GLP-1 stands for glucagon-like peptide-1. It is a gut hormone, which means your digestive system releases it as part of the conversation between your gut and your brain about food.

The key point is that GLP-1 is one of the natural gut-hormone receptors that medicines can activate. When that signalling is engaged, it helps regulate appetite, slows how quickly the stomach empties and improves the way the body responds to insulin.1 Those three effects are the reason the hormone attracts so much attention in both diabetes care and weight management.

Appetite regulation is the part most people notice. A slower-emptying stomach and better-regulated hunger signals can mean feeling full sooner and less hungry between meals. That does not make eating effortless, and it does not replace the need for a reduced-calorie diet and more physical activity. It simply makes those habits easier for many people to keep up.

The insulin effect explains why GLP-1 medicines were first developed for diabetes. Better insulin response is useful when blood sugar control is the goal. In the UK, Mounjaro is licensed both for weight management in adults and for type 2 diabetes.1

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 The strengths are stepped, so most people start low and move up with prescriber guidance.

What makes it distinctive is that it does not rely on GLP-1 alone. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic).1

A question we hear often is whether a dual-action medicine is automatically better. That is not something to assume from the mechanism alone. Both tirzepatide and semaglutide are effective, licensed treatments; which suits you depends on your health profile, tolerance and prescriber advice, not the headline number alone. If you are choosing between them, the practical factors are your suitability, your prescriber's advice and how you tolerate the medicine.

How long does it stay in the body? Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system.

06 · Other GLP-1 medicines

Other GLP-1 medicines you may hear about

Mounjaro is not the only medicine that acts on the GLP-1 pathway. Semaglutide is sold as Ozempic and Wegovy, and liraglutide is sold as Saxenda and in products such as Erly. Each has its own licence, dosing pattern and prescriber considerations, so it helps to know the basics of what separates them.

Wegovy (semaglutide) is a once-weekly injection. Its pens are 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg, and titration steps up after at least four weeks, reaching the 2.4mg maintenance dose from week 17. Liraglutide is different in an important practical way: it is supplied at a concentration of 6mg per millilitre and is a daily injection, unlike once-weekly Mounjaro and Wegovy. For some people the daily routine is a downside, for others it is simply a habit.

Oral options also exist. Wegovy tablets and the tablet orforglipron (Foundayo) are covered by their own product information.

Which is the "safest" GLP-1? There is no single answer. Each licensed medicine has a documented side-effect profile, and suitability depends on the individual: other medicines you take, your medical history, whether you could become pregnant, and how you responded to earlier treatment. The safest choice is the one a prescriber has reviewed against your details, supplied from a regulated pharmacy, with a plan for follow-up.

What about switching between them? Switching between medicines is decided by a prescriber, who also decides which dose to start on; Wegovy's licensed dose ladder starts at 0.25mg. 07 · Trial evidence

What the trial evidence says about tirzepatide

Mounjaro's weight-management evidence comes from the SURMOUNT-1 trial. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2

These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 Trial averages do not predict your own result.

The same trial reported how many people reached particular thresholds: at 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2

To make a percentage feel real, take a worked example: for someone starting at 100 kg, 20.9% is about 21 kg.

SURMOUNT-5 is the first large head-to-head trial of tirzepatide and semaglutide for weight loss.15 Other trials, such as the semaglutide STEP 1 study or liraglutide studies, tested different people under different conditions, so their numbers should not be lined up against SURMOUNT-1 as a ranking.

Finally, think about what a trial can and cannot tell you. It tells you what happened on average to a group of people who were closely monitored, who took the medicine as directed and who followed lifestyle advice. It does not tell you how you will feel on the first few doses, how your appetite will change or whether you will find the routine easy. 08 · Natural and supplements

Can you raise GLP-1 naturally? Supplements, cinnamon and food

Cinnamon, berberine, fibre products and so-called "nature's Ozempic" supplements are not medicines, are not regulated for this use and have no comparable trial evidence.

Supplements are typically low-cost and available cheaply without assessment. That cheapness is part of the problem rather than a benefit: there is no clinical oversight, no review of whether the product suits you and no monitoring if something goes wrong. Dosing is not standardised, so two products with the same label can differ meaningfully. Dramatic weight-loss claims in marketing are a warning sign, not a selling point.

That does not mean food is irrelevant. A varied diet with plenty of protein and fibre-rich plant foods supports appetite and general health, and regular activity helps whatever else you do. Be especially wary of products that borrow the language of prescription medicines. Anyone offering to supply a GLP-1 medicine without a consultation or prescription is not operating within UK rules, and counterfeit injectable products are a real risk. The MHRA publishes counterfeit medicine alerts, and you can report suspected problems with a medicine through the Yellow Card scheme.7

10 · Safety and storage

Safety, the patient leaflet and storing your pen

Every prescription GLP-1 medicine comes with a patient information leaflet, and for Mounjaro it is published on the electronic medicines compendium.13 The leaflet covers possible side effects, who should not use the medicine, how to inject and how to store the pen. Mounjaro is not suitable for everyone. You should not take it if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems. Always share your full medical history and medicine list during consultation.

If you notice a symptom that is not listed, do not assume it is related or unrelated: discuss any new symptom with your prescriber, and report suspected side effects through the Yellow Card scheme.19

Two interaction points are worth knowing. Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 And if you take insulin or sulfonylureas, doses may need adjusting because of the risk of low blood sugar. That is a conversation to have with your prescriber, not a reason to change anything yourself.

Storage is practical and worth getting right. Keep unopened pens in the fridge, at 2–8°C, until first use; they keep until the printed expiry date. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Do not use a pen that has been above 30°C or has been frozen.1 If an unopened pen has been left out of the fridge, check the patient information leaflet and contact the pharmacy rather than guessing.

Follow the pen's instructions for injecting, and use the sharps bin supplied with your order. If something arrives faulty, damaged or the wrong item, contact us within three days, as dispensed prescription medicines cannot otherwise be returned.

Tirzepatide's weight-management evidence includes SURMOUNT-1 over 72 weeks.2 The SURMOUNT-4 trial looked at continued treatment for maintenance of weight reduction, a reminder that this is a longer-term approach rather than a quick fix.14 The semaglutide STEP 1 trial is the equivalent landmark for Wegovy.

The practical lesson from the research is about expectations. These medicines work alongside a reduced-calorie diet and increased physical activity. Results vary from person to person. Some people respond strongly, some modestly, and tolerability differs. A good treatment plan includes regular reviews so that dose and approach can be adjusted.

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1 Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.
