01 · Step one

First, check it is a real plateau

A real plateau is a stall that outlasts normal day-to-day and week-to-week scale noise. Weight loss on Mounjaro is not linear, and plateaus of a few weeks are normal and expected as the body adapts to a lower weight.1 One flat week is not a plateau.

Early loss also includes water weight, so the trend over weeks matters more than any single weigh-in.13 Weigh at the same time of day, on the same day of the week, and look at the direction of travel across a month rather than reacting to each reading.

Also ask where you are on the dose ladder. Many people are still on the lower rungs at three months, and slower early progress does not predict failure, because the dose you respond to may still be ahead of you.2

03 · Step three

When to talk to your prescriber about your dose

Talk to your prescriber if the stall continues after you have tightened up the basics. Doses step up by 2.5mg at a minimum of 4-week intervals, from 5mg through 7.5mg, 10mg and 12.5mg to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 Higher is not automatically better: many people settle at 5mg, 10mg or 15mg.

Treatment response is reviewed by your prescriber. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1

04 · Why plateaus happen

Why plateaus happen on Mounjaro

Plateaus happen because the body adapts to a lower weight, and weight loss on Mounjaro is not linear.1 A stall of a few weeks does not mean the medicine has stopped working.

Mounjaro activates two gut-hormone receptors, GIP and GLP-1, which help regulate appetite, slow how quickly the stomach empties and improve how the body responds to insulin.1 In practice most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. The medicine supports your eating pattern; it does not replace it.

That is why one of the first questions at a plateau is about food. Portions can creep back up without anyone noticing, and the appetite effect can feel so normal after a few months that you stop paying attention to what you eat. The SURMOUNT-1 results were achieved alongside diet and exercise, not instead of them.2

Another common reason is simply time. The 2.5mg starting weeks are about adjustment rather than weight loss, and most measurable progress builds from 5mg onward, month by month.1 If you are comparing yourself with someone further along the ladder, you may not be plateaued at all.

05 · Realistic expectations

Realistic expectations by month

Think in months, not days. In SURMOUNT-1, a trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages, and individual results vary.2

As a worked example, for someone starting at 100 kg, 20.9% is about 21 kg. That figure was measured at 72 weeks on 15mg, not at 12 weeks and not on a lower dose.

By three months, most people are still on the lower rungs of the dose ladder. A sensible expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation.2 If you are in that range, a slow patch is likely to be part of the normal pattern rather than a failure.

Trial averages do not predict your own result, so use them as context rather than a target. Judge progress by trend, not individual weigh-ins.1

06 · Food, protein and activity

Food, protein and activity: the basics that move the trend

The first lever is an honest review of what you eat. Portions drift back up, and the plateau response in the licence starts there.1 A week or two of simple tracking, written down rather than estimated from memory, usually shows more than any guess.

Protect protein and activity as well. Keep changes small and sustainable. A drastic cut to food intake to break a stall is not something the licence supports, and it is worth discussing any big change with your prescriber first, especially if you already feel very full or nauseous after injections.

07 · Sleep, alcohol and the basics

Sleep, alcohol and the injection routine

Sleep and alcohol are the two named basics to check when you stall.1 Neither needs to be perfect, but an honest look at both is part of the licensed response to a plateau.

Then check your injection routine. Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food.1 There is no best time of day. What matters is consistency.

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time. If more than 4 days have passed, skip it and take the next one on your normal day. Never take two doses within 3 days of each other.1 Repeated gaps can look like a plateau on the scale.

Store pens in the fridge at 2–8°C; once in use a pen can be kept not above 30°C for up to 30 days, then discarded.1

08 · What not to do

What not to do when the scale stops moving

Do not double doses, take extra doses or add unlicensed products to force the scale down. A plateau is not a reason to double doses or add unlicensed products.1

Be wary of supplements marketed as natural alternatives. They are low-cost supplements with no clinical oversight, dosing is not standardised, and dramatic weight-loss claims should be treated with caution. They are not a licensed answer to a plateau.

Finally, never buy pens from sellers offering to supply without a consultation or prescription. 09 · Beyond the scales

Other signs of progress when the scale stalls

The scale is only one measure. In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.2 These are trial-average secondary outcomes, not promises.

10 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

11 · NHS and private routes

NHS and private support during a plateau

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 If you are planning a pregnancy, Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. Full rollout is expected to take years.

Whichever route you are on, the plateau advice is the same: check the basics, then talk to the prescriber who knows your history. Licensed private treatment is for adults with a BMI of 30 or above, or between 27 and 30 with at least one weight-related condition, used alongside a reduced-calorie diet and increased physical activity.1

In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. Tirzepatide takes roughly 4–5 weeks to clear the system after a final dose, and its appetite effects fade as it does. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.
Do you have to stay on GLP-1 forever for weight loss?
There is no single answer, and the decision is made with your prescriber. Treatment response is reviewed: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.
