How skinny people are getting Mounjaro, and why it is a problem

Slim people are not getting Mounjaro through a legitimate route. Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or between 27 and 30 with at least one weight-related condition such as high blood pressure, prediabetes or type 2 diabetes.1 A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable.11

Where thin people are getting it, the routes are the ones to avoid: sellers offering to supply without a consultation or prescription, inaccurate information given to a provider, or leftover pens passed on by others. Pens from these sources may be counterfeit. Only buy from a pharmacy you can verify on the GPhC register, and report suspected fakes through the MHRA.7

03 · Risks if you are slim

Why it is risky at a healthy weight

Mounjaro works by activating GIP and GLP-1 receptors, helping you feel fuller sooner and slowing stomach emptying.1 That is helpful when weight loss is medically indicated, but reduced appetite is a poor fit if you have little to lose.

The most common side effects, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort. Hair shedding and fatigue are also recognised; hair shedding is linked to rapid weight loss itself rather than a direct drug effect.13 Severe, persistent stomach pain that may spread to the back, with or without being sick, could be pancreatitis: stop using Mounjaro and get medical help straight away.

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

NHS access

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS rollout began in June 2025 with the highest-need groups first, so it is not an NHS option for slim people either.4

What is the lowest BMI for Mounjaro?
For weight management, Mounjaro is licensed from a BMI of 27, and only when you also have at least one weight-related condition such as high blood pressure or prediabetes. Without such a condition the threshold is a BMI of 30. A prescriber reviews your history before deciding.
What happens if a thin person takes GLP-1?
The medicine still reduces appetite and slows stomach emptying, so a thin person may experience nausea, fatigue and hair shedding without a medical benefit. It is not licensed for this use, which is why we do not supply it outside the criteria. Speak to your GP if you are concerned.
Why would you be refused Mounjaro?
A prescriber may decline an order if your BMI and conditions fall outside the licensed criteria, or if your medical history or current medicines make it unsuitable. If a consultation is declined, you are refunded automatically in full.
Who is Mounjaro not suitable for?
It is not suitable for adults outside the licensed BMI criteria, or where your history or medicines make it inappropriate. A prescriber assesses this individually. Mounjaro should not be used if you are allergic to tirzepatide or any of its ingredients, or during pregnancy or while trying to conceive; stop it at least one month before trying to conceive. Your prescriber takes extra care with a history of pancreatitis, severe gut disease, diabetic retinopathy or gallbladder problems. If you take the oral contraceptive pill, the SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1
What drugs should not be taken with Mounjaro?
Check every medicine with your prescriber. If you take insulin or a sulfonylurea, watch for low blood sugar, as those doses may need adjusting. Oral contraceptives can be less well absorbed: switch to a non-oral method, or add a barrier method such as condoms, for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase. Tell the prescriber about everything you take, including over-the-counter products.
Why can't I eat much while taking Mounjaro?
Mounjaro helps you feel fuller sooner and slows how quickly the stomach empties, so smaller meals feel like enough. Eat slowly, stop at the first feeling of fullness and stay hydrated. If nausea or vomiting is persistent or you cannot keep fluids down, contact your clinical team — dehydration needs prompt attention.
How long do people typically take Mounjaro?
It varies. In SURMOUNT-1, results were measured over 72 weeks, and individual results vary. Tirzepatide has a half-life of about five days, so it takes roughly 4–5 weeks to clear after a final dose. Your prescriber reviews your treatment regularly.
