Food noise is the everyday phrase for constant, intrusive thoughts about food: what to eat next, what you should not eat, or when the next chance will be. It is not a medical diagnosis, and there is no single test for it. People describe it differently, from a background hum to something that crowds out concentration.

That matters because it shapes what works. A hum that rises at 4pm when you have skipped lunch may respond to routine. A constant, distressing preoccupation may need professional support.

It also helps to separate three things that feel alike. Physical hunger builds gradually and eases when you eat. Cravings are specific, often for a particular food, and tied to cues. Food noise is the mental chatter around eating, which can persist even when you are not hungry. Different habits help different layers, so keep a short note for a week on when the thoughts arise and what was happening just before.

07 · Supplements and shortcuts

Supplements, "natural Ozempic" and quick fixes

Supplements marketed as natural alternatives to weight-loss injections are low-cost, come with no clinical oversight, and make dramatic weight-loss claims. Their dosing is not standardised, and they are not medicines or regulated for this use.

If you are tempted, the questions to ask are simple. Is it licensed as a medicine for weight management? Does a prescriber check your medical history and current medicines? Does anyone follow up with you? A supplement bought without assessment answers no to all three.

The same caution applies to "hacks" for tricking your body out of hunger. Steady meals are a calmer route.

Be especially wary of anyone offering prescription injections without a consultation or prescription. Genuine Mounjaro is a prescription-only medicine. It still works alongside a reduced-calorie diet and increased physical activity, not instead of them. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. For someone starting at 100 kg, 20.9% is about 21 kg. These are trial averages, and individual results vary.2 The trial measured weight, not food noise, so it does not tell us how much the thoughts quieten.

Progress is gradual. Everyone starts on 2.5mg weekly, which is about letting the body adjust rather than weight loss itself, and the dose can step up by 2.5mg at minimum four-week intervals to a maximum of 15mg. Many people settle at 5mg, 10mg or 15mg, and higher is not automatically better.1 Experiences differ widely: some people notice appetite change in week one, others little until a higher dose, and both are normal.1

On access, Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the primary-care rollout began in June 2025 with the highest-need groups first.4 Full rollout is expected to take years, so many people who meet the licensed criteria choose regulated private treatment rather than waiting.

09 · Keeping it quiet after stopping

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4 to 5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1

  • keep your activity routine going as the dose falls away;

What you can do is build fuller meals, with protein and fibre, move regularly, drink fluids and avoid triggers, then speak to a prescriber if that is not enough.
