The 7.5mg is the amount of tirzepatide you inject once a week, not the size of the pen. Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly, supplied as a pre-filled multi-dose KwikPen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Each KwikPen contains four weekly doses.13

So a 7.5mg pen is a pen designed to give four injections of 7.5mg, one a week, over four weeks. The pen does the measuring for you.

That is why the question "how many ml" rarely matters in practice. You are never asked to draw up a volume, read a syringe or convert milligrams to millilitres. You turn the dose knob until the window shows your dose, and inject.

05 · Dose versus volume

Dose versus volume: why mg is what matters

Milligrams measure how much tirzepatide you receive; millilitres measure how much liquid carries it. For a pre-filled pen, only the first is something you ever act on. The licensed strengths run from 2.5mg to 15mg, and the dose you are prescribed is described in mg.1

This is different from some other injectables. Daily liraglutide, for instance, is supplied at 6mg per millilitre, which makes volume a more visible part of that product's labelling. Mounjaro is a once-weekly pen, and the device handles the conversion, so you do not need a calculator, a syringe or a conversion chart.

It also explains why two pens with different strengths can look almost identical on the bench. The pen body, the dial and the number of weekly doses are the same design; what changes is the strength you are prescribed. Always check the strength printed on the label before your first injection from a new pen, and confirm it matches your prescriber's instructions.

06 · Dial and clicks

How the dial works and why you should not count clicks

You set the Mounjaro KwikPen by turning the dose knob until the dose indicator shows the correct dose in the window; you confirm the dose by the number in the window, never by counting clicks.13 This answers a question we see often: how many clicks for 5mg from a 7.5mg pen? There is no click count to use. If you are prescribed 5mg you should have a 5mg pen, not a 7.5mg pen dialled back.

Dialling a different dose from a pen of another strength, or trying to approximate a dose by clicks, is not licensed and not how the pen was designed. "Microdosing", meaning smaller or more spread-out doses than the pen is designed to deliver, is not licensed, not supported by the pen's design and not endorsed by any UK regulator. Altering doses outside the licence means unknown dosing accuracy and no safety data.

07 · How much is left

How much is left in the pen after four doses

After four weekly injections the pen is finished. The pen is built to deliver four doses; do not try to extract more.13

The pen includes a safeguard. It will not let you dial past the medicine left inside, and if the window will not turn to a full dose, the pen does not contain enough for one and should not be used for a partial injection.13

08 · Moving between doses

Where 7.5mg sits on the dose ladder

7.5mg is the third step of the Mounjaro ladder. Everyone starts on 2.5mg once a week, which is about letting your body adjust rather than weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on response and tolerance.1

Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better. Your prescriber guides the pace.

In SURMOUNT-1, a trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 Individual results vary.

09 · Storage and travel

Storing your 7.5mg pen

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, useful for short trips, after which it should be discarded.1 Keep the cap on to protect the pen from light, and never use a pen that has been frozen.

You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. For hot climates, use an insulated pouch, and check destination rules for prescription injectables before you fly.13

Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit; figures circulating online often come from other countries' labels.

10 · Ingredients and new pens

Ingredients, newer pens and what to avoid

The active ingredient is tirzepatide; the full list of other ingredients is in the patient information leaflet in the box, so use that rather than a summary.13 The leaflet is also the place to check allergy information.

Rely on the leaflet and label that arrive with your own pen, and ask the pharmacy if something looks different from what you expected. The strength in mg and the four-dose design are what you verify.

As for what to avoid, the leaflet and your prescriber are the right source for your own medicines and conditions. Do not alter doses, do not buy from sellers offering to supply without a consultation or prescription, and tell your prescriber about any new symptoms. You can report suspected side effects through the MHRA Yellow Card scheme.19

11 · Injecting and missed doses

Injecting, timing and missed doses

Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food. Licensed sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.13 There is no best time of day. What matters is picking a day and time you will remember.

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time. If more than 4 days have passed, skip it and take the next one on your normal day. Never take two doses within 3 days of each other. You can change your injection day as long as at least 3 days (72 hours) have passed since your last dose.1

12 · Buying safely

Buying a genuine pen

Buy Mounjaro only from a pharmacy you can verify on the register.
