A quick overview

What to know
before you start.

  • The medicine acts from the first injection, but the 2.5mg starting dose is for adjustment, not weight loss itself.
  • Some people notice appetite change in the first week; others feel nothing until a higher dose. Both are normal.
  • Most measurable progress builds from 5mg onward, month by month. Think in months, not days.
  • Your prescriber reviews response after 6 months on the highest dose you can tolerate.
  • Judge progress by trend over weeks, not by a single weigh-in.
In this guide

01 · The core answer

How long Mounjaro takes to work, stage by stage

Mounjaro takes effect from your first injection, but you will not see the full benefit for many months. Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1, which help regulate appetite and slow how quickly the stomach empties.1 In practice, most people feel fuller sooner and less hungry between meals.1 That is the "working" you may feel first. The scales follow later.

StageWhat is usually happeningWhat to expect
First injectionThe medicine begins acting on appetite and digestionSome people notice a difference straight away; many notice little
First weeks on 2.5mgAdjustment phaseLosses are usually modest; mild nausea or tiredness can follow each injection
Stepping up to 5mg and beyondDose rises after at least 4 weeks if toleratedMost measurable progress builds from here, month by month
The early monthsStill on the lower rungs of the ladder for most peopleMid single-digit percentage loss is a sensible expectation, with wide variation
72 weeksWhen the headline trial results were measuredThe full trial averages are a long-term figure, not a three-month one

The pace is deliberate. The dose ladder starts low so your body can adjust, and each step is spaced out for tolerability rather than speed.

02 · Your next step

Is your timeline normal, and what should you do next?

Almost certainly yes, if you are in the first weeks and feel little. Experiences differ widely: some people notice appetite change and early loss in the first week, others feel little until a higher dose, and both are normal.1 A quiet start is not a sign the medicine has failed.

Your practical next steps are simple. Inject on the same day each week, follow your prescriber's titration plan and keep to your reduced-calorie diet and activity. Then judge progress over several weeks. If you are considering treatment and want to see how it works in practice, our weight loss clinic page explains the pathway.

03 · The 2.5mg starting weeks

What happens on the 2.5mg starting dose

The 2.5mg dose is about letting your body adjust, not about weight loss itself.1 Everyone starts here, whatever their weight, and the first pen holds four weekly doses. That is why a slow first month is expected rather than worrying.

The first weeks are about adjustment. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.13 Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.

This also answers a common question: can you lose weight on 2.5mg of Mounjaro? Some people do see early loss, but losses at this stage are usually modest. The starting dose is not designed to be the dose that does the heavy lifting. Your prescriber decides the pace, and doses should never be split.

Many readers ask whether Mounjaro works immediately. The honest answer is that the medicine acts from the first dose, but an obvious change in appetite or weight is not assured on any particular day.

04 · From 5mg onwards

Why progress usually picks up from 5mg

After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better.1

Weight loss on Mounjaro is gradual by design. Most measurable progress builds from 5mg onward, month by month.2

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

As a worked example, for someone starting at 100 kg, 20.9% is about 21 kg. That is the figure at 72 weeks on the top dose, not a three-month figure.

05 · What to expect by three months

What to expect after the first few months

Three months in, most people are still on the lower rungs of the dose ladder, so trial-average losses at this stage are modest compared with the headline figures.2 In SURMOUNT-1, average weight loss built month by month, with the full 15–21% averages measured at 72 weeks, not 12.2

A sensible 3-month expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation.2 Slower early progress does not predict failure, because the dose you respond to may still be ahead of you.2

This is also the stage when other changes may show up before the scales do, such as clothes fitting differently. In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures.2 These are trial-average secondary outcomes, not promises. They are one reason prescribers talk about weight management as health management.

06 · If it feels like nothing is happening

Why Mounjaro might not seem to be working in the first week

A quiet first week is common and usually reflects the starting dose, not a failing medicine. The 2.5mg dose exists for tolerability, and some people simply do not feel appetite change until a higher dose.1 Weighing yourself daily can also mislead, because early loss includes water weight and day-to-day fluctuation hides the trend.

Work through these basics before drawing conclusions:

  • Check you are injecting on the same day each week, as the pen instructions describe.
  • Be honest about portions, which tend to drift back up.
  • Protect protein intake and keep up physical activity.
  • Look at sleep and alcohol, which both affect appetite and weight.
  • Look at the trend over several weeks rather than single weigh-ins.

Plateaus are part of the picture too. Weight loss on Mounjaro is not linear: plateaus of a few weeks are normal and expected as the body adapts to a lower weight.2 Within the licence, the responses are to review food intake honestly, protect protein and activity, check the basics, and discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum.1 A plateau is not a reason to double doses or add unlicensed products.

07 · Your prescriber's review

When your prescriber reviews your response

Response is formally reviewed, not left to guesswork. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.2

Because titration takes time, most people reach their maintenance dose well after the first month. That is why judging the medicine on the starting pen alone is premature. At EverydayMeds, ongoing monthly reviews, dose-titration and follow-up assessments by the prescribing team are part of the service, and repeat orders re-check suitability.

The response review also matters for your wider health. In the trials, most participants with prediabetes returned to normal blood-sugar levels, though these are trial averages and not promises.2

08 · Injection timing and routine

Does the time of injection change how fast it works?

No. There is no "best" time of day: Mounjaro works the same morning or evening, with or without food.1 What matters is the same day each week. Pick a day and time you will remember, and many people choose a weekend morning so early side effects fall on days off.

Mounjaro is injected under the skin once a week. Licensed sites are the stomach area, the front of the thigh, or the back of the upper arm if someone else gives the injection, and you should rotate the site with each dose.1

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day.1 If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day, and never take two doses within 3 days of each other.1 You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1

Consistency keeps levels steady. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1

09 · Semaglutide and Ozempic compared

How does this compare with semaglutide and Ozempic?

Semaglutide, sold as Wegovy and Ozempic, is a single-action GLP-1 medicine, whereas tirzepatide activates both GIP and GLP-1 receptors.1 Ozempic is semaglutide licensed for type 2 diabetes. It is not a UK weight-loss medicine, prescribing it privately for weight loss is off-label, and a regulated UK pharmacy will not supply Ozempic for weight loss. Both are gradual, both use stepped dose ladders, and neither is a quick fix.

For a trial-level comparison, SURMOUNT-5 was the first large head-to-head trial of tirzepatide and semaglutide for weight loss.15 If you are weighing the two medicines, a prescriber can explain which suits your health history.

The general principle holds across both: expect adjustment first, then steady progress as doses rise.

10 · Side effects and eating

Side effects and food in the early weeks

The commonest early effects are mild nausea, altered appetite and tiredness in the first days after each injection, and they usually settle.13 Your prescriber can advise if they do not settle.

If you notice a new symptom that worries you, talk to your prescriber. You can also report suspected side effects through the MHRA Yellow Card scheme.19

Treatment works alongside a reduced-calorie diet and increased physical activity. If particular foods upset your stomach, mention it to your prescriber, who can advise on diet and dose pace.

Remember to store pens correctly. Keep them in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded.1

11 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

12 · NHS and private routes

Getting started: NHS or private

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).4 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

If you go privately with us, you start with a free online consultation, which is reviewed by a GPhC-registered prescriber (GPhC 9012878). The price shown is the price paid. Your medicine is supplied through official LillyDirect supply, with free next-day tracked delivery and same-day dispatch when ordered by 3pm. All six doses are in stock.

Mounjaro is a prescription-only medicine, used alongside a reduced-calorie diet and increased physical activity. Individual results vary, and this page is information, not a substitute for medical advice.

Your questions, answered

Mounjaro price UK — frequently asked questions

Answers to common questions about Mounjaro prices, NHS access and ongoing treatment.

Ask our pharmacy team
How long till Mounjaro starts working?
Mounjaro acts from your first injection, and some people notice appetite change in the first week. Others feel little until a higher dose, which is also normal. Measurable weight loss builds month by month, mostly from 5mg onwards, and the trial results were measured at 72 weeks, so think in months rather than days.
Does Mounjaro work immediately?
The medicine begins acting after the first dose, but you should not expect an obvious change straight away. Some people notice appetite change and early loss in the first week, while others feel little until a higher dose. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.
Can you lose weight on 2.5mg of Mounjaro?
Some people do, but losses on 2.5mg are usually modest. This starting dose is about letting your body adjust, not about weight loss itself. Most measurable progress builds from 5mg onward, month by month, so a slow start on 2.5mg is expected and does not predict how you will respond later.
What to expect after the first shot of Mounjaro?
Expect a mild, adjusting start. Mild nausea, altered appetite and tiredness in the first days after an injection are the commonest early effects and usually settle. Some people feel fuller sooner, others notice nothing yet. Keep injecting on the same day each week and contact your prescriber if symptoms worry you.
Why isn't Mounjaro working in the first week?
A quiet first week is common, because the 2.5mg starting dose is for adjustment and experiences differ widely. Early weight changes also include water weight and day-to-day fluctuation. Check your injection routine, portions, protein, sleep and alcohol, then judge the trend over several weeks. Your prescriber can review your dose if progress stays slow.
What is the best time to inject Mounjaro?
There is no best time of day. Mounjaro works the same morning or evening, with or without food. What matters is injecting on the same day each week. Choose a day and time you will remember; many people pick a weekend morning so early side effects fall on days off.
How quickly does semaglutide kick in?
Semaglutide, the medicine in Wegovy and Ozempic, is also gradual, with a stepped dose ladder. Both Wegovy and Mounjaro are licensed treatments; a prescriber can advise on which suits you and your own situation.
When will my prescriber review whether Mounjaro is working?
If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you. The review is measured on the highest tolerated dose, not from your first injection, because titration takes time.
