The kidney risk with Mounjaro is indirect rather than a direct effect on kidney tissue. The product information states it plainly: severe vomiting or diarrhoea can cause dehydration, which can strain the kidneys.1

This matters most in the early weeks of treatment and after a dose increase, when stomach side effects are more likely for some people. Mounjaro slows how quickly the stomach empties, which is part of how it works and also part of why some people feel nauseous.1

Practical steps follow directly from the guidance. Keep fluids up. If you cannot keep fluids down, get medical help straight away rather than waiting to see whether it settles. If you have existing kidney disease, tell your kidney specialist about any bout of vomiting or diarrhoea, because they may want to review you.

Does Mounjaro have wider health benefits?

In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.2 These are trial-average secondary outcomes, not promises.

Those findings describe general cardiometabolic health. They are not kidney test results, and they should not be read as proof that kidney function will improve on treatment. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

If a kidney specialist recommends a GLP-1-type medicine for a kidney-related reason, that is a clinical decision made on your results. It is separate from the weight-management licence under which we supply Mounjaro. 05 · Weight loss and kidneys

Can losing weight restore kidney function?

Weight loss cannot be promised to restore kidney function. Kidney damage has many causes, and what helps depends on the cause. That is why a specialist assessment matters more than any general rule.

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary.

That result says nothing about kidney tests. If you have been told your kidney function is reduced, ask your GP or specialist what is driving it and whether weight is relevant in your case. Do not start or stop any treatment on the assumption that weight loss alone will reverse it.

06 · Who needs extra care

Who needs extra care before starting

Mounjaro is not suitable for everyone. You should not take it if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems.13

Kidney problems often sit alongside other conditions, so your full history matters. Always share your full medical history and medicine list during consultation.

If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar, such as shakiness, sweating or confusion. Your prescriber may need to adjust those medicines.1 This is an adjustment of doses, not a ban on combining them.

07 · Thyroid and liver function

Does Mounjaro help thyroid or liver function?

For thyroid function, the position is about safety, not benefit. In rodent studies tirzepatide caused thyroid C-cell tumours; it is not known whether this applies to humans, and no increase has been shown in human trials.1

As a precaution, tell your prescriber if you or a close relative have had medullary thyroid cancer or the inherited condition MEN 2. They will weigh this in your assessment. Routine thyroid monitoring is not required for most people.1

08 · When to seek help

When to seek help during treatment

Get medical help straight away if you cannot keep fluids down, because dehydration is the route by which kidneys can be strained.1 Do not wait for several days of symptoms to pass if you are struggling to drink.

Some problems need urgent attention. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting, which could be pancreatitis. Also get medical help straight away for pain in the upper right abdomen, fever or yellowing of skin or eyes, which could be gallbladder problems, and for signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.1

If you notice a new or lasting symptom that is not listed in the patient leaflet, tell your prescriber, and you can report suspected side effects through the MHRA Yellow Card scheme.19
