03 · What trials measured

What the trials measured, and what they did not

The main trial of tirzepatide for weight management ran for 72 weeks. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary.

It is tempting to read that as a recommended course length, but it is a study duration. Researchers pick a follow-up point so they can compare groups fairly. It does not mean treatment is meant to end at that point, and it does not mean nothing happens afterwards. Reading it as either would be a mistake.

What the figures do tell you is that results build over a long period. The full averages quoted in the headline were measured at 72 weeks, not at three months or six months. That is why people who judge the medicine by the first few weekly weigh-ins often feel disappointed, when the trend over months is the thing that matters.

The trial also reports how many participants reached particular thresholds. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 These numbers describe a group of people who stayed in the study, and they describe the outcomes of that study only.

Treatment also works alongside a reduced-calorie diet and increased physical activity, not instead of them, so your own result depends on much more than the pen.

04 · The titration phase

The dose ladder

Everyone starts on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself.1 After at least four weeks the dose increases to 5mg.

From there it can step up by 2.5mg at a minimum of four-week intervals, through 7.5mg and 10mg to 12.5mg, up to a maximum of 15mg weekly. Whether you move up depends on how you respond and tolerate it. Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace.1

This matters for duration because the ladder is the part of treatment with a built-in structure, with at least four weeks on each dose. After that, the length of treatment is open, and it depends on whether you settle on a dose, whether that dose works for you and whether your prescriber agrees to continue.

05 · The review point

The review point that shapes how long you continue

The clearest built-in decision point is the review after 6 months on the highest dose you can tolerate. Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1

Read that sentence carefully, because it is often misquoted. It says that at that point on a tolerated top dose, a prescriber looks at your response and has a conversation. If you have lost more than that threshold, continuing is a natural conclusion, and if you have lost less, the options are open and are discussed together.

The review also considers more than the scale. A good prescriber looks at side effects, how you are eating, whether the dose still suits you and whether you are still using treatment alongside a reduced-calorie diet and increased physical activity. If nausea or tiredness is dominating your week, that is relevant. If the dose is one you tolerate but you have not yet reached the top of the ladder, that is relevant too.

Can you take Mounjaro for only three months?

You can use Mounjaro for a short period if you and your prescriber agree, but a short course reaches only the early part of the dose ladder. Three months in, most people are still on the lower rungs, so trial-average losses at this stage are modest compared with the headline figures. In SURMOUNT-1, average weight loss built month by month, with the full 15–21% averages measured at 72 weeks, not 12.1

A sensible three-month expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation. Slower early progress does not predict failure, because the dose you respond to may still be ahead of you.1 That is a useful corrective if you were hoping a quick burst would deliver the trial headline.

Early loss also includes water weight, so the trend over weeks matters more than any single weigh-in. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.

If you are considering a short course, think about what you want from it. If the aim is to build a routine, learn how the medicine feels and see how you respond, a few months is a fair test. If the aim is to reach a specific large change, a short course is unlikely to get you there on its own, and you should be realistic about that with your prescriber.

07 · Staying on 2.5mg

Can you stay on 2.5mg and still lose weight?

The 2.5mg strength is a starting dose, not a treatment dose, and it is designed for adjustment rather than for weight loss itself. Everyone starts there so that the body can adjust to the medicine.1 Some people notice appetite change at this dose, and some lose weight early, but the licensed pathway moves on from it after at least four weeks.

That is why prescribers do not usually plan to keep someone at 2.5mg for the long term. Most measurable progress builds from 5mg onward, month by month.1 If you stay on the starting dose, you may well feel less of the appetite effect you were hoping for, and your prescriber would review whether that is the right plan.

That is a clinical decision, made with you, and it is different from deciding alone to stay on the lowest dose indefinitely.

Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better. So the useful question is not whether 2.5mg can work, but which dose is the lowest one that gives you a response you and your prescriber are happy with. That could be 5mg for some people and a higher strength for others.

08 · Plateaus and maintenance

Plateaus, maintenance doses and the question of stopping losing weight

Weight loss on Mounjaro is not linear, and plateaus of a few weeks are normal and expected as the body adapts to a lower weight. Most people do notice the pace of loss changing over a long course, which is why the question of whether you eventually stop losing weight comes up so often. The honest answer is that progress slows and levels off for many people, and that is a normal part of the process rather than a sign that treatment has failed.

Within the licence, there are clear steps to take. Review your food intake honestly, because portions drift back up. Protect protein and activity. Check the basics, such as sleep and alcohol. Then discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum.1 A plateau is not a reason to double doses or add unlicensed products.

This is also where the idea of a maintenance dose comes in. Many people settle at 5mg, 10mg or 15mg, depending on response and tolerance.1 The dose you settle on is the one you continue with while treatment is reviewed. 09 · Stopping Mounjaro

Stopping Mounjaro: what to expect and what to ask

If you decide to stop, tell your prescriber first, because the decision is best made as part of a review. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1

Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average.14 Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.

Useful questions to bring to that conversation include the following:

  • Which habits have changed, and which of them can I keep without the medicine?
  • Would stepping down within the licence be better than stopping outright?
  • What should I monitor in the weeks after the last dose?
  • When should I get in touch if something changes?

If you have been out of the routine, perhaps through a break in supply, the missed-dose rule matters. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day, and never take two doses within 3 days of each other.1 Check with your prescriber about restarting after a longer gap, because the right dose to resume on is a clinical decision.

Side effects are worth reporting whether or not you stop. If you notice anything new, discuss it with your prescriber and report it through the Yellow Card scheme19. 10 · NHS and private routes

NHS and private treatment: does the route change how long you stay on it?

The route you use affects how you get started more than how long treatment can last, and the honest picture is that access differs. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46

Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting. If you are unsure which group you fall into, your GP is the best first conversation.

11 · Practicalities over months

Practicalities that matter over a long course

A treatment that lasts many months needs a routine you can keep, and a few practical rules make that easier. Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food.1 There is no best time of day. What matters is the same day each week, and many people choose a weekend morning so early side effects fall on days off.

Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose. You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.

Storage is the other long-term habit. Keep pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, which is useful for short trips, after which it should be discarded. Keep the cap on to protect it from light, and never use a pen that has been frozen.1

Travel often comes up on a longer course. You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. Most holidays need no special arrangements, and for hot climates use an insulated pouch. Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit.

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
