04 · How the KwikPen works

How the KwikPen is dosed in mg

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen, the KwikPen, in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 The strength on the pen label is the dose it delivers when you dial it, and that is expressed in milligrams, not in units or millilitres.

The pen is set by turning the dose knob until the dose indicator shows the correct dose in the window. You confirm the dose by the number in the window, never by counting clicks.13 This matters because many online calculators and videos for other injectables talk about clicks, marks and lines. None of that applies to a Mounjaro pen. If the window says 2.5mg, that is your dose.

Each pen contains four weekly doses, and the pen will not let you dial past the medicine left inside.13 That design choice protects you from taking more than the pen holds. It also means you do not need to work out a volume to know whether a dose is correct. The pen limits itself.

It helps to think of the pen as a measuring device with the calculation already built in. Someone using a vial must decide how much liquid equals the dose, because the vial label states a concentration and the syringe states units. Someone using a KwikPen skips that step completely.

05 · Where units come from

Where the idea of units comes from

Units are a feature of the syringe, not of the medicine. A syringe marked in units is a volume-measuring tool, and the marks simply divide its barrel into equal slices of liquid. Milligrams measure the weight of the active ingredient. To get from one to the other you need a third piece of information: the concentration, meaning how many milligrams of medicine sit in each millilitre of liquid.

That is why one fixed answer to "how many units is 2.5mg" does not exist. If a vial is more concentrated, a smaller volume carries the same dose, so fewer marks are drawn up. If it is more dilute, the same dose needs more liquid and more marks. The calculation is only as good as the concentration figure you feed into it, and an error in that figure carries straight through to the dose.

Online charts usually assume a particular concentration and then present the result as though it were universal. Readers skim past the disclaimer and keep the table. That is where mistakes happen.

Another point worth knowing is that products sold as vials of tirzepatide are not the licensed UK Mounjaro pen. Insulin syringes, pens and why not to mix them

A common follow-up question is whether the medicine can be drawn out of a KwikPen with an insulin syringe to get a number of units. The pen is not designed for that. It is a sealed, multi-dose delivery device, and its dosing accuracy comes from the mechanism turned by the dose knob. Taking medicine out by another route means you are no longer relying on that mechanism.

The pen is set by turning the dose knob until the dose indicator shows the correct dose, and you confirm the dose by the number in the window.13 That is the whole of the licensed method. A syringe adds steps that the design never intended: piercing the cartridge, estimating a volume, and guessing at how much medicine each mark represents.

People sometimes reach for a syringe because they want a smaller amount than the pen will dial, often hoping to ease side effects. The licensed safety position on microdosing is that it is not licensed, not supported by the pen's design and not endorsed by any UK regulator. The licensed answer is not to shrink the dose by hand but to talk to your prescriber, who can hold a dose longer or step down within the licence.

The answer from the product information is clear and worth stating plainly. "Microdosing", meaning taking smaller or more spread-out doses than the pen is designed to deliver, is not licensed, not supported by the pen's design and not endorsed by any UK regulator.

The licensed dose ladder already starts low for tolerability. Everyone begins on 2.5mg once a week, and this starting dose is about letting your body adjust, not about weight loss itself.1 If side effects are the concern, your prescriber can hold a dose longer or step down within the licence. Altering doses outside the licence means unknown dosing accuracy and no safety data.

This also explains the behaviour of the pen when it runs low. If the window will not turn to a full dose, the pen does not contain enough for one and should not be used for a partial injection.13

After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1

Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace.1

The 2.5mg starting weeks are about adjustment, and experiences differ widely: some people notice appetite change and early loss in week one, others feel little until a higher dose, and both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.1

Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend, not individual weigh-ins.

For context on what higher doses achieved in a trial: in the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them. Notice that the figures begin at 5mg: 2.5mg is the entry step, not the target.

Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1 Reporting how you feel to the prescribing team helps decide when the next step makes sense.

09 · Injecting and missed doses

Injecting, timing and missed doses

Once the window reads 2.5mg, the injection itself is simple, and the licensed instructions cover everything you need. Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food. Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose. Each KwikPen contains four weekly doses.113

There is no "best" time of day. Mounjaro works the same morning or evening, with or without food. What matters is the same day each week. Pick a day and time you will remember; many people choose a weekend morning so early side effects fall on days off, and then stick to it. For how long to hold the pen against the skin, follow the pen's instructions rather than any figure quoted elsewhere.

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.1

You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 This is useful if your routine changes, for example with shift work or travel, and again it needs no conversion or special measurement.

Rotation of sites is easy to overlook. Using a slightly different spot each week within the licensed areas helps the skin recover, and it is one habit worth building from the very first dose. If you are injecting a family member or a friend is helping, the back of the upper arm becomes an option only when someone else gives the injection.

Compare this with a vial routine, where every injection starts with a calculation, a draw-up and a check of the marks. The pen routine replaces all of that with a dial, a window and a single injection. The simplicity is deliberate and, for most people, a reason to prefer the licensed pen route.

10 · Storage and travel

Storage, travel and keeping the dose intact

A pen only delivers the dose on its label if it has been stored properly, so storage deserves as much care as dialling. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, useful for short trips, and must then be thrown away. Do not use a pen that has been above 30°C or has been frozen. Keep the pen cap on to protect it from light.113

You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements; for hot climates use an insulated pouch. Check destination rules for travelling with prescription injectables before you fly.1

Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit, because figures circulating online often come from other countries' labels.13

What does this have to do with units? Storage affects whether the medicine you dial is the medicine you expect. A pen that has frozen or overheated is not one to dose from, however precisely the window reads. Dosing accuracy begins with proper handling, and no calculator corrects for a damaged pen.

Practical habits help. Keep a spare pen in the fridge, never frozen. Write the date you started using a pen on the box so you know when its in-use period ends. Store the sharps bin out of reach of children and bring it back to be disposed of appropriately according to local arrangements.
