this in your assessment. Routine thyroid monitoring is not required for most people.1

Kidneys and liver. Mounjaro is not known to damage kidneys or liver directly, and no dose adjustment is needed for mild to moderate kidney or liver impairment. The practical risk is indirect: severe vomiting or diarrhoea can cause dehydration, which can strain the kidneys — so keep fluids up and contact your clinical team if you cannot keep fluids down. People with severe impairment need individual prescriber assessment.1

Mood. Regulators have looked specifically at whether GLP-1 medicines are linked to low mood or suicidal thoughts: the EMA's safety committee reviewed the evidence and concluded in 2024 that it does not support a causal link, and the MHRA keeps this under continuous review. That said, any big change in weight, appetite and routine can affect how you feel — tell your prescriber about any history of depression, and seek help promptly if your mood drops or you have distressing thoughts during treatment.17

If you have thoughts of harming yourself or ending your life, get help straight away. Call 999 or go to A&E if you are in immediate danger. In England you can call NHS 111 and choose the mental health option, any time. You can also call Samaritans free on 116 123, day or night. Tell your prescriber about any change in your mood while taking Mounjaro.

07 · Benefits that offset risk

The benefit side of the safety question

Safety is always a balance against benefit. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels. These are trial-average secondary outcomes, not promises — but they are why prescribers talk about weight management as health management rather than appearance.2

08 · Stopping and regain

Stopping treatment and weight regain

Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average.14 Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1 Appetite effects therefore fade slowly, not overnight.

09 · Safe supply in the UK

Where safety is won or lost: the supply route

Much of the real-world danger comes from where the medicine is bought, not the medicine itself. The MHRA publishes counterfeit medicine alerts, and sellers who offer to supply without a consultation or prescription cannot show what is in the pen or how it was stored.7

Store unopened pens in the fridge at 2–8°C until first use. Once in use, a KwikPen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Do not use a pen that has been above 30°C or has been frozen. Follow the leaflet and the pen's instructions for injecting.13

What about other weight loss drugs?

Semaglutide (Wegovy) and liraglutide are other licensed injectables; liraglutide is a daily injection, unlike once-weekly Mounjaro and Wegovy. Each has its own product information, so ask your prescriber or pharmacist to compare them for you.

Older medicines such as fen-fen, Belviq and Meridia have their own histories, covered in separate guides. Experimental or hyped products, including the Moderna weight loss drug story, are not licensed here, so they cannot be called safe or unsafe on UK evidence.

Supplements sold as natural alternatives are low-cost and available without assessment. Dosing is not standardised, and they are not equivalent to licensed medicines.

11 · NHS and private access

NHS and private routes

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

Either route should involve the same safeguards: a history review, clear advice on side effects and follow-up.
