Is constipation from Mounjaro normal, and does it go away?
Yes, it is a recognised and common effect. Like all medicines, Mounjaro can cause side effects, and most are digestive.1 Constipation sits alongside nausea, diarrhoea, vomiting and abdominal discomfort in the group of effects that affect more than 1 in 10 people.1
For many people it eases. Side effects are most likely when starting treatment or moving up a dose, and for most people they ease within days to a few weeks as the body adjusts.13 That is the general pattern, not a promise for any one person. Some people find constipation returns when they step up to a higher strength, which fits the same pattern.
If your bowels have not settled after the early weeks, or they keep flaring after each dose increase, that is useful information for your prescriber. It is not something to simply put up with.

Start with the basics, in order: fluids, fibre, movement. Fluid intake comes first, with regular water through the day, then fibre from vegetables and whole grains, then daily movement.1 If it persists, a pharmacist can recommend a suitable laxative.13

03 · Why it happens
Why Mounjaro causes constipation
Constipation happens for two main reasons: food moves through the gut more slowly, and many people eat and drink less. Both are direct results of how the medicine is meant to work.
Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.
That fuller feeling is the benefit. The side of the same coin is that smaller meals mean less bulk reaching the bowel, and a lower appetite often means fewer drinks too. A slower gut plus less fibre plus less fluid is a straightforward recipe for harder, less frequent stools.
This also answers a common question: the pattern is not a sign that something is wrong with you or with your pen. It is the expected downstream effect of a treatment that slows digestion. What matters is noticing it early and adjusting your routine before it becomes uncomfortable.

A daily routine to combat constipation
The most reliable approach is a small set of habits that you repeat every day, not a one-off fix. Put fluids first, then fibre from vegetables and whole grains, then daily movement.1
Fluids
Aim for regular water through the day, rather than waiting until you feel thirsty. Because Mounjaro reduces appetite, thirst cues can also feel quieter, so it helps to tie drinks to routines: one with each meal, one when you get up, one mid-afternoon. Keep a bottle where you can see it.
Staying well hydrated also helps with the other digestive effects of treatment, so this habit does double duty.13
Fibre from vegetables and whole grains
Build fibre up gradually. Adding a lot at once, especially while your stomach is already emptying slowly, can bring wind and bloating. Vegetables at lunch and dinner, and whole grains in place of refined ones, are a sensible start. Fibre works best alongside enough fluid; fibre without fluid can make hard stools harder.
Think in food swaps rather than numbers.
Daily movement
Gentle daily activity such as a walk helps keep the gut moving. It does not need to be intense. Regular movement also supports the reduced-calorie diet and increased physical activity that Mounjaro is licensed to be used alongside.1
Eat in a way that suits a slower stomach
Practical steps that help most people with digestive effects include eating smaller meals slowly, stopping at the first feeling of fullness, favouring plain lower-fat foods while adjusting, and avoiding lying down straight after eating.13 Those habits reduce discomfort around meals, which makes it easier to keep up the fibre and fluid that your bowels need.
05 · Laxatives and pharmacists
Laxatives: when to ask a pharmacist
If the routine is not enough, a pharmacist can recommend a suitable laxative.13 That is the right next step, ahead of trying whatever is advertised online.
There are several types of over-the-counter laxative, and they work in different ways. Which one suits you depends on your other medicines, your health history and how severe the problem is. Tell them you are taking tirzepatide, mention any other medicines, and describe what your stools are like and how long the problem has lasted.

What to avoid
Be wary of low-cost supplements marketed as natural alternatives for gut regularity or weight loss. They come with no clinical oversight, dosing is not standardised, and they are not a substitute for talking to a pharmacist who knows you are on a prescription medicine. Dramatic weight-loss claims on such products should make you more cautious, not less.
Also avoid reducing your food and drink further in the hope of feeling less bloated. Eating and drinking less is part of what causes the problem in the first place.
06 · Dose and timing
Constipation, dose increases and timing
Side effects follow a pattern. They appear in the first days after starting or increasing a dose, peak early, and settle as your body adapts.13 This is one reason Mounjaro uses a gradual dose ladder.
Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself.1 After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1
If you notice that bowel changes cluster after each step up, tell your clinical team. If a new dose is hard to tolerate, prescribers can hold the current dose longer before stepping up. Speak to your clinical team rather than pushing through severe symptoms.13
Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace.1 If constipation is a recurring problem at a particular strength, that is a fair reason to discuss whether staying put for longer makes sense.
If you miss a dose, follow the missed-dose rule in the patient leaflet, and never take two doses within 3 days of each other.1
07 · Constipation or blockage?
Constipation, gastroparesis or a blockage: when to seek help
Ordinary constipation is uncomfortable but usually manageable. What should prompt you to call is pain, bloating or more than a few days without a bowel movement. Contact your clinical team for any of those.1
- Severe, persistent stomach pain that may spread to your back, with or without vomiting, could be pancreatitis: seek immediate medical help.13
- Pain in the upper right abdomen, fever or yellowing of the skin or eyes could signal gallbladder problems and also needs immediate medical help.13 Signs of a severe allergic reaction, such as swelling of the face or throat and difficulty breathing, need immediate medical help.
- If nausea or vomiting is persistent or you cannot keep fluids down, contact your clinical team, because dehydration needs medical help straight away.13
Slower stomach emptying is part of how tirzepatide works,1 but whether symptoms amount to a medical problem is a clinical judgement. If your symptoms feel severe, are getting worse, or simply do not feel like your usual constipation, do not wait for them to pass. For severe or persistent stomach pain, get medical help straight away.
It is also worth being honest about what laxatives cannot tell you. If you are taking them and still have severe or persistent stomach pain, get medical help straight away.

08 · Trial figures and context
What the trials say about stomach side effects
In the trials supporting Mounjaro's weight-management licence, nausea was reported by 24.6% of people on 5mg, 29.0% on 10mg and 28.0% on 15mg, compared with 8.5% on placebo.2
Diarrhoea was reported by 18.7% of people on 5mg, 20.8% on 10mg and 22.5% on 15mg, compared with 7.8% on placebo.2 Nausea, vomiting and diarrhoea were more common while the dose was being increased and became less frequent over time.2
The takeaway is that digestive effects, in both directions, are expected and tend to be concentrated around dose changes.
Most people carry on. 1.9% of people on 5mg, 3.3% on 10mg and 4.3% on 15mg stopped treatment permanently because of gastrointestinal side effects, compared with 0.5% on placebo.2 That is a small minority, but it shows why raising problems with your prescriber early matters: there are often adjustments to try first.

09 · Stopping and next steps
What if you want to stop, and where to go next
If constipation is making you consider stopping, talk to your clinical team first, as they may be able to hold your dose or adjust your plan. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 That means any digestive effects may not vanish the day after your last pen.
