Can GLP-1 medicines help you stop smoking?

They are not an approved way to stop smoking, so you should not treat them as one. The UK licence for Mounjaro covers weight management in adults and type 2 diabetes.1 Nicotine dependence is not on that list, and we do not prescribe for it.

You will see headlines about semaglutide and tobacco, and about GLP-1 and addictive behaviours in general. Those headlines describe early research questions.

02 · Your decision

What should you do if you want to quit?

Start with a proven route, not an off-label experiment. Your local NHS stop-smoking service, your GP and your community pharmacist can all set up support and licensed stopping aids.

If your real concern is weight, that is a separate question. Mounjaro is a licensed weight-management treatment for adults who meet the criteria, and it works alongside a reduced-calorie diet and increased physical activity.1

03 · Why the question is asked

Why GLP-1 and smoking are being linked

Researchers are looking at whether GLP-1 medicines might affect cravings for food, alcohol and nicotine. It is a hypothesis, not a proven effect in people trying to quit.

Tirzepatide activates two natural gut-hormone receptors, GIP and GLP-1, which help regulate appetite, slow how quickly the stomach empties and improve how the body responds to insulin.1 Most people feel fuller sooner and less hungry between meals. That describes appetite, not nicotine cravings, and the licence reflects it.

It is worth being clear about the gap. A medicine that reduces hunger might also change other urges, but that is not established, so nobody should rely on it. If you notice any change in cravings, mood or habits on treatment, mention it to your prescriber rather than drawing your own conclusions.

04 · Weight gain after quitting

Weight gain after quitting smoking

Fear of putting on weight stops many people trying to quit, and it is a fair concern to raise with a clinician. Appetite often changes when people stop smoking, and some people eat more as a habit replacement.

This is where the GLP-1 idea gets muddled. A weight-management medicine and a stop-smoking aid do different jobs. Using Mounjaro to offset post-quitting weight gain would mean taking a prescription medicine for a purpose outside its licence, in someone who may not meet the weight criteria at all.

If you are in the range where treatment is appropriate, the licensed use is weight management in adults, alongside diet and activity.1 Eligibility for private treatment is decided by a prescriber, not by whether you are quitting smoking.

One further point matters if you ever start and then stop. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued lost a further 5.5% on average.14 Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.

Practical steps for weight during a quit attempt are mostly about routine: regular meals, planned snacks, and movement.

05 · Activity and habits

Movement as a quitting ally

Physical activity helps with both weight and the restlessness many people feel when they stop smoking. Increased physical activity is part of the Mounjaro licence, not an optional extra.1 Nothing special is required: the evidence-backed pattern is regular moderate activity such as brisk walking, plus resistance work for muscle.

Start where you are and build. On injection days with early side effects, lighter movement still counts. Exercise also protects against regain if you later stop treatment.

It gives your hands and your mind something to do, and it needs no equipment. Pairing a quit date with a simple routine is more realistic than trying to change everything at once.

06 · Alcohol and other cravings

Alcohol, cravings and GLP-1

People often ask whether GLP-1 medicines curb alcohol cravings as well. Reports in the news describe research interest, not an approved benefit.

What about drinking while on treatment? There is no direct interaction between Mounjaro and alcohol, but drinking on a reduced appetite and emptier stomach can hit harder than usual. If you also take insulin or a sulfonylurea, alcohol increases the risk of low blood sugar.

If you are concerned about your drinking or think you may be dependent, speak to your GP or an NHS alcohol service. We do not want anyone to rely on a weight-loss pen for a problem that needs proper support.

07 · Safety and what to report

Safety, side effects and reporting

Any new symptom while on treatment deserves a conversation. If you are quitting smoking at the same time, it can be hard to tell which change comes from which. Withdrawal can affect sleep, mood and appetite, and a medicine can also do so.

Do not adjust your own dose to see what happens.

In SURMOUNT-1, 4.3% of people on 5mg, 7.1% on 10mg and 6.2% on 15mg stopped treatment because of side effects, compared with 2.6% on placebo.2 If side effects bother you, your clinical team can help.

You can report a suspected side effect through the MHRA Yellow Card scheme.19

08 · How to read the headlines

How to read GLP-1 smoking headlines

Articles on this topic tend to follow a pattern: an early finding, a plausible mechanism, then a leap to "could help you quit". Treat each step separately. A signal in a database is not the same as a controlled trial, and a controlled trial in one group does not automatically apply to you.

Ask four questions of any claim. Was the medicine being tested for smoking, or was smoking noticed incidentally? Were the people similar to you? Is the result about quitting, or only about cutting down? And is the medicine licensed for the use being discussed? For GLP-1 and smoking in the UK, the last answer is no.

Be wary of sellers who promise specific results or offer to supply without a consultation or prescription. Genuine Mounjaro comes through a regulated pharmacy; you can check ours on the GPhC register (registration 9012878).

09 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

10 · NHS access

NHS access and stop-smoking support

NHS stop-smoking support is available now if you want to quit. Ask your GP or pharmacist how to be referred.

Separately, Mounjaro is available on the NHS for weight management, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.56 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

Neither route is a smoking-cessation treatment. They sit side by side: one for quitting, one for weight.

Can GLP-1 receptor agonists help with smoking cessation?
They are not licensed for it in the UK. Mounjaro is licensed for weight management and type 2 diabetes, and researchers are still exploring whether GLP-1 medicines affect nicotine cravings. Nobody should plan a quit attempt around them. Established NHS stop-smoking services and licensed aids, accessed through your GP or pharmacist, are the proven route.
Does Mounjaro stop you smoking?
Mounjaro is not licensed to stop smoking and we do not prescribe it for that purpose. Its licensed uses are adult weight management and type 2 diabetes. Some people do, which is why concern about weight is common. It is not inevitable. Routine, regular meals and activity such as brisk walking help. If weight is a medical concern, ask a clinician whether licensed weight management is appropriate for you, rather than starting a medicine to offset quitting.
Should I tell my prescriber that I smoke?
