Do you inject Mounjaro with a syringe in the UK?

No. Mounjaro is a once-weekly prescription medicine that comes as a pre-filled multi-dose KwikPen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 The dose is set on the pen and the needle is attached to the pen. A separate syringe is not needed.

Be cautious about any video or seller showing a vial of tirzepatide drawn into a syringe. Dose accuracy cannot be checked that way, and the contents of an unregulated vial cannot be verified. Genuine UK supply is dispensed after a consultation reviewed by a GPhC-registered prescriber.7

03 · Injecting with the pen

How the pen injection works

Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food. Rotate the site with each dose. Each KwikPen contains four weekly doses.1

You set the dose by turning the dose knob until the indicator shows the correct dose in the window. You confirm the dose by the number in the window, never by counting clicks. The pen will not let you dial past the medicine left inside. If the window will not turn to a full dose, the pen does not contain enough for one and should not be used for a partial injection.13 Follow the pen's instructions for attaching the needle and completing the injection.

Choosing an injection site

Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Units, millilitres and doses

The pen is dosed in milligrams, shown in the window, so syringe markings such as units do not translate to a Mounjaro dose. Do not try to convert between them. Liraglutide, a different daily medicine, has its own concentration of 6mg/ml, and its figures must never be applied to Mounjaro.

Missed doses and storage

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.1

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded. Keep the pen cap on to protect it from light.13

How do I use Mounjaro with a syringe?
You do not. The UK Mounjaro KwikPen is pre-filled, so you dial the dose in the window and inject under the skin once a week. Drawing medicine into a syringe is not the licensed method. Follow the pen's instructions and the patient leaflet, and ask your prescriber or pharmacy if anything is unclear.
How much is 2.5mg of Mounjaro in a syringe?
There is no syringe measure for it, because the pen is dosed in milligrams. You set 2.5mg by turning the dose knob until that number shows in the window. Do not convert to units or millilitres, as that would make your dose inaccurate. Your pharmacy can show you how the pen works.
What do units on a syringe mean for Mounjaro?
Nothing, for the UK pen. Units belong to insulin-style syringes, whereas the Mounjaro KwikPen shows the dose in milligrams in its window. If a seller gives you unit-based instructions, treat it as a warning sign and speak to a pharmacist.
Is it possible to inject Mounjaro wrong?
Yes, mistakes can happen, such as using the wrong site or not completing the injection as the pen instructs. If you are unsure whether a dose went in, do not repeat it; contact your prescriber or pharmacy. If you have taken more Mounjaro than you should, talk to a doctor immediately.
What not to do after injecting Mounjaro?
Do not take an extra dose to make up for a doubt, and never take two doses within 3 days of each other. Do not freeze the pen, and keep the cap on to protect it from light. Dispose of used needles in a sharps bin, and ask your prescriber if you are concerned.
How do I know if my injection went wrong?
Follow the pen's instructions to check the injection completed. If you think some of the dose was lost or you are unsure, do not inject again to compensate. Contact your prescriber or pharmacy for advice. Report any unexpected symptoms to your prescriber and through the Yellow Card scheme. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of the skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.
What happens if I mess up an injection?
Do not repeat the dose straight away. A repeat could mean taking more than intended, and doses must never fall within 3 days of each other. Contact the prescribing team or your pharmacy and describe what happened. If you think you have used more Mounjaro than you should, talk to a doctor immediately.
