No food is banned on Mounjaro, but some choices make treatment easier and more effective. Prioritise protein at each meal, vegetables and fibre, and regular fluids. Many people feel worse after very fatty, fried or very sugary food.1 Treatment is licensed alongside a reduced-calorie diet, so the aim is a sustainable deficit, not a perfect food list.
Some practical ways to fit protein in when you are full quickly:
- Start meals with the protein food, then add vegetables and other items if you still have room.
- Choose smaller, more frequent portions if large plates sit heavily.
- Keep easy protein options to hand for days when cooking feels like too much effort.
- Watch high-calorie drinks, because coffee drinks in particular are an easy place for the calorie deficit to leak, while plain coffee and tea are fine.1

05 · Exercise and strength
Exercise: the second lever for muscle
Resistance exercise is the lever that tells your body to keep its muscle. The two levers are adequate protein, with a protein source at every meal, which is harder when appetite is low, and resistance exercise two or more times a week.1
Increased physical activity is part of the licence, not an optional extra. Nothing special is required: the evidence-backed pattern is regular moderate activity, such as brisk walking, plus resistance work for muscle.1 Start where you are and build. Resistance work does not have to mean a gym. Bodyweight movements, resistance bands or lighter weights at home all count as a starting point.
On injection days with early side effects, lighter movement still counts. Mild nausea, altered appetite and tiredness in the days after each injection are the commonest early effects and usually settle.1 Many people schedule harder sessions for days when they feel best and keep a gentle walk for the rest.
Exercise also protects against regain if you later stop treatment.1 That makes building a routine now one of the most useful things you can do, whatever happens with the medicine in future.

There is no single test you can do at home, so treat these as practical prompts rather than diagnosis. Notice whether everyday tasks feel harder, whether the weights you use are getting heavier to lift, or whether you feel noticeably weaker than your weight loss alone would explain. If so, talk to your prescriber or pharmacist.
Judge progress by trend, not individual weigh-ins.1 The scales cannot tell fat from muscle or water, so strength, how clothes fit and how you feel are useful companions to the number.

07 · Hydration and fat loss
Hydration, fat loss and what to look for
Drinking enough matters more on Mounjaro than usual. Reduced appetite means less fluid from food, and vomiting or diarrhoea can dehydrate you quickly, which is the main route to the rare kidney problems reported with this class.13 Keep water within reach through the day.
Does water help you burn fat? Water supports you, but drinking more water does not burn fat directly. Its value on treatment is protecting against dehydration and helping you feel well enough to eat and exercise. Treat inability to keep fluids down as a contact-your-clinical-team issue, not something to push through.13
How can you tell fat is leaving your body? There is no single sign that tells you. Practical markers such as your waist measurement, how clothes fit and your energy levels, tracked over weeks, tell you more than any single symptom. Early loss includes water weight, so expect early changes to be mixed rather than pure fat.1
08 · Stopping treatment
What happens when you stop
After your final dose, tirzepatide takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 Appetite effects fade as the medicine leaves your body, so it makes sense to plan your eating and exercise routine before that happens.
Keeping protein and resistance work going builds the habits that matter after the medicine. Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.

09 · Side effects and access
Side effects and getting treatment
The commonest early effects are mild nausea, altered appetite and tiredness in the days after each injection, and they usually settle.1 Muscle loss is not a separate listed side effect, but some lean tissue loss can come with rapid weight loss. If you notice new symptoms, discuss them with your prescriber and report suspected side effects through the Yellow Card scheme.19
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first.46 Many people who meet the licensed criteria choose regulated private treatment rather than waiting.

10 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
