How to check an online pharmacy is legitimate

The most reliable test is the GPhC register, which lists every registered pharmacy in Great Britain. Take the registration number from the website footer and search for it on the GPhC register entry for our pharmacy (9012878).12 Check that the name and website address match what you are looking at. A number that does not appear, or belongs to a different business, is a reason to stop.

The MHRA advice is simple: only buy from a pharmacy you can verify on the GPhC register, never from social media, beauty salons or unverified websites.7 That rule covers most of the risk. Pretty branding, reviews on the seller's own site and a professional logo prove nothing, because anyone can build them in an afternoon.

Next, look at the process. A legitimate pharmacy asks about your health, your medicines and your weight-related conditions before it supplies a prescription-only medicine. Genuine treatment always involves a prescription and a clinical consultation.7 If a seller offers to supply without a consultation or prescription, it is not following UK rules, and you cannot rely on what is in the pen.

05 · Fake pens and alerts

What the MHRA has found: fake weight-loss pens

Falsified pens are not a theoretical worry. The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified.7 Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.7 Insulin in a pen you believe holds a weight-loss medicine is dangerous, especially for someone who does not have diabetes.

This is why the wording of an advert matters less than the route the pen took to reach you. Genuine Mounjaro is manufactured by Eli Lilly. Genuine UK stock is supplied through Lilly's official distribution to licensed pharmacies, and every pen carries a batch number and expiry date that can be checked.13 Our supply is official LillyDirect supply, direct from Eli Lilly's UK distribution.

Common routes for fake or unlicensed products tend to share a pattern. They are sold outside a pharmacy, they skip the consultation, and they lean on a very low price or dramatic weight-loss claims. Sellers may describe a product as a research chemical, a compounded peptide or a cheaper alternative. None of those descriptions is a licensed UK medicine route, and none can be checked against the GPhC register.

Supplements are a different problem. Products marketed as natural alternatives to prescription injections are low-cost supplements with no clinical oversight, and dosing is not standardised. They are not a substitute for a licensed medicine, and they do not carry the safety review that a prescriber consultation does.

What should you do if you think you have bought a fake pen? Do not inject it. Keep the packaging and the pen, and contact the seller if you can. 06 · Checking your pen

How to check your pen is genuine and still good

A genuine UK Mounjaro pen comes from a registered pharmacy with a batch number and expiry date, arrives cold-chain packaged, and the solution inside is clear and colourless.13 Run through these checks the moment your parcel arrives, before you inject.

Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, or has been above 30°C. In use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away.13 Store unopened pens in the fridge until first use; they keep until the printed expiry date. If you are unsure about an unopened pen that has been out of the fridge, check the patient information leaflet and ask the pharmacy rather than guessing.

Look at the packaging as well. Confirm the batch number and expiry date are present on the pen and the box. Check the label shows the strength you were prescribed. Use the pen as designed. Talk to your clinical team rather than self-adjusting.

If supply or packaging ever seems wrong, check the pharmacy on the GPhC register and contact the pharmacy before injecting.13 A responsible pharmacy will want to hear about it and will not push you to use a pen you are uneasy about.

07 · Amazon, social media and shops

Amazon, social media, salons and high-street shops

Is GLP-1 safe from Amazon? Prescription-only injectables should never be bought from a general marketplace. Mounjaro is a prescription-only medicine, supplied after a consultation reviewed by a GPhC-registered prescriber.1 A marketplace listing that lets you add a pen to a basket skips that step entirely. Treat such listings, and anything that looks like a GLP-1 on a general shopping site, as unsafe. Look-alike products and supplements can also appear under similar names, and they are not the medicine.

The MHRA's advice is explicit: never buy from social media, beauty salons or unverified websites.7 Social media sellers rarely show a registration number, cannot be checked on the GPhC register, and cannot show where the pen came from. A salon offering injections as a treatment is not acting as a registered pharmacy, however friendly the setting.

What about the price signal? Across the UK pharmacies we track, the gap between providers for the same dose ranges from £27 to £41 (checked 10 October 2026). That is a real difference worth comparing, but price should only be compared among sellers who pass the safety checks. A pen that is much cheaper than every registered pharmacy is not a bargain; it is a risk.

Finally, ask a basic question of any seller: what happens if something is wrong? A registered pharmacy has a support team, a returns policy and a prescribing team reachable after you order. A seller on a chat app does not.

08 · Who can safely take it

Can anyone take GLP-1 for weight loss?

No. Safe buying also means the medicine is right for you. Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.1 It must be used alongside a reduced-calorie diet and increased physical activity. A prescriber will assess your BMI, medical history and current medicines before deciding whether it is suitable.11

Some people should not take it. You should not take Mounjaro if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive.13 Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems.13 Always share your full medical history and medicine list during consultation.

On thyroid safety, rodent studies showed tirzepatide caused thyroid C-cell tumours. It is not known whether this applies to humans, and no increase has been shown in human trials.1 As a precaution, tell your prescriber if you or a close relative have had medullary thyroid cancer or the inherited condition MEN 2. Routine thyroid monitoring is not required for most people.

This is a strong reason not to buy from an unregulated source. A seller with no consultation cannot know whether the medicine is unsafe for you. The questions a prescriber asks are not paperwork; they are the safety check. Which GLP-1 is best for weight loss online? There is no single answer for everyone. Mounjaro and Wegovy are both licensed once-weekly injections, and the right choice depends on your health, your medicines and your prescriber's judgement. 09 · NHS versus private

NHS or private: where you can get it safely

Both routes are safe because both involve a clinical assessment. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

NHS access to Wegovy runs through specialist weight-management services; ask your GP. If you are unsure whether you qualify on the NHS, your GP is the place to start, and the NHS pages explain the medicine in plain language.5

A private route is not a lesser one. A regulated online pharmacy supplies the same prescription medicine, through a prescriber's review and a registered pharmacy. The difference is the cost and the speed. On the NHS, a prescription charge in England is £9.90 per item where it applies, but the issue is eligibility and timing, not the charge. Privately, you pay for the pen.

10 · Safety of the medicine

Side effects and safety once you have the pen

A genuine pen is only half of safe use; you also need to know what to watch for. Serious side effects are uncommon but need urgent attention.13 Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting, which could be pancreatitis. Also seek immediate medical help for pain in the upper right abdomen, fever or yellowing of the skin or eyes, which could be a gallbladder problem, and for signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.

If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar: shakiness, sweating and confusion. Your prescriber may need to adjust those medicines. Tell the prescriber about every medicine you take at consultation; an unregulated seller will not ask.

Mounjaro is not known to damage kidneys or liver directly, and no dose adjustment is needed for mild to moderate kidney or liver impairment.1 The practical risk is indirect: severe vomiting or diarrhoea can cause dehydration, which can strain the kidneys. Keep fluids up and get medical help straight away if you cannot keep fluids down. People with severe impairment need individual prescriber assessment.

On mood, regulators have looked specifically at whether GLP-1 medicines are linked to low mood or suicidal thoughts. The EMA's safety committee reviewed the evidence and concluded in 2024 that it does not support a causal link, and the MHRA keeps this under continuous review.17 Even so, any big change in weight, appetite and routine can affect how you feel. Tell your prescriber about any history of depression, and seek help promptly if your mood drops or you have distressing thoughts during treatment.

If you have thoughts of harming yourself or ending your life, get help straight away. Call 999 or go to A&E if you are in immediate danger. In England you can call NHS 111 and choose the mental health option, any time. You can also call Samaritans free on 116 123, day or night. Tell your prescriber about any change in your mood while taking Mounjaro.

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4 to 5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 That is one more reason to avoid a pen of unknown origin: you cannot undo an injection.

11 · What to expect from treatment

What the medicine does, and what to expect

Knowing what the genuine medicine does helps you spot a seller promising something unrealistic. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1.1 Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.

Mounjaro is a once-weekly injectable made by Eli Lilly, licensed in the UK for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose KwikPen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg. Each pen is designed to deliver four weekly doses.

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 For someone starting at 100 kg, 20.9% is about 21 kg. These are trial averages, individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 Trial averages do not predict your own result.

In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.2 These are trial-average secondary outcomes, not promises.

A trustworthy seller describes results this carefully, with the trial name and a note that results vary.
