Yes, some people report feeling tired or sleepy while on Mounjaro, and it is worth taking seriously rather than dismissing. Feeling tired is a very common side effect in the UK patient leaflet, affecting more than 1 in 10 people.13

What matters in practice is that tiredness rarely has a single cause. Mounjaro changes appetite, so many people eat noticeably less. Your sleep, hydration, stress, mood and other health conditions also affect energy. Sorting out which factor is driving your own sleepiness is the useful step.

It is relevant if you have started Mounjaro, moved up a dose, or are thinking of starting and want to know what to expect. Treat tiredness that is mild and settles within the first few weeks differently from tiredness that is heavy, persistent or gets in the way of driving or work.

03 · Why tiredness happens

Why you might feel tired on Mounjaro

Several explanations are commonly discussed, and more than one can apply at the same time. None of them replaces a conversation with your prescriber.

Eating much less. Tirzepatide activates GIP and GLP-1 receptors, which helps regulate appetite and slows how quickly the stomach empties. Most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 If appetite falls so far that meals become very small or skipped, low energy would not be surprising.

Dehydration. Feeling sick, being sick or having loose stools can leave you short of fluid, and that alone can make you drowsy and headachy. Our guide to what makes you sick on Mounjaro covers the digestive side.

Disturbed sleep. Stomach discomfort, heartburn or nausea at night can break up your sleep, so you wake unrefreshed even after a full night in bed.

Other causes. Stress, shift work, low mood, thyroid problems, anaemia, other medicines and sleep disorders can all cause sleepiness, whether or not you take Mounjaro. That is why timing and pattern matter when you describe it to a clinician.

04 · Sleep apnoea link

Sleep apnoea and daytime sleepiness

If you were sleepy before treatment, obstructive sleep apnoea is worth considering, because it fragments sleep and is a well-known cause of daytime drowsiness.

In the SURMOUNT-OSA trials, tirzepatide substantially reduced the severity of obstructive sleep apnoea in adults with obesity. On average, breathing interruptions fell by around half or more over a year, and some participants improved enough to no longer need the same level of treatment.16 Obstructive sleep apnoea also counts as a weight-related condition in the eligibility criteria.

If you use CPAP, continue it and keep your sleep clinic informed.16 If you snore heavily, wake gasping or a partner notices pauses in your breathing, raise it with your GP or prescriber.

05 · What may help

How to manage tiredness on Mounjaro

The most useful first step is to look at the basics, then involve your prescriber if they do not help.

  • Keep eating regular, balanced meals even when you are not hungry. Small portions are fine; skipping meals for long stretches is a likely route to low energy.
  • Drink fluids steadily through the day, especially if you have had sickness or loose stools.
  • Protect your sleep with a regular bedtime, a cool dark room, and a light evening meal if night-time reflux or nausea disturbs you.
  • Stay gently active. Treatment is meant to run alongside increased physical activity, and light movement often helps energy better than resting.
  • Note the pattern. Write down when you inject, when you feel worst and what you have eaten. A simple record, or the free GLPTrackr companion app, makes the conversation with your prescriber far more useful.

If sleepiness is making driving or operating machinery feel unsafe, do not do either until you have taken advice.

06 · How long it lasts

How long does the tiredness last?

For most people energy returns to normal within a few weeks. For many people any tiredness eases as the body adjusts to a dose, but that is not true for everyone, and it can return after a step up.

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.2 That means if you stopped, any effect would not vanish overnight. It is also a reason not to stop on your own: talk to your prescriber first, because they may be able to adjust your dose or pace instead.

If tiredness has lasted well beyond the first few weeks on a stable dose, treat it as something to investigate rather than something to wait out. A blood test or a review of other medicines may be worthwhile.

07 · When to seek advice

When to seek medical advice

Speak to your prescriber or GP if tiredness is heavy, persistent or worsening, or if it is affecting your safety, work or daily life. Contact them sooner if it comes with other new symptoms such as repeated vomiting, an inability to keep fluids down, dizziness or a marked change in mood. For fainting, get medical help straight away.

Get medical help straight away for sudden severe symptoms, such as collapse, chest pain, difficulty breathing or confusion. Do not wait for a routine review in those situations.

08 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

How long does tirzepatide fatigue last?

Take it seriously if tiredness is sudden, severe, getting worse, or comes with other new symptoms such as faintness, repeated vomiting, inability to keep fluids down or confusion. Speak to your prescriber or GP promptly, and get medical help straight away for sudden severe symptoms, chest pain, fainting or difficulty breathing. Persistent fatigue also deserves a check for other causes, such as sleep apnoea.
Could sleep apnoea be making me sleepy on Mounjaro?
It could be. Obstructive sleep apnoea fragments sleep and causes daytime sleepiness. In the SURMOUNT-OSA trials, tirzepatide substantially reduced its severity in adults with obesity.
