Fatigue on tirzepatide usually reflects a mix of the body adjusting and simply eating less.1 Both parts make sense once you look at what the medicine does.

Tirzepatide activates two natural gut-hormone receptors, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals.1

That is the intended effect, and it supports a reduced-calorie diet. The side of it nobody advertises is that a body used to a certain intake is suddenly running on less. Energy can dip while it adapts, especially if meals become small, skipped or low in protein.

Digestive effects add to this. Nausea, vomiting and diarrhoea are the most common side effects, and they make it harder to eat and drink enough. The most reliable steps are the unglamorous ones: balanced meals, enough fluid and gentle activity.1

Keep meals balanced, with protein especially. Smaller portions are expected, but each one still needs to count. Protein and overall nutrition matter, which is another reason the reduced-calorie diet should stay balanced.1

Stay hydrated. Regular water through the day helps with tiredness, and also with constipation and the general wellbeing of your gut.1

Keep gentle activity going. A short walk can feel like the last thing you want when tired, but moving a little is part of the advice. It is also part of the programme this medicine is licensed alongside.1

Manage the digestive effects. Eat smaller meals slowly, stop at the first feeling of fullness, favour plain lower-fat foods while adjusting, and avoid lying down straight after eating.1 Less nausea means you eat and drink better, which helps your energy.

Do not skip meals to speed things up. Going faster is not better. Loss pace guided by your prescriber, rather than maximum speed, protects how you feel.1

Which drink gets rid of fatigue? Do energy drinks help?

No drink removes fatigue on Mounjaro. Stay hydrated and keep meals balanced.1

If you take insulin or a sulfonylurea alongside Mounjaro, low blood sugar is a different matter. Watch for shakiness, sweating and confusion, and speak to your prescriber, who may need to adjust those medicines.1 Do not treat this on your own with guesswork.

Plain water through the day remains the simplest, most useful drink. If you struggle to drink enough because of nausea, that is a reason to contact your clinical team, not to reach for a quick fix.

08 · Other causes of tiredness

When tiredness may not be the medicine

Tiredness has many possible causes, and not all of them are Mounjaro. If you feel exhausted all the time, speak to your prescriber or GP.

Tiredness can have causes unrelated to the medicine; raise it with your prescriber or GP.

Obstructive sleep apnoea is a recognised weight-related condition, and in the SURMOUNT-OSA trials tirzepatide substantially reduced its severity in adults with obesity. If you use CPAP, continue it and keep your sleep clinic informed.16

Any big change in weight, appetite and routine can affect how you feel. Regulators have looked specifically at whether GLP-1 medicines are linked to low mood or suicidal thoughts: the EMA's safety committee reviewed the evidence and concluded in 2024 that it does not support a causal link, and the MHRA keeps this under continuous review. Tell your prescriber about any history of depression, and seek help promptly if your mood drops or you have distressing thoughts during treatment.17

If nausea or vomiting is persistent or you cannot keep fluids down, contact your clinical team — dehydration needs prompt attention.1

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.1 Tiredness on its own is not usually one of these, but if it comes with any of them, get medical help straight away.
