Eat in the way that suits the medicine. Constipation is most likely to flare when you start treatment or step up, so the dose ladder is a useful tool as well as a schedule. Everyone starts on 2.5mg once a week, and the starting dose is about letting your body adjust, not about weight loss itself.1 After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals, up to a maximum of 15mg weekly, depending on how you respond and tolerate it.

Most side effects follow a pattern: they appear in the first days after starting or increasing a dose, peak early, and settle as your body adapts.1 If a new dose is hard to tolerate, prescribers can hold the current dose longer before stepping up. Speak to your clinical team rather than pushing through severe symptoms.

Higher is not automatically better. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and your prescriber will guide the right pace.1 If constipation keeps recurring at a particular strength, raise it at your review rather than waiting.

In the weight-management trials, nausea, vomiting and diarrhoea were more common while the dose was being increased and became less frequent over time.1 Constipation is a different symptom, but the same principle applies: the body usually adapts, which is why the first weeks after each change are the time to be most deliberate about fluids and fibre.

One practical point: consistent habits matter more than any single tactic. Keep the same injection day each week, as the schedule is built around it, and track how your bowels respond after each increase so you can describe the pattern to your prescriber.

07 · When to get help

When constipation needs a clinician

Constipation needs professional attention when it comes with pain or bloating, or when there is no bowel movement for more than a few days.13 In those cases contact your clinical team rather than trying to manage it alone.

Some symptoms that can feel like bowel trouble are more serious. Get medical help straight away for severe, persistent stomach pain that may spread to your back, with or without vomiting, which may signal pancreatitis. Also get medical help straight away for pain in the upper right abdomen, fever or yellowing of the skin or eyes, which may signal gallbladder problems.1 These are uncommon, but they need medical help straight away. Signs of a severe allergic reaction, such as swelling of the face or throat and difficulty breathing, also need immediate medical help.

If persistent vomiting or an inability to keep fluids down is added to constipation, contact your clinical team, because dehydration needs prompt attention.1
