What the impact means for you, and what to do next
The impact of weight loss jabs on you depends on three things: whether you are eligible, which dose you settle on, and whether you can sustain the diet and activity changes alongside it. Mounjaro is licensed for adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used with a reduced-calorie diet and more physical activity.1
If you meet those criteria, the practical question is how to get it. NHS access exists but is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than wait. How weight loss jabs work in the body
Weight loss jabs work by changing appetite signals, not by burning fat directly. Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes.1
Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. That is the central point about impact: the medicine makes the diet easier to follow, it does not replace it.
This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic). Wegovy is dosed on its own ladder of 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once weekly, stepping up after at least four weeks and reaching the 2.4mg maintenance dose from week 17.18 Liraglutide, sold as Saxenda and in products such as Erly, is a daily injection, unlike the once-weekly Mounjaro and Wegovy.
The jab comes as a pre-filled multi-dose pen, the KwikPen, in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg. Each pen contains four weekly doses. The practical consequence is a very low-effort routine: one injection, once a week, on the same day, at any time of day, with or without food.1
People sometimes ask whether a jab is a shortcut. It is better described as a tool that changes hunger. Because it is licensed alongside a reduced-calorie diet and increased physical activity, the best-supported impact comes from combining all three, which is exactly how the main trial was run. The same clinical team that prescribes it reviews how you are responding, adjusts the dose, and supports you with side effects, which is part of why an assessment by a GPhC-registered prescriber matters before anything is supplied.
Finally, the mechanism explains the side-effect profile. Slower stomach emptying and altered appetite are the reason that early nausea and fullness are common in the first days after each injection, and why the dose is raised gradually instead of all at once.
05 · Weight impact
The impact on body weight: what SURMOUNT-1 found
In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.
The spread matters as much as the average. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 So most people on treatment lost a meaningful amount, but the size of the loss differs from person to person.
To make that concrete, here is a worked example. For someone starting at 100 kg, 15% is about 15 kg, 19.5% is about 19.5 kg and 20.9% is about 21 kg. Those are straight arithmetic from the trial percentages and not a forecast for you. Your starting weight, dose, diet, activity and how you tolerate the medicine all shape the outcome, and trial averages do not predict your own result.
A number from one trial should not be set against another trial's number as if they were head to head.
The dose also explains why "impact" differs between people. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better. Your prescriber guides the pace based on your response and tolerance.
Lastly, the trial measured weight at 72 weeks, which is well over a year. That timescale is itself the main message: the impact builds slowly, month by month, and it is measured against what lifestyle changes alone achieve.
06 · Realistic pace
How fast do the effects arrive?
Weight loss on Mounjaro is gradual by design, so think in months, not days. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month.1
Experiences in the first weeks differ widely. Some people notice appetite change and early loss in week one, while others feel little until a higher dose, and both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1
The dose steps up gradually. Everyone starts on 2.5mg once a week, which is about letting the body adjust, not about weight loss itself. After at least four weeks the dose rises to 5mg, and can then step up by 2.5mg at minimum four-week intervals (7.5mg, 10mg, 12.5mg) to a maximum of 15mg weekly, depending on how you respond and tolerate it.1
Progress is formally reviewed. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1 That review is a safeguard, not a failure: it stops people paying for and taking something that is not helping them.
Practical habits make the trend easier to read. Weigh at the same time of day and judge by the direction over several weeks. Keep a note of side effects and appetite alongside the numbers, because your prescriber will want both. 07 · Side effects and wellbeing
Side effects, mood and libido
The commonest early effects are mild nausea, altered appetite and tiredness in the first days after each injection, and they usually settle.1
Tell your prescriber about every medicine you take, including antidepressants, at the consultation and at each review, because a prescriber assesses your medical history and current medicines before deciding whether treatment is suitable.1 Tell your prescriber about any change in your mood. Regulators have looked at whether GLP-1 medicines are linked to low mood or suicidal thoughts: the EMA's safety committee reviewed the evidence and concluded in 2024 that it does not support a causal link, and the MHRA keeps this under continuous review. Any big change in weight, appetite and routine can affect how you feel, so tell your prescriber about any history of depression, and seek help promptly if your mood drops or you have distressing thoughts during treatment. If you have thoughts of harming yourself or ending your life, get help straight away. Call 999 or go to A&E if you are in immediate danger. In England you can call NHS 111 and choose the mental health option, any time. You can also call Samaritans free on 116 123, day or night.
If you notice any new symptom you think may be linked to treatment, discuss it with your prescriber. Reduced libido is not listed as a side effect in the UK Mounjaro patient leaflet; tell your prescriber about any new or lasting symptom. You can also report it yourself through the MHRA Yellow Card scheme, which exists to collect exactly this kind of information.19 The most common side effects, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort, and for most people they ease within days to a few weeks.13

08 · Stopping and maintaining
Stopping and maintenance
Stopping a weight loss jab does not remove the drug instantly. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 That timeline helps explain why appetite changes are gradual in both directions.
Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.14 Habits you build while on treatment are the part you keep, so use the early months to build routines you can sustain, such as regular meals, steady activity and a plan for hunger as appetite returns.
The decision to stay on, lower or stop is a clinical one. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and your prescriber can tailor that. The review point is clear: less than 5% loss after 6 months on the highest dose you can tolerate triggers a conversation about continuing, adjusting or stopping.1 Do not stop or change dose on your own because of something you read online.
Missed doses and day changes are part of staying on track. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip it and take the next one on your normal day. Never take two doses within 3 days of each other.1 You can also change your injection day, as long as at least 3 days (72 hours) have passed since your last dose.
Cost planning belongs in the maintenance conversation too. Staying on treatment means a monthly pen, so plan what the yearly commitment looks like. The standard monthly price for a 5mg pen is £218.99, and every later order is £25 off the standard price.
09 · Eligibility and NHS access
Who can have a weight loss jab: licence, NHS and private routes
The licence sets who may be treated. The NHS route is real but phased. Mounjaro is available on the NHS, but access is being phased in over several years. 11 · Using it day to day
Living with the jab: injecting, storage and travel
The weekly routine is quick. Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food. Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.1 Follow the instructions that come with the pen, and your needles, sharps bin and guidance are included with every order.
There is no "best" time of day. Mounjaro works the same morning or evening. What matters is the same day each week. Many people choose a weekend morning so early side effects fall on days off.1
The pen is set by turning the dose knob until the dose indicator shows the correct dose in the window.
Storage is straightforward. Keep pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, which is useful for short trips, after which it should be discarded. Keep the cap on to protect it from light, and never use a pen that has been frozen.1
Travelling is manageable. You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. Most holidays need no special arrangements; for hot climates, use an insulated pouch, and check destination rules for travelling with prescription injectables before you fly. Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it, rather than relying on a remembered day limit.1

12 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
