What counts as a medical weight loss programme

A medical weight loss programme is a clinician-supervised plan, usually built around a prescription medicine, rather than a diet club or a supplement. Costs vary because providers bundle different things. Some quote one headline figure that covers the consultation, the medicine and reviews. Others split these into separate fees, and the total only emerges at checkout.

When you compare programmes, split the cost into the same parts every time. The medicine is the largest part. Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose KwikPen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

Next come the clinical costs. These include the initial assessment, any blood tests a provider insists on, and follow-up reviews. A practical way to compare is to write down three numbers for each provider: the cost of the first four weeks, the cost at your likely maintenance dose, and the total of any fees that are not the medicine. Providers that are hard to pin down on these three numbers are usually the ones that add costs later.

Treatment begins at 2.5mg, then steps up depending on how you respond. If you step up after at least 4 weeks on each dose, the first four pens (2.5mg, 5mg, 7.5mg and 10mg) cost £959.96 at standard prices. A new customer's first order is £40 less, and every later order £25 less.

After titration, your prescriber and you agree a maintenance dose. This is not always the highest. The year total depends on where you settle. A first year costs £2,711.88 if you settle at 5mg, up to £3,599.88 at 15mg. Both figures cover 52 weeks, which is 13 pens, at standard prices before discounts.

For a maintenance dose between those two, the monthly price is the useful number. At 7.5mg a pen costs £279.99, at 10mg it costs £290.99, and at 12.5mg it costs £319.99. Multiply by the number of pens you need and subtract the relevant discounts.

Three things change the real figure. First, the discounts: your first order is £40 off with code FIRST40, and every later order is £25 off the standard price with code DOSE25. Second, the dose you settle at. Third, whether you stay on treatment for the whole year. Some people pause, and there is no subscription to cancel, because we do not auto-ship. 07 · Comparing clinic quotes

How to compare quotes from named clinics

You may want to compare the cost of a specific clinic, such as Fit Medical Weight Loss, Medical Weight Loss by Healthogenics or Queen City Medical Weight Loss. The reliable route is to ask each clinic for a written, itemised quote and compare it on the same basis.

Use this checklist when you ask:

  • What is the first-month total? Include the consultation, any tests, the medicine and delivery.
  • Which medicine, and at which doses? A quote for a starter dose does not cover the full programme.
  • Is the medicine included, or billed separately? Some programmes charge a membership and then add the medicine on top.
  • What happens at each dose increase? Ask for every dose price in writing.
  • What are the reviews and who carries them out? You want a regulated prescriber, not a form with a score.
  • Can you stop without a fee? Subscriptions can be harder to leave than they are to join.

Reviews are worth reading, but read them for the pattern. Look for comments about billing surprises, how quickly questions were answered, and whether the clinical team followed up. Be wary of any review that promises a specific result, because results vary between people.

08 · NHS versus private

NHS versus private: the honest picture

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially people with a BMI of 40 or more and four or more qualifying conditions.46 Full rollout is expected to take years.5

Your GP or local NHS service can tell you where you sit in the rollout and whether you are eligible now. An NHS prescription is cheaper at the point of use, and if you pay prescription charges in England, the standard charge is £9.90 per item, though whether that applies depends on your circumstances and exemptions.

Private treatment is the alternative for people who meet the licensed criteria but are not yet reached by the NHS rollout. The licensed criteria are broader than NHS access. Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.1 It must be used alongside a reduced-calorie diet and increased physical activity.

A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable for you. That applies in both settings. A private prescription is not an easier route to the medicine. It is a faster route for people who qualify, and it must still be clinically appropriate.

The practical comparison looks like this. The NHS route costs little at the point of use but may involve a wait, and you may not meet the current criteria. The private route costs the price of each pen, but you can start once a prescriber approves you. Some people use private treatment while waiting and move to NHS care later, if their eligibility changes. If you plan to do that, tell your GP about your treatment so your records stay accurate.

Whichever route you choose, avoid anyone offering to supply without a consultation or prescription. That is not treatment, and it carries real risks, including counterfeit pens.

09 · What results to expect

What results to expect for the money

Cost only makes sense next to what the treatment does, so here are the trial figures, with the caveat that your own result may differ. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2

These are trial averages, and individual results vary. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2

A worked example makes the percentages concrete. For someone starting at 100 kg, 20.9% is about 21 kg, and 15% is about 15 kg. Someone starting at a different weight would scale those figures accordingly.

This is also why the cost per month is not the whole calculation. A programme that costs less but does not suit you is poor value. One that includes monthly clinical reviews, dose adjustment and side-effect support gives you the chance to find the dose that suits you.

A useful question to ask any provider is not how much weight you will lose, because nobody can promise it. Ask what monitoring they provide, who adjusts the dose, and what happens if the medicine is not suiting you. 10 · Why the medicine costs what it does

How the medicine works

The medicine is the main cost, so it helps to know what you are buying. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1

In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic).1

Tirzepatide has a half-life of about five days. After a final dose, it takes roughly 4 to 5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1

Buying safely: genuine pens and counterfeit risk

Price is a poor guide to safety, so check where the pen comes from. The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified. Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.7

Only buy from a pharmacy you can verify on the GPhC register, never from social media, beauty salons or unverified websites.12 Genuine treatment always involves a prescription and a clinical consultation. Anyone offering to supply without a consultation or prescription is a warning sign, however attractive the price.

Genuine UK stock is supplied through Lilly's official distribution to licensed pharmacies. Every pen carries a batch number and expiry date that can be checked.1 A genuine UK Mounjaro pen comes from a registered pharmacy, arrives cold-chain packaged, and the solution inside is clear and colourless.

Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, has been above 30°C, or has been in use out of the fridge for more than 30 days.13 If supply or packaging ever seems wrong, check the pharmacy on the GPhC register and contact the pharmacy before injecting.

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.14
Which organs can Mounjaro affect?
Mounjaro is not known to damage kidneys or liver directly; the practical risk is indirect, as severe vomiting or diarrhoea can cause dehydration, which can strain the kidneys. Get medical help straight away if you cannot keep fluids down. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction.
