Is Mounjaro More Effective in the Stomach or Thigh? (October 2026)

Nothing shows Mounjaro works better in the stomach or the thigh: the headline trial results are averages for people using the pen as directed, not results split by injection site.2 Choose the site the patient leaflet permits that is most comfortable and easiest to reach, and rotate it weekly.

A quick overview

What to know
before you start.

  • Pick a site listed in the pen's patient leaflet, based on comfort, reach and how easily you can see what you are doing.
  • Rotate the site each week to reduce soreness, bruising and lumps at the injection site.
  • Stomach-area nausea and discomfort come from how the medicine acts on digestion, not from where you inject.
  • Follow the pen's own instructions for injecting, and ask us if you are unsure.
In this guide

01 · The core answer

Stomach or thigh: what actually decides it

Neither site is shown to be more effective, so the decision is practical rather than clinical. The SURMOUNT-1 trial reported that people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 Those are trial averages, individual results vary, and the figures are not presented by injection site.

What shapes your results is the dose you reach, how steadily you stay on treatment, and the diet and activity you pair it with. A missed or rushed injection because the site felt awkward matters far more than which patch of skin it went into. If you want a deeper comparison of the two areas, our guide to comparing thigh and stomach covers the practical differences in more detail.

02 · Decision

How to choose your site

Start with the sites named in the patient information leaflet that comes with your pen, then pick by comfort and reach.13 Most people find the site they can see clearly and hold steady is the one they use most reliably.

If one site is sore or leaves a tender lump, change it. You can alternate between permitted sites week to week, which spreads the load on your skin. Our related guide on choosing a better injection site walks through the same decision from the comfort angle.

03 · What to do next

What to do if you are still unsure

If you are already on treatment, your prescribing team can talk through technique at your monthly review. If you are considering treatment, you can start with a free consultation for the Mounjaro pen, reviewed by a GPhC-registered prescriber. Needles, a sharps bin and injection guidance come with every order, so you will not be left guessing at your first injection.

04 · What the evidence covers

What the trials show

The trials behind Mounjaro measured weight change across whole groups of people, not site by site. In SURMOUNT-1, which enrolled 2,539 adults, people on tirzepatide lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

The medicine is a once-weekly injectable made by Eli Lilly, supplied in a pre-filled multi-dose KwikPen.1 The pen is designed to deliver the dose you have dialled under the skin, and your strength is what shapes the result.

05 · Why people think stomach works better

Why people believe the stomach works better

The belief usually comes from two things that are easy to confuse: where you inject and where you feel the medicine working. Mounjaro is about appetite, fullness and how quickly the stomach empties.1 Tirzepatide activates the gut-hormone receptors GIP and GLP-1 together, which helps regulate appetite, slows how quickly the stomach empties, and improves the way the body responds to insulin.

Because the effects are felt around the belly, such as fullness and sometimes nausea, it is natural to assume the injection should go near it. Feeling fuller sooner is a result of the dose in your system, whichever approved site you used.

Finally, many people simply find the stomach area the easiest place to inject, so it becomes the default. That is a convenience point. It is not evidence of a stronger response.

06 · Pain and side effects by site

Does the stomach or thigh hurt more, and what about reactions?

Mild redness, itching, a small bruise or a tender lump at the injection site is common and normally settles within a few days.13 Rotating sites each week and letting the pen warm briefly out of the fridge reduces stinging.

In numbers, injection-site reactions were reported by 8.0% of people taking tirzepatide in the weight-management trials, compared with 1.8% on placebo.1 That tells you reactions are a known, usually minor part of treatment, but it does not tell you which site is gentler.

Contact your clinical team if a reaction is spreading, hot, very painful, or lasts more than a week. Do not simply push on with the same spot if it is clearly irritated. Move to a fresh area and report anything unusual.

Stomach-area symptoms are not injection-site problems

Do not confuse a reaction in the skin with the digestive effects of treatment. The most common side effects are nausea, diarrhoea, vomiting, constipation and abdominal discomfort, and they are most likely when starting treatment or moving up a dose.1 They usually ease within days to a few weeks as the body adjusts. Those symptoms follow from how the medicine works on digestion. They are not a sign that you injected in the wrong place.

07 · Injection technique and spacing

Pinching, belly button spacing and thigh areas to avoid

Your pen's instructions for use are the authority on technique.

Pinching. Follow the instructions that come with the KwikPen on whether to pinch or gather the skin.

Belly button. Inject in the stomach area at least 5cm from the belly button.

Thigh areas to avoid. Skip skin that is bruised, tender, scarred, hard or irritated, and skip anywhere your clothing or movement would rub the site. Use the front of the thigh, as the leaflet describes.

Between injections. Rotate each week. If you always use the same spot, you increase the chance of soreness and lumps. A simple pattern, such as alternating between permitted sites or moving along a line, keeps it easy to remember. The free GLPTrackr companion app can help you log which site you used.

If anything about technique is unclear, ask your prescribing team. Support is available Monday to Friday, and the aftercare includes side-effect support.

08 · Rotation and routine

Why rotating sites matters more than which site

Rotation is the part of site choice with actual support. Rotate the site with each dose; letting the pen warm briefly out of the fridge reduces stinging.13 Over a long course of once-weekly injections, using fresh skin each time keeps treatment comfortable enough to continue.

A sensible routine looks like this:

  • Let the pen warm briefly out of the fridge before use so it is not fridge-cold.
  • Pick your site for the week, different from last week, from the areas the leaflet permits.
  • Check the skin is clean, intact and not tender.
  • Inject following the pen's instructions, then dispose of the needle in your sharps bin.
  • Note the site in a diary or in GLPTrackr so next week is easy.

Keep the same day each week where you can. Speak to your clinical team rather than pushing through severe symptoms.1

09 · Comparing with other injections

Does the same apply to other weight-loss injections?

Questions about site often come up for other GLP-1 medicines too, such as semaglutide. Each medicine has its own product information and its own approved sites, so check the leaflet in your box.

If you are weighing medicines against each other, injection site is a minor consideration. Efficacy, tolerability, dosing and cost matter more. Our guides on semaglutide versus tirzepatide and Mounjaro versus Wegovy go through the trial evidence properly, including the SURMOUNT-5 head-to-head. Ozempic is semaglutide licensed for type 2 diabetes. It is not a UK weight-loss medicine, and a regulated UK pharmacy will not supply it for weight loss: see Mounjaro compared with Ozempic.

10 · When to ask for help

When to contact your team or report a problem

Most injection-site issues are minor and settle on their own, but some deserve a message to your clinical team. Contact them if a reaction is spreading, hot or very painful, or lasts more than a week.13

Some symptoms are not about the skin at all and need urgent attention. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting, which could be pancreatitis. Also seek immediate medical help for pain in the upper right abdomen, fever or yellowing of the skin or eyes, which could be a gallbladder problem, or for signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.1

Do not assume stomach pain is down to your injection site. If it is severe or persistent, stop using Mounjaro and get medical help straight away. You can also report any suspected side effect through the MHRA Yellow Card scheme.19

Mounjaro is a prescription-only medicine, supplied after an online consultation reviewed by a GPhC-registered prescriber, and it is used alongside a reduced-calorie diet and increased physical activity. Individual results vary. This page is information, not a substitute for medical advice. If you are exploring treatment more broadly, our weight loss clinic explains how the service works.

Your questions, answered

Mounjaro price UK — frequently asked questions

Answers to common questions about Mounjaro prices, NHS access and ongoing treatment.

Ask our pharmacy team
Is Mounjaro more effective in the stomach or the thigh?
Nothing shows Mounjaro works better in either site. The trial results, such as SURMOUNT-1 averages, are reported for groups of people rather than by injection site. Choose a site permitted in your pen's patient leaflet based on comfort and reach, and rotate it each week.
Where is the best place to get a Mounjaro shot?
The best place is a site named in your pen's patient leaflet that you can reach and see easily, and that is free of bruising or tenderness. No site is shown to give stronger results. Rotating to a fresh spot each week matters more than any single location.
Does it hurt more to inject in the stomach or thigh?
Letting the pen warm briefly out of the fridge and rotating sites each week reduces stinging. If a spot stays sore or lumpy, move to a fresh permitted area.
Should I pinch my stomach when injecting Mounjaro?
Follow the instructions for use that come with your KwikPen, as they say whether to pinch or gather the skin and how to hold the pen. If it is unclear, ask your clinical team.
What happens if you inject too close to the belly button?
Keep injections in the stomach area at least 5cm from the belly button. If you do inject too close, watch for soreness or irritation and use a better area next week. Contact your team if a reaction spreads, is very painful or lasts more than a week.
Where not to inject in the thigh?
Avoid skin that is bruised, tender, scarred, hard or irritated, and avoid spots that rub against clothing. Choose a flat, easy-to-reach area as your leaflet describes. Move to a different spot each week so the same patch of skin is not used repeatedly.
Do you pinch skin giving a subcutaneous injection?
It depends on the device and the site, so follow the instructions supplied with your pen rather than a general rule. The Mounjaro KwikPen's instructions for use are written for that pen. If you are unsure at your first injection, ask your prescribing team for guidance.
