What is well established is how the medicine behaves in the body. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1 A long half-life means a once-weekly dose lingers and levels shift gradually, rather than switching on and off.

Others notice nothing until a later dose. None of these patterns predicts how well treatment will work for you, so do not read too much into a quiet first week or a rough one.

Mounjaro works by activating two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 Slower stomach emptying is the main reason digestive symptoms such as fullness, nausea and burping show up early.

05 · Dose increases

Why side effects return when the dose goes up

Side effects are most likely when starting treatment or moving up a dose, which is why they often return after a step up.1 After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1

Each increase gives your body a bigger dose of the same medicine, so a familiar pattern can repeat: effects in the first days, an early peak, then settling. You may read that effects peak within a precise number of hours and ease by a particular day of the week. Some people do find their weekly rhythm looks like that, but it is a personal pattern, not an assured schedule. If yours differs, that is not a sign something is wrong.

How common are the stomach effects? In the trials supporting Mounjaro's weight-management licence, nausea was reported by 24.6% of people on 5mg, 29.0% on 10mg and 28.0% on 15mg, compared with 8.5% on placebo.2 Diarrhoea was reported by 18.7% of people on 5mg, 20.8% on 10mg and 22.5% on 15mg, compared with 7.8% on placebo.2 Individual results vary.

Most people carry on despite these effects. 1.9% of people on 5mg, 3.3% on 10mg and 4.3% on 15mg stopped treatment permanently because of gastrointestinal side effects, compared with 0.5% on placebo.2 A small minority, in other words, found the effects unmanageable.

What are the common side effects of Mounjaro?

Like all medicines, Mounjaro can cause side effects, and most are digestive. The most common — affecting more than 1 in 10 people — are nausea, diarrhoea, vomiting, constipation and abdominal discomfort.1 Common effects (up to 1 in 10) include indigestion, reduced appetite, dizziness, reflux, burping, hair shedding and injection-site reactions.1

Timing differs a little between them:

  • Nausea, burping and reflux tend to arrive early, soon after dosing, because the stomach empties more slowly.
  • Diarrhoea and constipation can appear in the first days of a new dose and may alternate for some people.
  • Fatigue is most noticeable in the first weeks and after dose increases.1
  • Injection-site reactions show up at the site itself, often soon after the injection.
  • Hair shedding comes later, because it is linked to weight loss rather than to each individual dose.

Fatigue usually reflects a mix of the body adjusting and simply eating less. Keep meals balanced (protein especially), stay hydrated, and keep gentle activity going. For most people energy returns to normal within a few weeks; tell your clinical team if tiredness is severe or not settling.1

Because the stomach empties more slowly, a large or rich meal can sit heavily. Many people find that the old habit of finishing the plate no longer works. Listen to the sense of fullness, and do not force the next mouthful.

Constipation, the other early complaint, is very common on Mounjaro for weight management (it may affect more than 1 in 10 people), because food moves through the gut more slowly and many people eat and drink less.1 Fluid intake comes first, then fibre from vegetables and whole grains, and daily movement. If it persists, a pharmacist can recommend a suitable laxative; contact your clinical team for pain, bloating or more than a few days without a bowel movement.1

09 · Injection site and hair

Injection-site reactions and hair shedding: different timelines

Injection-site reactions come quickly and fade quickly, whereas hair shedding is slower and tied to weight change. Mild redness, itching, a small bruise or a tender lump at the injection site is common and normally settles within a few days.1

Rotating sites each week and letting the pen warm briefly out of the fridge reduces stinging. Contact your clinical team if a reaction is spreading, hot, very painful, or lasts more than a week.1 Injection-site reactions were reported by 8.0% of people taking tirzepatide in the weight-management trials, compared with 1.8% on placebo.2

Hair loss was reported by 4.9% of people taking tirzepatide in the weight-management trials, compared with 1.0% on placebo.2 Temporary hair shedding is a recognised side effect, linked to rapid weight loss itself rather than a direct drug effect. For most people it settles as weight stabilises. Protein intake and overall nutrition matter.1 Because it follows weight loss, it does not appear in the first days the way nausea does.

10 · Serious symptoms

Side effects that need urgent help, whenever they start

Serious side effects are uncommon but need urgent attention, and they should never be put down to normal adjustment. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis).13

Seek immediate medical help for pain in the upper right abdomen, fever or yellowing of skin or eyes (possible gallbladder problems), or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.13 If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.13

Mood matters too. The EMA's safety committee reviewed the evidence and concluded in 2024 that it does not support a causal link between GLP-1 medicines and suicidal thoughts, and the MHRA keeps this under continuous review.17 Even so, any big change in weight, appetite and routine can affect how you feel. Tell your prescriber about any history of depression, and seek help promptly if your mood drops or you have distressing thoughts during treatment.

11 · Other GLP-1 medicines

Do other GLP-1 medicines follow the same pattern?

Broadly yes: side effects from GLP-1 medicines are mostly digestive and cluster around starting and increasing the dose. Mounjaro and semaglutide both slow stomach emptying and affect appetite, so early nausea and bowel changes are shared themes.1

The detail differs by product. Wegovy is also injected once weekly, with pens of 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg, and its dose steps up after at least 4 weeks, reaching the 2.4mg maintenance dose from week 17.18 Each step is another point where effects can return.

12 · Talking to your team

When to talk to your prescriber about timing

Talk to your prescriber whenever side effects are more intense, longer lasting or later in arriving than expected. A keen eye on timing gives them useful information. Keep a simple note each week: injection day, symptoms, how strong they were, and how long they lasted.

Most side effects follow a pattern: they appear in the first days after starting or increasing a dose, peak early, and settle as your body adapts. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time. If more than 4 days have passed, skip the missed dose and take the next one on your normal day — never take two doses within 3 days of each other. Store pens in a fridge at 2–8°C and never freeze them; once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away.1
