03 · NHS eligibility explained

Who qualifies for Mounjaro on the NHS?

The earliest NHS group is people with the highest clinical need. The rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).6

Beyond that opening stage, NICE recommends Mounjaro for adults with a BMI of at least 35 plus at least one weight-related condition.4 Full rollout is expected to take years.

Meeting the criteria does not give you an automatic right to an appointment on a particular date.

The same medicine is prescribed for type 2 diabetes under separate NICE guidance, so an NHS prescription for diabetes is a different pathway from weight management.8 If you are unsure which applies to you, ask your GP which pathway they are referring to.

04 · Ethnic-background adjustment

Do BMI thresholds change for some backgrounds?

Yes. People from South Asian, Chinese, other Asian, Middle Eastern, Black African or African-Caribbean backgrounds are at higher risk of weight- related health problems at a lower BMI, so clinical thresholds are usually reduced by 2.5 kg/m² for these groups.4

If you are from one of these backgrounds, tell your GP or prescriber so your BMI is read against the adjusted threshold.

Your own BMI is simple to work out: weight in kilograms divided by height in metres squared. Your GP practice can measure it accurately, and it is worth asking them to record it so it is on file when the rollout reaches your group.

05 · How to ask your GP

How to get weight loss help on the NHS

Start with your GP practice and ask what is currently offered locally. NHS England describes weight-management injections as rolling out in stages, so what your practice can do depends on where your area has reached.6 Weight-loss injections also sit alongside other NHS support, which we cover in are weight loss injections available on the NHS.

Before the appointment, it helps to have these ready:

  • Your recent weight, height and BMI, with the date they were measured.
  • A list of weight-related conditions you have been diagnosed with, such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease or prediabetes.
  • Every medicine you take, including over-the-counter products.
  • Any history of pancreatitis, gallbladder problems or thyroid cancer in you or a close relative.

Ask three direct questions. Does my practice or local service offer Mounjaro for weight management yet? Which group am I in? What is the expected wait, and what support is offered in the meantime? Write down the answers so you can compare them later.

06 · Licence versus NHS rules

Why the licence is wider than NHS access

The licence describes who the medicine is approved for; NHS criteria describe who the NHS funds, and they are deliberately narrower during a phased rollout. Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.1

It must be used alongside a reduced-calorie diet and increased physical activity. A prescriber will assess your BMI, medical history and current medicines before deciding whether it is suitable for you.11

So you can be clinically suitable under the licence without yet qualifying for an NHS prescription. That gap is not a judgement that you do not need treatment.

07 · What Mounjaro is and does

What Mounjaro is, and what GLP-1 medicines are used for

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1

This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic).1

08 · What results to expect

What trial results show, and what they do not

The main trial for Mounjaro in weight management is SURMOUNT-1. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2

These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 Trial averages do not predict your own result, and the same applies whether Mounjaro is prescribed on the NHS or privately.

The medicine supports a reduced-calorie diet; it does not replace one.

09 · Safety and who should not take it

Who should not take Mounjaro

Mounjaro is not suitable for everyone. You should not take it if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive.13 Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.

Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems. Always share your full medical history and medicine list during consultation.13

On thyroid safety: in rodent studies tirzepatide caused thyroid C-cell tumours; it is not known whether this applies to humans, and no increase has been shown in human trials. As a precaution, tell your prescriber if you or a close relative have had medullary thyroid cancer or the inherited condition MEN 2 — they will weigh this in your assessment. Routine thyroid monitoring is not required for most people.1

These checks apply on the NHS and privately. Serious side effects are uncommon but need urgent attention. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you notice any other new or lasting symptom, tell your prescriber and report it through the Yellow Card scheme.

10 · The private route

If you do not yet qualify on the NHS

A regulated private prescription is the other route, and it follows the licence rather than the NHS rollout stage. Mounjaro is a prescription-only medicine, supplied only after an online consultation reviewed by a GPhC-registered prescriber. It is used alongside a reduced-calorie diet and increased physical activity. Individual results vary.

Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full. If you ever need to stop or switch treatment, tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1

Scotland, Wales and Northern Ireland

If you are prescribed Mounjaro by any route, store unopened pens in the fridge at 2–8°C until first use; they keep until the printed expiry date. Once in use, a KwikPen can be kept out of the fridge at a temperature not above 30°C for up to 30 days, and must then be thrown away. Follow the patient information leaflet for the rest.13
