What a 2.5mg dose of Mounjaro means in practice

A 2.5mg dose is a dose of tirzepatide measured in milligrams, and the pen you hold is built to deliver it without you converting anything. Mounjaro is a once-weekly injectable prescription medicine that comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

The pen is set by turning the dose knob until the dose indicator shows the correct dose in the window. You confirm the dose by the number in the window, never by counting clicks.1 That number is in mg. It is the figure your prescriber, your label and the leaflet all use.

02 · Your next step

What to do if you still want a ml figure

Read the patient information leaflet supplied in the box, because it is the manufacturer's own description of the device.13 If anything in it is unclear, ask the pharmacist or prescriber who supplied the pen. Do not try to work out a volume yourself and use it to adjust what you inject.

The practical point is simple. You turn the knob until the window shows the dose you were prescribed. If you are starting treatment, that is 2.5mg. There is nothing to measure, draw up or calculate. 03 · Why mg, not ml

Why the dose is in mg and not ml

The dose is in mg because that is how the medicine is licensed and prescribed. Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly, and its six strengths are named by the milligrams of tirzepatide they deliver.1

Millilitres describe how much liquid there is. Milligrams describe how much active medicine you receive. With a vial and syringe you would have to translate between them yourself, which is where dosing errors creep in. A KwikPen removes that step: the pen does the translation and you read one number.

04 · Using the pen dial

How the KwikPen dial works

You set the dose by turning the knob and reading the window. The Mounjaro KwikPen is set by turning the dose knob until the dose indicator shows the correct dose in the window — you confirm the dose by the number in the window, never by counting clicks.1

Each pen contains four weekly doses; the pen will not let you dial past the medicine left inside.1 This is a useful safety feature. It means you cannot accidentally dial more than remains.

If the window will not turn to a full dose, the pen does not contain enough for one and should not be used for a partial injection.1 In that situation, do not top up from another pen and do not inject what is left. Contact the pharmacy that supplied it, and keep the pen so they can advise.

05 · Dose ladder

Where 2.5mg sits on the dose ladder

2.5mg is the starting step, and everyone begins there. Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself.111

After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.111 Many people settle at a maintenance dose of 5mg, 10mg or 15mg — higher is not automatically better, and your prescriber will guide the right pace.

Because each strength is its own pen, you do not change the dose by adjusting a volume. You move to the next pen when your prescriber says so. We explain what to expect at this step in whether the starting dose works.

06 · Injecting safely

How to inject once the dose is set

Mounjaro is injected under the skin once a week, on the same day each week. Specifically, it is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food.1

Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.1 For how long to hold the pen in place, follow the pen's instructions.13

There is no best time of day. Mounjaro works the same morning or evening, with or without food. What matters is the same day each week. Pick a day and time you will remember (many people choose a weekend morning so early side effects fall on days off) and stick to it.1

07 · No dose splitting

Why you should not split or stretch a dose

Taking less than the pen is designed to deliver is not a safe workaround. "Microdosing" — taking smaller or more spread-out doses than the pen is designed to deliver — is not licensed, not supported by the pen's design and not endorsed by any UK regulator.1

The licensed dose ladder already starts low (2.5mg) for tolerability, and your prescriber can hold a dose longer or step down within the licence if side effects are the concern. Altering doses outside the licence means unknown dosing accuracy and no safety data.1

If side effects worry you, talk to your prescriber about staying longer on a step or stepping down within the licence. That is a supported route and keeps your dosing accurate.

08 · Missed doses and timing

Missed doses and changing your day

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day.1 If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1

You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1

None of this involves measuring a volume. Each time, you dial the prescribed dose and read the window.

09 · Storing the pen

Storing and travelling with your pen

Keep the pen in the fridge until you start it. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use — useful for short trips — and must then be thrown away. Do not use a pen that has been above 30°C or has been frozen.113

Keep the pen cap on to protect it from light, and never use a pen that has been frozen. You can fly with Mounjaro: carry pens in hand luggage (hold luggage can fall below freezing), ideally in a cool bag with the prescription label or a copy of your prescription.1

Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit.13

10 · What to expect on 2.5mg

What to expect in the starting weeks

The starting weeks are about adjustment, and experiences differ widely. Some people notice appetite change and early loss in week one, others feel little until a higher dose — both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.1

Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.13 Tell your prescriber about any new or lasting symptom, and you can report suspected side effects through the MHRA Yellow Card scheme.19

Weight loss on Mounjaro is gradual by design. Most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so think in months, not days.2 Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity.

11 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

12 · NHS and private access

Getting Mounjaro: NHS and private routes

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46

Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
