What to do first if you are tired on Mounjaro
Start by noting when the tiredness appears, because timing is the most useful clue. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects, and they usually settle.1 If your low-energy days sit in the few days after your weekly dose and ease before the next one, that fits the expected pattern.

04 · Why tiredness happens
Does Mounjaro drain your energy?
For some people it does, particularly early on. Tiredness in the first days after each injection is among the commonest early effects, alongside mild nausea and altered appetite, and these usually settle.13
Mounjaro contains tirzepatide, which activates GIP and GLP-1 receptors. Together these help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin.1 Most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. That change in how much you eat is one thing worth checking if your energy dips.

Use your injection day to protect your energy
There is no best time of day to inject: Mounjaro works the same in the morning or evening, with or without food.1 What matters is injecting on the same day each week. Many people choose a weekend morning so that early side effects fall on days off.
That is a practical way to manage tiredness without changing the medicine. If your worst day is the second day after your injection and that lands on a busy work day, move your injection day. You can change your weekly injection day as long as at least 3 days (72 hours) have passed since your last dose.1
If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip it and take the next dose on your normal day. Never take two doses within 3 days of each other.1 Doubling up to catch up is not safe and will not help your energy.
06 · Dose pace and tolerability
Tiredness after a dose increase: ask about pace
Everyone starts on 2.5mg once a week, and the starting dose is about letting your body adjust rather than weight loss itself. After at least 4 weeks the dose increases to 5mg, then can step up by 2.5mg at minimum 4-week intervals to a maximum of 15mg weekly, depending on how you respond and tolerate it.111
Many people settle at 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace.1 If tiredness follows each step up, tell your prescriber.
07 · Eating and drinking
What to eat and drink when your appetite is small
Mounjaro is used alongside a reduced-calorie diet and increased physical activity, not instead of them.2 Reduced does not mean as little as possible. When appetite falls sharply, it is easy to under-eat without noticing.
Ask your prescriber or GP for a target that suits you. In the meantime, a few non-clinical habits are worth trying:
- Plan meals around foods you can face on low-appetite days rather than skipping meals.
- Keep a drink to hand through the day and notice if you rarely drink.
- Eat at regular times so a day does not slip by unfed.
- Write down what you eat for a few days; the gaps are often obvious on paper.

08 · Starvation mode and other myths
Starvation mode, quick fixes and energy drinks
If you are eating very little and feel weak, dizzy or exhausted, tell your prescriber.
Anything promising dramatic or instant results deserves scepticism. Stimulant-heavy products can also mask a problem you need reviewed, so check with your prescriber or pharmacist before adding supplements to your routine.

If a symptom is not on the UK patient leaflet's list of side effects, it is not a listed side effect; tell your prescriber about any new or lasting symptom, and you can report suspected side effects through the MHRA Yellow Card scheme.19
09 · Judging progress
Keep tiredness in proportion to your progress
Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month.1 Some people notice appetite change in week one, others feel little until a higher dose, and both are normal.
In the SURMOUNT-1 trial of 2,539 adults, people on tirzepatide alongside diet and exercise lost on average 15% of body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages and individual results vary.2 Think in months, not days, and judge progress by trend rather than single weigh-ins, since early loss includes water weight.
If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you. 10 · When to get help
When tiredness needs a proper review
Speak to your prescriber if tiredness is severe, getting worse, lasting well beyond the early weeks or stopping you doing normal things. Do the same if it comes with other symptoms that worry you. If you feel suddenly very unwell, get medical help straight away.
Bring your log: dose, injection day, sleep, food and drink, and when the tiredness peaks. Mention every other medicine and supplement you take. Our prescribing team can review your dose and side effects, and you can report suspected side effects through the Yellow Card scheme.19

11 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
