What to do if you think you have food poisoning on Mounjaro

Start with fluids and a call to your clinical team if things are not settling. Drinking enough matters more on Mounjaro than usual: reduced appetite means less fluid from food, and vomiting or diarrhoea can dehydrate you quickly, which is the main route to the rare kidney problems reported with this class.13

Keep water within reach and take small, frequent sips rather than large gulps. If you cannot keep fluids down, contact your clinical team — dehydration needs medical help straight away.

If you take insulin or a sulfonylurea, tell your prescriber as well, because doses may need adjusting when you are ill and eating little (low blood sugar risk).1

Fever or blood in your stools are not typical of treatment side effects, so tell your prescriber if you notice either. 04 · Why a bug feels different

Why a stomach bug can feel different on Mounjaro

Tirzepatide activates the GIP and GLP-1 receptors, which helps regulate appetite and slows how quickly the stomach empties.1 Food therefore sits for longer, and large portions already feel heavy.

That is why an infection on top of treatment may feel more intense or unfamiliar.

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears. Stopping because you feel ill will not clear it quickly, so speak to your prescriber instead.1

No food is banned on Mounjaro, but many people feel worse after very fatty, fried or very sugary food.13 While recovering from a bug, plain and small is sensible. Smaller, slower meals work better because the stomach empties more slowly.

Does food poisoning count as weight loss?

Food poisoning can lower the scale number briefly, but it is not a treatment strategy. Mounjaro works alongside a reduced-calorie diet and increased physical activity, not through illness.2 In SURMOUNT-1, people on tirzepatide lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages and individual results vary.2

Eating very little while ill makes protein and muscle harder to protect, so return to regular protein-containing meals as soon as you can manage them.13

Standard food hygiene applies exactly as it does for everyone: keep chilled food cold, cook meat thoroughly and wash hands and surfaces.

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
