In this guide
01 · The core answer
Does Mounjaro affect your gallbladder, and does it matter for you?
Yes, it can, and for most people the risk is small but real. The product information lists gallstones as a common side effect and gallbladder inflammation as uncommon.1 The practical question is whether your own history makes it more relevant, and what you would do if symptoms appeared.
Gallstones (cholelithiasis) are a common side effect in the weight-management trials, affecting between 1 in 100 and 1 in 10 people.1 Inflammation of the gallbladder (cholecystitis) was uncommon in the weight-management trials, affecting between 1 in 1,000 and 1 in 100 people.1
| Question | What the product information supports |
|---|---|
| Can Mounjaro cause gallstones? | Yes. Gallstones are listed as a common side effect in the weight-management trials. |
| Can it inflame the gallbladder? | Yes, but this is uncommon. |
| What is the link? | Acute gallbladder problems were linked to the amount of weight lost. |
| Who needs extra care? | People with a history of gallbladder problems; your prescriber weighs this in your assessment. |
| What needs urgent help? | Upper right abdominal pain, fever, or yellowing of skin or eyes: get medical help straight away. |
02 · Your next step
What should you do if this applies to you?
If you have had gallstones, biliary pain or gallbladder surgery, say so in your consultation. Your prescriber will take extra care, or may recommend against treatment, if you have gallbladder problems in your history.13 Disclosing it is not a barrier to a conversation; it is what lets the prescriber make a safe decision with you.
If you have already had your gallbladder removed, the picture is different, and we cover it in our guide to Mounjaro after gallbladder removal. For the wider class of medicines, see how other weight-loss injections compare on gallbladder effects.
Everything starts with a free consultation reviewed by a GPhC-registered prescriber.
03 · Why the gallbladder is affected
Why does Mounjaro affect the gallbladder?
The product information ties acute gallbladder problems to the amount of weight lost.1
Tirzepatide activates the GIP and GLP-1 receptors, which regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin.1 Those effects are what help most people feel fuller sooner and eat less. Weight loss itself is the intended outcome of treatment, and in the weight-management trials it was substantial: in SURMOUNT-1, average loss over 72 weeks reached 20.9% on 15mg, compared with 3.1% with lifestyle changes alone. Individual results vary.2
04 · Warning signs to act on
What are the signs of a gallbladder problem on Mounjaro?
The signs that need medical help straight away are pain in the upper right abdomen, fever, or yellowing of the skin or eyes.13 If you have any of these, seek immediate medical help rather than waiting to see whether it settles.
Severe, persistent stomach pain that may spread to your back, with or without vomiting, is listed separately as possible pancreatitis: stop using Mounjaro and get medical help straight away.13
Mild stomach upset is common as the body adjusts, but severe pain in the upper right side, or pain with fever or yellowing of the skin or eyes, needs medical help straight away. If you take a medicine and notice a suspected side effect, you can report it through the MHRA Yellow Card scheme.19
Our team also offers side-effect support as part of ongoing monthly reviews, so you can raise concerns with the prescribing team between doses.
05 · Gallbladder history and eligibility
Can you take Mounjaro with gallstones or after gallbladder removal?
It depends on your history, and the decision belongs to a prescriber. Mounjaro is not suitable for everyone, and your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy or gallbladder problems.13
Always share your full medical history and medicine list in your consultation. That includes known gallstones, previous biliary pain, and any gallbladder surgery. A prescriber can then weigh up your reasons for treatment against your history.
People who have had the gallbladder removed often ask whether treatment is still possible and whether weight loss is harder afterwards. Our guide on private prescriptions after gallbladder surgery explains how the consultation works in that situation, and the Mounjaro gallbladder guide goes deeper on the topic.
Eligibility for treatment otherwise follows the licence: weight management in adults alongside a reduced-calorie diet and increased physical activity. On the NHS, NICE recommends Mounjaro for adults with a BMI of at least 35 plus at least one weight-related condition, and access is being phased in over several years.4
06 · Lowering your own risk
How can you reduce gallbladder risk on Mounjaro?
There is no proven way to prevent pancreatitis or gallbladder problems while taking Mounjaro.
Sensible, practical points that follow from this:
- Disclose any gallstones, gallbladder pain or surgery before treatment starts.
- Keep up your monthly reviews and report new symptoms early.
- Learn the warning signs: pain in the upper right of the abdomen, fever or yellowing of the skin or eyes could be a gallbladder problem, so get medical help straight away.
An inflamed gallbladder is a medical problem, not a home-remedy one.
Likewise, because dehydration from severe vomiting or diarrhoea can strain the kidneys, keep fluids up and get medical help straight away if you cannot keep fluids down.1
07 · Other side effects to know
How does the gallbladder fit with other Mounjaro side effects?
Gallbladder effects sit alongside a wider picture of mostly digestive side effects, and most are managed with your prescriber. Mounjaro is not known to damage the kidneys or liver directly, and no dose adjustment is needed for mild to moderate kidney or liver impairment.1 People with severe impairment need individual prescriber assessment.
Skin and other changes are covered in our page on how Mounjaro can affect your skin, and a separate piece looks at Mounjaro and circulation. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion), because your prescriber may need to adjust those medicines.13
Weight loss on tirzepatide also came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures in trials. These are trial-average secondary outcomes, not promises.1 Weighing those benefits against gallbladder risk is exactly the conversation to have with a prescriber.
If you stop treatment, tirzepatide takes about four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 That matters if gallbladder symptoms appear: medical assessment comes first, and your clinician will guide any decision about further doses.
