Why there is no click count for 2.5mg on a 12.5mg pen
There is no approved click count, because the pen is not meant to be used that way. The KwikPen is set by turning the dose knob until the dose indicator shows the correct dose in the window. You confirm the dose by the number in that window, never by counting clicks.13
A 12.5mg pen is built to give 12.5mg each week. Its four weekly doses are all the same strength. Altering doses outside the licence means unknown dosing accuracy and no safety data.

What to do if you need 2.5mg and only have a 12.5mg pen
Do not try to work out a partial dose. Speak to your prescriber or pharmacist, who can arrange the right strength for you.
- If you are starting treatment: everyone begins on 2.5mg weekly, using a 2.5mg pen. The starting dose is about letting your body adjust, not about weight loss itself.1
- If you have been on a higher dose and had side effects: your prescriber can hold a dose longer or step down within the licence. You do not need to improvise.1
- If you have restarted after a gap: ask the prescriber which strength is right now, rather than guessing from the pens you have at home.

04 · How the pen works
How the KwikPen dial works
The pen is a pre-filled, multi-dose KwikPen that comes in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Each strength is its own pen, filled and set up to deliver that weekly dose.
You turn the dose knob until the indicator shows the correct dose in the window. Each pen contains four weekly doses; the pen will not let you dial past the medicine left inside.13 That lockout is a safety feature. It stops you dialling more than the pen holds, but it is not a tool for measuring smaller amounts.
If the window will not turn to a full dose, the pen does not contain enough for one and should not be used for a partial injection.13 So in normal use you never count anything. You look at the number and check it matches your prescribed dose.
05 · Why not to divide a pen
Why dividing a higher-strength pen is a bad idea
Dividing a pen is not licensed, and the risks are real. "Microdosing" means taking smaller or more spread-out doses than the pen is designed to deliver. It is not licensed, not supported by the pen's design and not endorsed by any UK regulator.1
Altering doses outside the licence means unknown dosing accuracy and no safety data.1 In plain terms, nobody has tested what a partial dial setting delivers, so you cannot know how much medicine you received.
There are also practical problems. The pen can also be in use for only a limited time. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away.1
Finally, the licensed ladder already starts low for tolerability. If side effects are the worry, your prescriber has safe options that need no improvisation.
06 · The licensed dose ladder
The licensed dose ladder and why 2.5mg matters
The licence already provides a low, gentle start. Everyone starts on 2.5mg weekly. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1
Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace.1 The 12.5mg pen is a step on that ladder for people who have already built up to it, not a source of lower doses.
If you feel a step has been too big, tell your prescriber. Holding a dose or stepping down within the licence is a normal clinical decision, and it is far safer than counting clicks.
07 · Using your pen correctly
Using your pen correctly each week
Mounjaro is injected under the skin (subcutaneously) weekly, on the same day each week, at any time of day, with or without food.1 Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.1 Follow the pen's instructions for the injection itself.
If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.1
You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1
08 · Storage and travel
Storage and travel with your pen
Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Keep the pen cap on to protect it from light. Do not use a pen that has been above 30°C or has been frozen.1
You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. For hot climates use an insulated pouch, and check destination rules for travelling with prescription injectables before you fly.1
Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit. Figures circulating online often come from other countries' labels.1
09 · Setting expectations
Why a low dose is not a wasted dose
People sometimes want to carve out small doses because they fear stronger ones. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month.1
In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages, and individual results vary.2 Some people notice appetite change early, and others feel little until a higher dose. Both are normal.
Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle. Treatment is reviewed by your prescriber, who adjusts the pace to you.
