03 · Unopened pens

Unopened pens: fridge storage until the expiry date

An unopened pen belongs in the fridge until the printed expiry date. That is the full instruction for a sealed pen in its box.13 The fridge range is 2–8°C, and the pen should never be frozen.1

Keep spare pens in their original box. The box carries the batch number and expiry date, and it helps protect the pen from light. A genuine UK pen comes from a registered pharmacy with a batch number and expiry date, and arrives cold-chain packaged.13

Place pens on a shelf in the main body of the fridge rather than against the back wall or near the freezer compartment. Those spots are the likeliest to dip toward freezing. A fridge door is more exposed to temperature swings every time it opens, so a middle shelf is the safer choice.

The printed expiry date is the answer for your specific pen, because different batches carry different dates.

Once you have started a pen, it can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away.1 The licence allows an in-use pen to be kept out of the fridge not above 30°C for up to 30 days, which is helpful for short trips.1

In practice, a pen holds four weekly doses, so a pen used on schedule is finished within its window.1 Trouble arises only when doses are delayed, skipped or spread out.

Keep the cap on between doses to protect the pen from light.1 Use the pen's own instructions for each injection, and rotate the injection site with every dose.1

05 · Freezing and heat

Freezing, heat and light: what ruins a pen

A frozen pen must never be used, even after it thaws. The product information is clear: never use a pen that has been frozen.1

Common ways pens freeze by accident include storage against the back wall of a fridge, next to an ice box, or in a cool bag with a frozen ice pack pressed against the pen. Wrap any ice pack in cloth, and keep the pen separate from it. Hold luggage on a flight can also fall below freezing, which is why pens go in hand luggage.13

Heat is the opposite risk. An in-use pen tolerates temperatures not above 30°C for up to 30 days, but it is not designed for a hot car, a sunny windowsill or a beach bag.1 If a pen has been somewhere hot, treat it as suspect and contact the pharmacy before injecting.

Light matters too. Keep the pen cap on to protect it from light.1

06 · Spotting a spoiled pen

How to tell if a Mounjaro pen has gone bad

Check the liquid, the date and the storage history before every new pen. The solution inside a genuine pen is clear and colourless.13 Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, or has been out of the fridge for more than 30 days, or above 30°C in use.13

Use this quick check:

  • Is the liquid clear and colourless, with no particles?
  • Is the printed expiry date still ahead of you?
  • Has the pen ever been frozen?
  • If it is in use, is it inside its 30-day window?
  • Does the box carry a batch number and expiry date from a registered pharmacy?

A pen can look perfectly normal and still be outside its limits, so appearance is only part of the test. The dates and storage history matter just as much. If you cannot answer yes to every question, do not inject, and contact your pharmacy.

Travelling with a Mounjaro pen

You can fly with Mounjaro, and most holidays need no special arrangements. Carry pens in hand luggage, ideally in a cool bag with the prescription label or a copy of your prescription.13 In-use pens tolerate up to 30 days not above 30°C, and for hot climates you should use an insulated pouch.13

08 · Stretching a pen

Can you stretch a pen or use it past its limit?

No, a pen should not be stretched beyond its design or its in-use window.

It is not licensed, not supported by the pen's design and not endorsed by any UK regulator.1 Altering doses outside the licence means unknown dosing accuracy and no safety data.

A spread-out schedule also collides with the in-use limit. A pen stretched across many weeks would pass its 30-day in-use limit while still holding medicine.

09 · Doses, timing and leftovers

Missed doses, changed days and leftover pens

Storage limits and dosing rules interact, so it helps to know the timing rules. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1

You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 There is no best time of day; Mounjaro works the same morning or evening, with or without food, and what matters is the same day each week.1

If you stop treatment or change dose with unused pens, do not sell or pass them on. Mounjaro is a prescription-only medicine dispensed for the named patient. Ask a pharmacy how to dispose of unwanted pens safely, and use the sharps bin supplied with your order for used needles.
