01 · The free route
The NHS route: how free weight loss medication works
The NHS is the only place where weight loss medication is free at the point of treatment, and access depends on clinical criteria rather than personal preference. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition.45
The primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions. Full rollout is expected to take years.6 That means meeting the NICE criteria on paper does not mean you will be offered treatment this month. Your place depends on how your local area is phasing the rollout.
A realistic NHS plan has four steps:
- Check your numbers. Work out your BMI and list any weight-related conditions, such as high blood pressure, type 2 diabetes or sleep apnoea.
- Book a GP appointment. Ask directly whether you fall into a current rollout group and whether a referral is needed.
- Ask about specialist services. NHS access to Wegovy runs through specialist weight-management services; ask your GP.
- Ask about timing. Request an estimate of when your group will be reached, and ask what support is offered while you wait.
Our guide to whether you qualify for free injections walks through the eligibility questions in more detail, and our page on getting prescribed weight loss medication covers what a prescriber looks at.
02 · Honest picture
What "free" does and does not mean
"Free" usually means free of a private fee, not free of every cost. The medicine itself is funded on the NHS, but in England a standard prescription charge of £9.90 per item can apply unless you are exempt. Check your own exemption status with your GP or pharmacy.
Many websites use "free" to describe something else, such as a free consultation or free delivery. Those are real services, but the medicine is not free. With us, the consultation is free, there is no obligation, and the price shown is the price paid. Delivery is free next-day tracked, with Saturday delivery at £14.99. The pen itself is paid for. See our page on weight loss injections with free UK delivery for what is included.
Free also does not mean immediate. NHS capacity is limited, and the rollout prioritises people with the greatest clinical need. If you are reading this because you meet the licensed criteria but sit outside the first rollout groups, the choices are to wait, to start private treatment, or to start with lifestyle support now and review later.
03 · If you cannot wait
The private route: what it costs and what it includes
If waiting is not an option, regulated private treatment is the alternative, and it starts with a free consultation that carries no obligation. A GPhC-registered prescriber reviews your answers; payment is taken when you place the order, and if the prescriber decides the treatment is not suitable, the order is refunded automatically in full. Our free consultation page explains the process.
The Mounjaro pen at the 2.5mg starting dose has a standard price of £169.99 per month (one 4-week pen). A new customer's first order is £40 off at any dose with code FIRST40, which makes your first 2.5mg pen £129.99. Every later order gets an automatic £25 off the standard price (code DOSE25). Switching from another provider at 15mg? Your first order is £289.99.
For planning, if you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96 at standard prices. A first year costs £2,711.88 if you stay on 5mg after your 2.5mg starting pen, up to £3,599.88 if you step up through every dose to 15mg. These totals are before discounts; a new customer's first order is £40 less and every later order £25 less. Prices were checked 12 October 2026, and all six doses are in stock.
04 · Who qualifies on the NHS
Who qualifies for Mounjaro on the NHS?
Adults with a BMI of at least 35 plus at least one weight-related condition fall within the NICE recommendation for Mounjaro.4 That is the national recommendation, but it is not the same as being offered treatment today, because access is phased in over several years.
The primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.6 Full rollout is expected to take years.
NHS access to Wegovy runs through specialist weight-management services; ask your GP about referral and waiting times.
Practical preparation helps. Bring your recent weight and height, a list of weight-related conditions you have been diagnosed with, and the medicines you take. This lets the GP place you quickly against the current criteria.
05 · Private licence criteria
Licensed criteria for private treatment
Privately, the licence is broader than the first NHS rollout groups. Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.1
It must be used alongside a reduced-calorie diet and increased physical activity. A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable for you.1 Meeting the licence does not mean automatic approval. The decision is clinical.
This gap, between the licence and the first NHS groups, is why many people who meet the licensed criteria choose regulated private treatment rather than waiting. It is a personal decision with a cost attached, and staying with NHS care is entirely reasonable if you fall inside a current rollout group.

06 · What treatment involves
What Mounjaro treatment involves
Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. It is licensed in the UK for weight management in adults and for type 2 diabetes, and comes as a pre-filled multi-dose pen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1
Everyone starts on 2.5mg once a week. The starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at minimum 4-week intervals up to a maximum of 15mg weekly, depending on response and tolerance. Many people settle at 5mg, 10mg or 15mg; higher is not automatically better.1
In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages and individual results vary.2 For someone starting at 100 kg, 20.9% is about 21 kg.
Progress is gradual. The first weeks are an adjustment phase, and most measurable progress builds from 5mg onward, month by month. Think in months, not days. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1 Serious side effects are uncommon but need urgent attention. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.
07 · Cheaper alternatives
Cheaper alternatives to Mounjaro in the UK
Cheaper does not automatically mean equivalent, so compare like with like. Across the UK pharmacies we track (checked 12 October 2026), Mounjaro pens ranged from £143.96 to £339.00 per month depending on dose and provider. The 2.5mg starting pen ranged from £143.96 to £179.99, and the gap between providers for the same dose ran from £27 to £41. Boots' Mounjaro dose prices ran from £179.00 to £339.00 per month.
Shopping around within those figures is the simplest saving, because the medicine is the same. Always check what is included, such as consultation, delivery, reviews and support, rather than comparing the headline pen price alone.
Wegovy (semaglutide) is a different licensed medicine. Its pens are 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once weekly, with titration stepping up after at least 4 weeks and reaching the 2.4mg maintenance dose from week 17. Your first Wegovy pen costs £99.99 as a first-order price. Your prescriber can advise which medicine suits you.
Liraglutide (Saxenda, and products such as Erly) is supplied at 6mg per millilitre and is a daily injection, unlike once-weekly Mounjaro and Wegovy. Orlistat is another named option; figures vary, and your prescriber can advise. Tirzepatide's mechanism is also different: it activates two gut-hormone receptors, GIP and GLP-1, whereas semaglutide is single-action.1

08 · Supplements and "natural" options
"Poor man's Ozempic" and natural alternatives
"Poor man's Ozempic" is a marketing nickname, usually attached to berberine, a low-cost supplement with no clinical oversight. It is not a licensed weight-loss medicine, and it does not work in the way prescription GLP-1 medicines do. Supplement dosing is not standardised, and quality can differ between products.
Treat dramatic weight-loss claims in supplement marketing with caution. Anyone offering a supply of prescription injections without a consultation or prescription is a safety red flag, and counterfeit pens are a real risk. Check that any pharmacy is on the GPhC register; ours is registration 9012878.
"Mimicking Ozempic naturally" cannot be done, because the medicine's effect comes from activating gut-hormone receptors. What you can do is the part every licensed treatment already depends on: a reduced-calorie diet and increased physical activity.1 Our free weight loss diet plan for the UK sets out a simple framework that costs nothing to start.
09 · Tablets and newer drugs
Weight loss tablets and newer medicines
Weight loss tablets are prescribed after a clinical assessment, not chosen from a shelf, and the same principle applies as for injections: a prescriber checks your BMI, health conditions and current medicines. Oral options now exist, including Wegovy tablets and Foundayo. See current Wegovy tablet prices on the relevant product page.
Newer medicines are not necessarily better for you. Mounjaro activates two gut-hormone receptors, GIP and GLP-1. A tablet may suit someone who prefers not to inject, which is a format preference rather than a proven efficacy advantage. Your prescriber can discuss options as they become relevant to you.
There is no single best weight loss pill. The right choice depends on your BMI, conditions, other medicines, preferences and budget. For any prescription medicine, the safest way to find out is a consultation.
Tell your prescriber about everything you take. If you take the contraceptive pill, Mounjaro slows stomach emptying, which can reduce how well it is absorbed. Switch to a non-oral method, or add a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase. Because Mounjaro slows stomach emptying, it can affect how quickly some oral medicines are absorbed, particularly when you start or increase the dose. Tell your prescriber especially about warfarin, insulin and sulfonylureas, where doses may need reducing to avoid low blood sugar.1
