Should you build a GLP-1 stack? The honest answer
For most people the answer is no: you do not need a stack, and some stacks are unsafe. If you are considering weight-loss treatment, the decision is between one properly prescribed licensed medicine and nothing at all. It is not between a licensed medicine and a cocktail of extras.
The term stack comes from bodybuilding and peptide-wellness forums. There it means layering several compounds to chase a bigger effect. Medicine does not work like that. Every licensed treatment has been tested on its own, at set doses, in a defined population, with known side effects and a monitored supply chain. When you add a second active ingredient, you step outside everything that testing told us.
Mounjaro (tirzepatide) is the clearest example. It is a once-weekly injectable licensed in the UK for weight management in adults and for type 2 diabetes.1 Its mechanism already covers GIP and GLP-1 receptors, so there is no gap that another GLP-1 product would fill. Layering on top of it adds risk without a proven benefit.
The two decisions you are actually facing
Decision one: is treatment relevant to me? Licensed weight-management medicines are for adults who meet clinical criteria, assessed by a prescriber. NICE recommends Mounjaro for adults with a BMI of at least 35 plus at least one weight-related condition.4 Your own situation may differ, which is why a prescriber reviews each person individually.
Decision two: what support is worth adding? The licensed approach is treatment alongside a reduced-calorie diet and increased physical activity.1 Anything beyond that, such as a protein shake or a vitamin, is a discussion for your prescriber rather than something to improvise from social media.

02 · What stacking means
What people mean by a GLP-1 stack
A GLP-1 stack is an informal label for combining a GLP-1 receptor medicine with other products to try to improve results. The label has no clinical definition, and different websites use it for very different things.
In practice, the word covers four loose groups. The first is the core medicine itself, such as semaglutide or tirzepatide. The second is what wellness sellers call secondary peptides, which are unlicensed compounds marketed to protect muscle or boost fat loss. The third is supplements, meaning low-cost products sold as natural alternatives or add-ons. The fourth is nutritional support, such as protein-rich meals and shakes used when appetite is low.
These groups carry completely different levels of risk. Nutritional habits are everyday choices that you can discuss with your prescriber. Supplements are cheaply available without assessment, and their dosing is not standardised. Unlicensed peptides and duplicate GLP-1 medicines sit at the dangerous end, because nobody has tested them together with the licensed medicine.
Why the term spreads so quickly
Social media rewards bold claims. A post that says a combination is faster, stronger or smarter travels further than one that says to take a single licensed medicine as prescribed. Sellers of research peptides and so-called shred stacks benefit from that attention. A useful test
Ask three questions about any product in a so-called stack. Is it licensed in the UK for this purpose? Has it been tested together with the medicine you take? Is it supplied by a regulated pharmacy after a prescriber review? If the answer to any is no, it is not part of a safe plan. Mounjaro itself is a prescription-only medicine, supplied after an online consultation reviewed by a GPhC-registered prescriber, and it is used alongside a reduced-calorie diet and increased physical activity.1
Wanting the best result from treatment is reasonable. The safest way to get it is to take the medicine as prescribed, keep your reviews and put effort into food and activity rather than into adding unproven products.
03 · Why combining is unsafe
Why combining GLP-1 medicines is not safe
Combining GLP-1 medicines is unsafe because no one has tested the combination. These combinations have no safety data and no regulated supply chain. If something promises faster results outside a licensed pathway, that is the warning sign.
The mechanism already covers the target
Tirzepatide activates the GIP and GLP-1 receptors. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. Because Mounjaro already acts on the GLP-1 pathway, adding semaglutide or liraglutide on top is duplication, not complementary action.
Because Mounjaro slows stomach emptying, combining it with other products can affect how quickly some oral medicines are absorbed. Stacking two products with the same effect has no safety data behind it.
Dose is already built in
Mounjaro is designed to be increased step by step through six strengths, from 2.5mg to 15mg, so that the dose can be tailored to the person.1 Your prescriber decides when to move up, based on how you are responding and tolerating it. Adding another product to chase a bigger effect bypasses that careful sequence.
What the trials tested
The efficacy figures people quote come from trials of one medicine at defined doses alongside diet and exercise. In the SURMOUNT-1 trial, people took tirzepatide with lifestyle changes and were compared with lifestyle changes alone.2 Those results say nothing about what happens when other compounds are added, so quoting them in support of a stack is misleading.
If you take other medicines for diabetes, tell your prescriber. Insulin and sulfonylureas may need dose adjustments because of the risk of low blood sugar. That is a supervised adjustment, not a reason to experiment.
04 · Research peptides and copies
Research peptides, compounded copies and counterfeit risk
When a seller names a compound such as retatrutide, remember that it has no UK licence, and we do not suggest anyone use it.1
What no regulated supply chain means
With a licensed medicine, the pen comes from the manufacturer through authorised distribution, in the right strength, with a patient leaflet, batch information and cold-chain storage. With a research peptide or compounded copy, you rely on the seller's word. Many such sellers use dramatic weight-loss claims to attract buyers. Treat those claims as marketing, not evidence.
The MHRA publishes counterfeit medicine alerts and runs the Yellow Card scheme for reporting suspected side effects and fake products.7 If you have bought something and feel unwell, speak to a healthcare professional and report it through Yellow Card. If you have severe, persistent stomach pain, pain in the upper right abdomen with fever or yellowing of the skin or eyes, or swelling of the face or throat and difficulty breathing, get medical help straight away.
How to spot a safe supplier
A legitimate UK online pharmacy appears on the GPhC register, requires a consultation reviewed by a prescriber, and dispenses a licensed product. Warning signs include a seller offering to supply without a consultation or prescription, prices that look far too low, vague origin, no pharmacy registration number and promises of specific results. The same caution applies to unlicensed injectable products that call themselves a GLP-1 but are sold without prescriptions. The safest rule is plain: if it needs a prescription and the seller does not ask for a consultation, walk away.
05 · Supplements and natural claims
Supplements, "natural GLP-1" claims and appetite tablets
Supplements marketed as natural GLP-1 boosters are not licensed weight-loss medicines and cannot be treated as equivalent to them. They are low-cost supplements with no clinical oversight, and their dosing is not standardised. That is the honest summary of an area full of exaggeration.
Nature's Ozempic and similar labels
Phrases such as "nature's Ozempic" borrow the credibility of a licensed medicine for a product that has not been through the same testing. If a seller quotes dramatic results, ask what trial they come from, how long it ran and who was in it. Often there is no answer.
Boots Appetite Control Tablets and similar products
People often ask whether appetite-control tablets from high-street retailers work. If you are comparing, the useful questions are what the active ingredient is, what it is licensed for and whether anyone reviews your suitability. Why the price gap matters
Cost is the main attraction of supplements. But a lower cost is a framing point, not a reason to believe a product works. A licensed medicine reflects development, trials, quality controls and pharmacy oversight. Food
Many people find they feel fuller sooner and less hungry between meals, which makes a reduced-calorie diet easier to follow. That is also the moment to think about food quality, because smaller portions leave less room for nutrients. Build meals around foods that you tolerate well and that keep you satisfied. If you find eating hard, or you feel unwell after meals, tell your prescriber rather than trying to fix it with a product from the internet.
Meal replacements and shakes are a common part of what the internet calls a stack. They are an everyday food choice rather than a medicine, and whether one suits you is a question for your prescriber or a dietitian. Activity
Increased physical activity is part of the licensed use. Build it gradually and choose something you will keep doing.

07 · Results, stopping and switching
Results, stopping treatment and switching medicines
Stopping a GLP-1 medicine means the drug leaves your system gradually. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 Plan any change with your prescriber rather than on your own.
What the trial shows
In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them. Trial averages do not predict your own result, and they say nothing about stacks.
Is Mounjaro the best GLP-1?
It is tempting to ask which medicine is strongest. The honest answer is that strength is not a single measure. Mounjaro activates both GIP and GLP-1 receptors, which distinguishes it from single-action medicines such as semaglutide.1 SURMOUNT-5 was the first large head-to-head trial of tirzepatide and semaglutide for weight loss.15 The right medicine is the one a prescriber judges suitable for you.
Switching, not stacking
If your current medicine is not working for you, the answer is a prescriber-led switch, not adding a second product. Wegovy (semaglutide) is a once-weekly alternative whose pens run 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg, stepping up after at least four weeks to the 2.4mg maintenance dose from week 17.18 You can see the Wegovy pen page, where your first order is £99.99 as a first-pen price. Liraglutide, by contrast, is a daily injection supplied at 6mg per millilitre.

08 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

Cost, NHS access and how to order safely
Safe treatment can be reached through the NHS or a regulated private pharmacy. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
What private treatment costs
Each pen lasts four weeks. At standard prices, the 2.5mg starter pen is £169.99, 5mg is £218.99, 7.5mg is £279.99, 10mg is £290.99, 12.5mg is £319.99 and 15mg is £329.99.
