How to Use an Insulin Syringe for Mounjaro (Honest Guide, October 2026)

You should not draw Mounjaro out of a KwikPen with an insulin syringe: the pen is built to dial and deliver a full weekly dose through its own needle, and syringe withdrawal is not licensed or supported by the pen's design.1 Insulin syringes are marked in units, while Mounjaro is dosed in mg, so there is no safe conversion. Current as of October 2026.

A quick overview

What to know
before you start.

  • Mounjaro is dosed in mg through the KwikPen dial; insulin syringe units measure a different product and do not convert to mg.
  • Withdrawing medicine from the pen with a syringe is outside the licence, with unknown dosing accuracy and no safety data.
  • If you also use insulin, tell your prescriber: your insulin dose may need adjusting because of low blood sugar risk.
  • Needles, a sharps bin and injection guidance are included with every order.
In this guide

01 · The core answer

Should you use an insulin syringe with Mounjaro?

No. Mounjaro comes as a pre-filled multi-dose KwikPen, and you set the dose by turning the knob until the correct dose shows in the window.1 The pen does not need a separate syringe, and the licensed method is the pen with its supplied needle.

Altering doses outside the licence means unknown dosing accuracy and no safety data.1 Our guides on the insulin syringe question and what syringe to use cover the same position in more detail.

02 · Your next step

What to do instead

If the pen is awkward, the answer is to ask, not improvise. Tell your prescriber about needle fear, dexterity problems or a pen that will not dial.

03 · Why mg and units differ

Why mg do not convert to syringe units

An insulin syringe is marked in units for insulin. Mounjaro is measured in milligrams of tirzepatide, and the pen delivers fixed strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

Our pages on 2.5mg in an insulin syringe and insulin needles explain the safety position further.

04 · Pen dial and clicks

Using the KwikPen dial correctly

You set the KwikPen by turning the dose knob until the indicator shows the correct dose in the window.

05 · Using Mounjaro with insulin

Can you use Mounjaro with insulin?

Some people do, under supervision. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines. They must be reviewed by whoever manages your diabetes. Do not change insulin doses yourself, and tell the prescriber about every medicine you use before starting.

06 · Injecting step by step

How to inject Mounjaro safely

Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food. Licensed sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.1 For the pen's own needle, follow the instructions that come in the box. Our guide to injecting with a syringe has more on this.

07 · Storage and missed doses

Storage and missed doses

Keep pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can stay out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Do not use a pen that has been above 30°C or frozen. Keep the cap on to protect it from light.1

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time. After that, skip it and take the next on your normal day, never two doses within 3 days of each other.1 If you are using the contraceptive pill, Mounjaro can reduce how well it is absorbed: switch to a non-oral method, or add a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.

Your questions, answered

Mounjaro price UK — frequently asked questions

Answers to common questions about Mounjaro prices, NHS access and ongoing treatment.

Ask our pharmacy team
How do I use an insulin syringe for Mounjaro?
You should not use one. The Mounjaro KwikPen is designed to be dialled to the dose shown in the window and injected with its own needle. Drawing medicine out with a syringe is outside the licence, with unknown dosing accuracy and no safety data. Speak to your prescriber if the pen is causing difficulty.
Can you use Mounjaro with insulin?
Some people do, under medical supervision. If you take insulin or a sulfonylurea, your doses may need adjusting because of the risk of low blood sugar. Tell your prescriber about all your medicines and never change insulin doses yourself. The two should never be mixed in one syringe.
What insulin syringe or needle should I use for Mounjaro?
Use the pen needle supplied with your KwikPen, following the leaflet. We do not recommend insulin syringes, because the pen is not designed for syringe withdrawal. Needles, a sharps bin and injection guidance are included with every order from us.
Is a dose in mg the same as units on an insulin syringe?
No. Units measure insulin, while Mounjaro is dosed in milligrams of tirzepatide.
Can I count clicks on the pen to find my dose?
No.
What if I am afraid of the pen needle?
Tell your prescriber. They can talk through options with you. Our team is available Monday–Friday 9am–5pm on 0333 207 0413 or at support@everydaymeds.co.uk.
