01 · The short answer
What actually helps Mounjaro work better
What helps is consistency with the licensed routine, not a trick. Tirzepatide activates two gut-hormone receptors, GIP and GLP-1, which help regulate appetite and slow how quickly the stomach empties, so most people feel fuller sooner and less hungry between meals.1 That fullness is the opening you use: it makes a reduced-calorie diet easier to follow. It does not do the dieting for you.
In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages and individual results vary.2 The comparison group shows that lifestyle matters, and the medicine adds to it.

02 · Where to focus
Which lever matters for you
The right lever depends on where you are in treatment. If you are in the first month, the priority is simply settling into the routine. If you are on the same dose for months, the question is whether a dose review is due. If you have reached your maximum tolerated dose, the question is food, activity and the basics.

03 · Dose and titration
Follow the dose ladder at your own pace
Everyone starts Mounjaro on 2.5mg once a week, and this starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at minimum 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1
Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber guides the right pace.1 Rushing up the ladder is not a way to make the medicine work better, and the licence does not allow steps faster than after at least 4 weeks.

04 · Injection routine
Get the injection routine right
A consistent routine is the easiest thing to get right. Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food. Licensed sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.1
There is no best time of day. Mounjaro works the same morning or evening. Pick a day and time you will remember; many people choose a weekend morning so early side effects fall on days off.1
Set the dose by turning the knob until the correct number shows in the window. Follow the pen's instructions for the injection itself.
05 · Missed doses and storage
Missed doses, injection day changes and storage
If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.1
You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 Move it deliberately and rarely, rather than drifting from week to week.
Storage affects whether the pen delivers what it should. Keep pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Keep the cap on to protect it from light, and never use a pen that has been frozen.13
06 · Diet and activity
Use the reduced appetite for diet and activity
The most useful thing you can do with lower appetite is eat in a way you can keep up. Mounjaro is used alongside a reduced-calorie diet and increased physical activity, and the trial results were achieved in that context.2 Because most people feel fuller sooner and less hungry between meals, a reduced-calorie diet becomes more achievable.1
When progress stalls, the licensed-approach response includes protecting protein and activity, and reviewing food intake honestly, because portions drift back up over time.1 A food diary or the free GLPTrackr companion app can show whether that has happened.
You will see long food lists online: foods to avoid, ideal snacks, whether bananas are fine. No food is banned on Mounjaro, including bananas. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1 If a particular food or meal size makes you feel unwell, note it and raise it with your prescriber or a dietitian, who can tailor advice to you.
07 · Realistic pace
Set a realistic pace
Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month. Some people notice appetite change and early loss in week one; others feel little until a higher dose. Both are normal.1
Three months in, most people are still on the lower rungs of the dose ladder. A sensible 3-month expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation.1 The full 15–21% averages in SURMOUNT-1 were measured at 72 weeks, not 12.2 For a worked example, 20.9% for someone starting at 100 kg is about 21 kg, but trial averages do not predict your own result.
Early loss includes water weight, so judge progress by the trend over weeks, not any single weigh-in. Slower early progress does not predict failure, because the dose you respond to may still be ahead of you.1
08 · Plateaus and fading effect
When Mounjaro seems to stop working
A plateau of a few weeks is normal and expected as the body adapts to a lower weight. Weight loss on Mounjaro is not linear.1 If your appetite feels less suppressed than it did, or you feel it needs to be "made effective again", work through this order:
- Review food intake honestly, including portion drift and drinks.
- Protect protein and activity.
- Check the basics: sleep and alcohol.
- Check your injection routine: same day, correct dose number in the window, pen stored properly and within 30 days (not above 30°C) of first use out of the fridge.
- Discuss with your prescriber whether a dose step-up is appropriate, if you are not yet at maximum.
A plateau is not a reason to double doses or add unlicensed products.1
Treatment response is formally reviewed. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1
09 · Beyond the scales
Measure more than the scales
Weight is one measure of whether treatment is working. In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels. These are trial-average secondary outcomes, not promises.2
That is why prescribers describe weight management as health management. If the scale is flat for a few weeks, track waist size, energy and how clothes fit alongside it, and bring those to your review. 10 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
11 · NHS and support
Support, side effects and access
If you have a new symptom, discuss it with your prescriber, and report suspected side effects through the Yellow Card scheme.19
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first.4 Many people who meet the licensed criteria choose regulated private treatment instead of waiting.
