A course of Mounjaro has no fixed length, because it is a weight-management treatment that runs for as long as it is working and is appropriate for you. The licensed plan is built around a ladder of doses rather than a countdown. Everyone starts on 2.5mg once a week, and the dose can then rise towards a maximum of 15mg depending on how you respond and tolerate it.1

It helps to think of treatment in three phases: starting dose, titration and maintenance. Starting and titration cover the early weeks as the dose rises; maintenance is the stretch on the dose that suits you. Should you plan for a short course or a long one?

Plan for months at the very least, and be open to treatment lasting well beyond the dose step-up. The trial that produced the headline figures ran for 72 weeks, and the average results were measured at the end of that period, not after a few weeks.2 If you are hoping for a quick, short burst of treatment, it is worth saying so in your consultation, because that expectation shapes what a sensible plan looks like.

Two decisions matter most when you are weighing this up. First, are you comfortable with a weekly injection for a long stretch of time? Second, are you prepared to pair it with a reduced-calorie diet and more physical activity, which is how the medicine is licensed to be used?1 If both answers are yes, the next step is a free consultation reviewed by a GPhC-registered prescriber.

03 · Dose timeline

The Mounjaro dose timeline, step by step

The dose timeline starts at 2.5mg and rises no faster than every four weeks. Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1

Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace.1 That is why two people can start on the same day and have very different course lengths in practice. One may reach their dose quickly and stay there. Another may hold at a lower strength for longer because it suits them.

The words "at least" and "minimum" in the licence matter. They set the fastest permitted pace, not a target. 04 · What to expect over time

What to expect as the months pass

Weight loss on Mounjaro is gradual by design, so the useful way to read a course is in months. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month.

Early experiences differ widely. Some people notice appetite change and early loss in week one, while others feel little until a higher dose, and both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.

Three months in, most people are still on the lower rungs of the dose ladder, so trial-average losses at this stage are modest compared with the headline figures: in SURMOUNT-1, average weight loss built month by month, with the full 15–21% averages measured at 72 weeks, not 12. A sensible 3-month expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation. Slower early progress does not predict failure, because the dose you respond to may still be ahead of you.

The trial figures themselves come with context. In SURMOUNT-1 of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages, individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

For someone starting at 100 kg, 20.9% is about 21 kg. That is a worked example of arithmetic, not a forecast of what you will lose or how quickly.

05 · The review point

The 6-month review: when a course gets reassessed

Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.4

This is the closest thing to a built-in decision point in a Mounjaro course. It is a conversation, not an automatic cut-off. The words "highest tolerated dose" matter, because the review is about how you have done on the highest dose you can tolerate, not on the starter pen.

06 · Stopping Mounjaro

What happens when a course ends

When you take your final dose, tirzepatide stays in your body for a while and then clears. It has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1

That clearance period is a pharmacology fact, not a recovery plan. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average.14

Practical points before a planned stop:

  • Tell your prescriber you are thinking of stopping, so the decision is shared and recorded.
  • Keep up the diet and activity habits you built, since the medicine is used alongside them, not instead of them.1
  • Report any new symptoms to your prescriber, and to the Yellow Card scheme if you think they are linked to the medicine.

07 · Pens and weekly routine

How pens, doses and routine shape the course

Each KwikPen contains four weekly doses, so a pen is the basic unit of a course. Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food. Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.1

There is no "best" time of day. Mounjaro works the same morning or evening, with or without food. What matters is the same day each week. Many people choose a weekend morning so early side effects fall on days off.13

Real life interrupts a weekly schedule, and the licence covers that. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other. You can also change your weekly injection day as long as at least 3 days (72 hours) have passed since your last dose.1

Storage can also affect how you pace a pen. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days — useful for short trips — after which it should be discarded.1 Unopened pens should be kept in the fridge and keep until the expiry date printed on the box.

08 · Plateaus and pace

Plateaus, patience and not shortening the course

Plateaus do not mean the course has failed or should be cut short. Weight loss on Mounjaro is not linear, and plateaus of a few weeks are normal and expected as the body adapts to a lower weight. Within the licence, the responses are to review food intake honestly (portions drift back up), protect protein and activity, check the basics (sleep, alcohol), and discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum.

A plateau is not a reason to double doses or add unlicensed products. The same goes for trying to shorten or stretch a course by changing how you dose. Altering doses outside the licence means unknown dosing accuracy and no safety data.1

If your worry is cost or length of treatment, talk to your clinical team rather than adjusting the pen yourself. Planning ahead also helps with budgeting: a first year costs £2,711.88 if you stay on 5mg after your 2.5mg starting pen, up to £3,599.88 if you step up through every dose to 15mg, at standard prices before discounts. For a maintenance dose between those, use that dose's monthly price: £279.99 at 7.5mg, £290.99 at 10mg and £319.99 at 12.5mg.

09 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

10 · NHS and private routes

NHS or private: does it change the length?

The medicine and the dose ladder are the same whichever route you use. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions). Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.4
