GPs and Financial Incentives to Prescribe Weight Loss Medications (October 2026)
GP practices in England are being offered contract funding linked to weight management, and the money goes to the practice for meeting set indicators, not to an individual GP for each prescription. Those indicators are about recording BMI, referring patients and appropriate prescribing. Your own eligibility for NHS Mounjaro still depends on NICE criteria. Position as of October 2026.

A quick overview
What to know
before you start.
-
Contract incentives reward practices for meeting indicators such as BMI recording and referrals, not for each prescription written.
-
Whether you qualify for NHS Mounjaro depends on NICE criteria and the phased rollout, not on any practice incentive.
-
NICE recommends Mounjaro for adults with a BMI of at least 35 plus a weight-related condition; the rollout started with the highest-need groups.
-
If you do not yet qualify on the NHS, a regulated private prescriber review is another route; across the UK pharmacies we track, pens range from £143.96 to £339.00 a month.
In this guide
01 · The short answer
Are GPs really paid to prescribe weight loss medications?
Practices, not individual doctors, are the target of the funding, and it is tied to meeting indicators rather than a fee per prescription.
The indicators are broadly three: better diagnosis and recording of BMI, more referrals into formal weight management services, and appropriate prescribing for eligible patients.
The funding cannot change the clinical criteria. A prescriber still has to decide a medicine is suitable for you, and NICE guidance still sets who the NHS should offer it to. For a wider look at how bonus payments are described, see our guide on GP practice bonuses for weight loss drugs.
02 · For patients
What the incentives mean for you as a patient
For most people the practical effect is a possible improvement in how consistently practices identify and offer treatment, not a change in whether you personally qualify. Funding that rewards recording BMI and referring patients may make a conversation with your GP more likely to happen. It does not make Mounjaro available to everyone who asks.
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria consider regulated private treatment instead of waiting.
If you are weighing this up, three practical steps help:
- Ask your GP whether you fall into a currently prioritised group, and whether your BMI and conditions are recorded accurately.
- Ask whether a referral to a weight management service is available locally.
- Ask honestly about timescales, because phased access means the answer may be a waiting period.
Our guide on whether GPs can prescribe weight loss injections covers what to expect from that conversation.
03 · If you do not qualify yet
The private route, and what it costs
If the NHS cannot treat you yet, a private prescriber review is a regulated alternative, and you can start with a free online consultation. Mounjaro is a prescription-only medicine, supplied only after an online consultation reviewed by a GPhC-registered prescriber.1 Our consultation is free, and unsuitable orders are refunded automatically.
Across the UK pharmacies we track (checked 12 October 2026), a Mounjaro pen costs between £143.96 and £339.00 per month depending on dose and provider. For the 2.5mg starting pen the range is £143.96 to £179.99. The gap between providers for the same dose runs from £27 to £41, so comparing is worth the effort.
At EverydayMeds the standard price of the Mounjaro pen is £169.99 at 2.5mg, with all six doses in stock. Your first 2.5mg pen costs £129.99 with code FIRST40, and every later order is £25 off the standard price (code DOSE25). Prices checked 12 October 2026. You can start your free consultation with no obligation.
04 · How GP funding works
Do GPs get paid for prescribing drugs in the UK?
GP practices are funded through a national contract, and a part of that funding rewards practices for meeting defined quality standards.
The same question comes up for other medicines, including statins. The answer follows the same logic: practices are rewarded for meeting indicators across a population of patients, such as how many eligible people are offered treatment. Whether any individual clinician benefits personally depends on how the practice is run, which is a matter for the practice and its contract.
Why does that matter? Prescribers are bound by professional standards and by the clinical criteria for each medicine. A reasonable test for any recommendation is whether it matches published criteria and whether your prescriber has reviewed your medical history and current medicines.
05 · NHS eligibility in 2026
Who is eligible for NHS weight loss injections?
NICE recommends Mounjaro for adults with a BMI of at least 35 plus at least one weight-related condition, and access is being phased in.4 The primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.6 Full rollout is expected to take years.
Your GP or integrated care board can tell you which stage your area has reached.
NHS criteria are narrower than the licence. The licence is wider, and that difference is why some adults are clinically suitable for treatment but are not yet in line for NHS supply.
06 · Licensed criteria
Licensed eligibility for private prescribing
Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.1 It must be used alongside a reduced-calorie diet and increased physical activity.
A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable. Because Mounjaro slows stomach emptying, it can affect how quickly some oral medicines are absorbed, particularly when you start or increase the dose. Tell your prescriber about everything you take, especially warfarin or other narrow-margin medicines, insulin, or sulfonylureas, where doses may need reducing to avoid low blood sugar. No adjustment is needed for paracetamol.1
For a broader view of who can prescribe and which medicines are used, see what medications doctors prescribe for weight loss. Other professionals have their own limits too: for example, whether dieticians can prescribe weight loss medication.
07 · What Mounjaro does
What Mounjaro is and how it works
Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly, licensed in the UK for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1
Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. Most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 This dual action distinguishes it from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic).
Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4 to 5 weeks to clear the system.1 Nothing needs doing while it clears and there is no way to speed it up.
08 · What the trials show
How much weight can you lose on Mounjaro?
In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages and individual results vary. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.
At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 As a worked example, for someone starting at 100 kg, 20.9% is about 21 kg.
Three months in, most people are still on the lower rungs of the dose ladder, so trial-average losses at this stage are modest compared with the headline figures: in SURMOUNT-1, average weight loss built month by month, with the full 15–21% averages measured at 72 weeks, not 12. A sensible 3-month expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation. Slower early progress does not predict failure — the dose you respond to may still be ahead of you. Your prescriber will review your progress at each stage of your dose.
09 · Side effects and safety
Side effects, other organs and reporting
The most common side effects, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort. Common effects (up to 1 in 10) include indigestion, reduced appetite, dizziness, reflux, burping, hair shedding and injection-site reactions. Side effects are most likely when starting treatment or moving up a dose, and for most people they ease within days to a few weeks as the body adjusts.13 Tell your prescriber about any new or lasting symptom. You can report suspected side effects through the MHRA Yellow Card scheme.19
If you have kidney, liver or other conditions, say so in your consultation so your prescriber can weigh suitability for you.
Ongoing monthly reviews, dose titration and follow-up assessments are included with our service, with side-effect support from the prescribing team. Our support team is available Monday to Friday, 9am to 5pm, on 0333 207 0413.
10 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
11 · Staying safe buying privately
Buying safely and ordering with us
Counterfeit weight loss pens are a real risk, so buy only from a registered pharmacy. The MHRA publishes counterfeit medicine alerts, and you can check any pharmacy on the GPhC register.712 Be wary of anyone offering to supply without a consultation or prescription, or of offers that promise specific results. EverydayMeds is GPhC-registered (9012878) and supplies Mounjaro through official LillyDirect supply, direct from Eli Lilly's UK distribution.
Ordering works in three steps:
- Complete the free online consultation, a short health questionnaire with no charge and no obligation.
- A GPhC-registered prescriber reviews it. Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full.
- Your order is dispensed by our registered pharmacy, with same-day dispatch when ordered by 3pm, then free next-day tracked delivery in temperature-controlled packaging.
To compare options, see buy Mounjaro online, or our overview of all weight loss treatments. If cost is your worry, read about financial assistance for Mounjaro. A first year costs £2,711.88 at standard prices if you settle at 5mg, up to £3,599.88 at 15mg; a new customer's first order is £40 less and every later order £25 less.
