Do you need to mix 15mg tirzepatide?

No. Mounjaro is the brand name for tirzepatide, a once-weekly injectable made by Eli Lilly, supplied as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 The medicine is already prepared inside the pen, so you never draw it up or dilute it.

What to do instead of mixing

Your decision is really between a prescribed pen and an unlicensed vial. A prescribed pen is dispensed by a registered pharmacy after a consultation reviewed by a GPhC-registered prescriber. You will not start on 15mg. 03 · Using the 15mg pen

How the 15mg KwikPen works in practice

The KwikPen is set by turning the dose knob until the dose indicator shows the correct dose in the window. You confirm the dose by the number in the window, never by counting clicks.13 Each pen contains four weekly doses, and the pen will not let you dial past the medicine left inside. If the window will not turn to a full dose, the pen does not contain enough for one and should not be used for a partial injection.13

Why unlicensed vials are a problem

Powder sold as tirzepatide is not the licensed UK medicine. It has no assured sterility, no assured potency and no prescriber oversight, and home mixing adds a further chance of error.

The MHRA publishes counterfeit medicine alerts and runs the Yellow Card scheme.7 Sellers offering to supply without a consultation or prescription should be treated with suspicion. Report any suspected side effect through Yellow Card.19

How do I mix 15mg tirzepatide?
You do not. Licensed Mounjaro 15mg is a pre-filled KwikPen, so there is nothing to reconstitute or dilute. You dial the dose in the pen's window and inject under the skin once a week. Powder vials sold online are unlicensed, so speak to a pharmacist or prescriber before using one.
How much bacteriostatic water do you mix with peptides?
There is no safe volume to give. Prescription Mounjaro never needs bacteriostatic water, and advice for unlicensed vials cannot be checked for safety. Wrong dilution means wrong dosing. If you have a vial, speak to a pharmacist or prescriber before using it.
What do you mix your tirzepatide with?
Nothing. The UK KwikPen contains ready-to-inject medicine. Adding water, diluents or other substances to a prescribed pen is not licensed and could affect sterility and dose accuracy. Follow the patient information leaflet supplied with your pen.
What is the microdosing chart for tirzepatide?
There is no approved microdosing chart. Microdosing is not licensed, not supported by the pen's design and not endorsed by any UK regulator. The licensed ladder already starts low at 2.5mg, and your prescriber can hold a dose longer or step down within the licence.
What is the bad side of tirzepatide?
The commonest early effects are mild nausea, altered appetite and tiredness in the first days after each injection, and these usually settle. Serious side effects are uncommon but need urgent attention. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. Tell your prescriber about any other new or lasting symptom. You can also report suspected side effects through the MHRA Yellow Card scheme.
What not to do on tirzepatide?
Do not alter doses outside the licence, mix or dilute the pen, or use a pen that has been frozen. If you miss a dose, take it as soon as you can within 4 days (96 hours); if more than 4 days have passed, skip it and take the next dose on your normal day — never take two doses within 3 days of each other. Follow the dial window, store pens in the fridge, and keep in touch with your prescriber.
Is tirzepatide stronger than Ozempic?
They work differently rather than one simply being stronger. Tirzepatide activates two gut-hormone receptors, GIP and GLP-1, whereas semaglutide (Wegovy/Ozempic) is single-action. Ozempic is semaglutide licensed for type 2 diabetes — it is not a UK weight-loss medicine; the licensed weight-management options are Mounjaro (tirzepatide) and Wegovy (semaglutide 2.4mg). Which suits you depends on your health, tolerance and your prescriber's advice.
