Semaglutide or tirzepatide: what you are actually comparing

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Semaglutide is the active ingredient in Wegovy and Ozempic, which are different products with different licences.

Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic).

The practical difference for cost is that the two medicines are not interchangeable at the same price point, and neither is a generic. The cheapest semaglutide or tirzepatide means choosing between two branded products, each made by a single company, each with its own dose ladder and its own price per month. Wegovy pens come in 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once-weekly strengths, stepping up after 4 weeks on each of 0.25mg, 0.5mg, 1mg and 1.7mg, so the 2.4mg maintenance dose starts from week 17.18

07 · Ozempic and licensing

Why Ozempic is not the cheap route to weight loss

Ozempic is semaglutide licensed for type 2 diabetes. It is not a UK weight-loss medicine, and prescribing it privately for weight loss is off-label. The licensed weight-management options are Mounjaro (tirzepatide) and Wegovy (semaglutide 2.4mg).18 So "Mounjaro vs Ozempic" for weight loss is really Mounjaro vs Wegovy, and a regulated UK pharmacy will not supply Ozempic for weight loss.

The honest answer is that the lowest price for the wrong product is not a saving. If you do not have type 2 diabetes, the licensed semaglutide product for weight management is Wegovy, and it is supplied after a consultation that checks whether it suits you. There is also a fairness point for people who do have type 2 diabetes. Diabetes supplies of Ozempic should go to those who need it for diabetes, prescribed by their own clinical team. Using diabetes stock for weight loss is exactly the off-label pattern regulated pharmacies avoid. If you take diabetes medicines, tell your prescriber, because doses of insulin or sulfonylureas may need adjusting to avoid low blood sugar when starting a GLP-1 medicine.

When you see an offer for Ozempic with no consultation, or a price far from the rest of the market, treat it as a warning. A genuine supply always involves a prescription and a clinical consultation, and the pharmacy can be checked on the GPhC register. Weighing a low-price offer against these checks is the quickest way to avoid a costly mistake.

08 · What the trials show

What the trials show for each medicine

Price only means something next to what you get for it. These are trial averages, individual results vary, and both medicines work alongside a reduced-calorie diet and increased physical activity, not instead of them.

In SURMOUNT-1, a trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 As a worked example, for someone starting at 100 kg, 20.9% is about 21 kg.

In the STEP 1 trial, adults on semaglutide 2.4mg (Wegovy) lost on average 14.9% of body weight over 68 weeks, compared with 2.4% on placebo.9 In STEP 1, 86.4% of adults on semaglutide 2.4mg lost at least 5% of body weight, compared with 31.5% on placebo. 69.1% lost at least 10% (vs 12.0%) and 50.5% lost at least 15% (vs 4.9%).9

SURMOUNT-5 was the first large head-to-head trial of tirzepatide and semaglutide for weight loss. In SURMOUNT-5, 751 adults took tirzepatide or semaglutide 2.4mg for 72 weeks. Average weight loss was 20.2% on tirzepatide versus 13.7% on semaglutide 2.4mg. At least 10% of body weight was lost by 81.6% on tirzepatide vs 60.5% on semaglutide; at least 15% by 64.6% vs 40.1%; at least 20% by 48.4% vs 27.3%; at least 25% by 31.6% vs 16.1%. Average loss was 22.8 kg vs 15.0 kg.15

Both are effective, licensed treatments. Which suits you depends on your health profile, tolerance and prescriber advice, not the headline number alone.

On tolerability, in STEP 1 gastrointestinal side effects were reported by 74.2% of people on semaglutide 2.4mg, compared with 47.9% on placebo, and 7.0% stopped treatment because of side effects, compared with 3.1% on placebo.9 Your prescriber will go through side effects with you.

09 · Who can get it

Who can get a prescription, and why that affects cost

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.1 A prescriber will assess your BMI, medical history and current medicines before deciding whether it is suitable for you.

That assessment is the first thing separating a genuine supply from a risky one. A price that sounds cheap but skips the consultation is not a saving, because the consultation is what establishes that the medicine is safe for you. Think about what eligibility means for your budget. If you meet the licensed criteria, private treatment is open to you now. Before the consultation, it helps to have a few things to hand: your height and weight, a list of any medicines you take, and any conditions you have been diagnosed with. Tell the prescriber about diabetes treatments in particular, as doses of insulin or sulfonylureas may need adjusting to avoid low blood sugar.

It is also worth telling the prescriber if you take an oral contraceptive. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Giving full information at the start avoids delays and wasted orders later.

Repeat orders re-check suitability, and there is no auto-shipping. That means your cost follows your clinical plan: you move up a dose only when your prescriber agrees, and you pay only for the pens you need. For many people that is the most reliable way to keep spend in line with need.

10 · NHS or private

NHS or private: the honest picture

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

If you are outside the current NHS groups, the question becomes how to pay for private treatment sensibly. Three habits keep cost down without cutting corners. First, order only the dose your prescriber has agreed. Second, use the new-customer discount on your first order and the automatic reorder discount afterwards. Third, plan the year rather than the month, so a rise at each step does not catch you out.

You may also use both routes in sequence. Some people start privately while they wait for an NHS decision, then move across if they become eligible. If that is you, tell your GP what you are taking, so the records are complete and the clinical team can plan around it.

None of this changes the core point: whichever route you take, the medicine should come with a prescription and a clinical consultation. That is true on the NHS and at a regulated private pharmacy, and it is the feature that no unregulated seller can offer.

11 · Tablets and alternatives

Tablets and "cheaper alternatives"

There is no tablet form of Mounjaro: tirzepatide is injection-only in the UK. Two GLP-1 weight-loss tablets are now licensed in the UK: Wegovy tablets (oral semaglutide, made by Novo Nordisk), approved by the MHRA in June 2026, and Foundayo (orforglipron, made by Eli Lilly), approved in August 2026.2021 They are different medicines from Mounjaro, with their own dosing, eligibility checks and trial results, and both are prescription-only.

Wegovy tablets and Foundayo are both prescription-only; see current Wegovy tablet prices on the product page. Tablet trial results should not be compared with Mounjaro's, because they were separate trials with different designs.

Other licensed options work differently. Orlistat (tablet) blocks some dietary fat absorption and produces smaller average losses. Wegovy and Saxenda (liraglutide) are other licensed injectables; liraglutide is a daily injection supplied at 6mg per millilitre, unlike once-weekly Mounjaro and Wegovy. A prescriber can advise on figures for any of these.

"Natural GLP-1" supplements, such as berberine, are not medicines, are not regulated for this use and have no comparable trial evidence. They are sold cheaply and without assessment, with no clinical oversight, and dosing is not standardised. Nicknames such as "poor man's Ozempic" or "nature's Ozempic" are marketing, not clinical description. A supplement is not a substitute for prescribed treatment, and its low cost is not a saving on treatment.

Whatever your budget, a prescriber decides which licensed option suits your health. A lower-cost medicine that does not suit you, or a supplement that does little, ends up costing more in time and disappointment than a properly chosen treatment.

12 · False economies

Cost-cutting shortcuts that are not savings

These are unsafe and unlicensed, and none is a real saving.

Talk to your clinical team rather than self-adjusting.

Each Mounjaro KwikPen is designed and licensed to deliver exactly four weekly doses. Use the pen as designed, then dispose of it.13

Mounjaro must not be combined with other GLP-1 medicines, unlicensed "research" peptides (such as retatrutide, which has no UK licence), or compounded copies. These combinations have no safety data and no regulated supply chain. If something promises faster results outside a licensed pathway, that is the warning sign.1 This is also why we do not treat compounded offers, so prominent in US pages, as a UK option.

The costs of getting this wrong are not small. Dosing errors, infection and unpredictable levels can all end in side effects that need medical care, and an unlicensed product has no recourse if something goes wrong. A regulated supply gives you a prescriber to contact, ongoing monthly reviews and side-effect support.

13 · Spotting fakes

Counterfeit pens: the real cost of a bargain

The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified. Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.7 Only buy from a pharmacy you can verify on the GPhC register, never from social media, beauty salons or unverified websites. Genuine treatment always involves a prescription and a clinical consultation.

Mounjaro is manufactured by Eli Lilly. Genuine UK stock is supplied through Lilly's official distribution to licensed pharmacies, and every pen carries a batch number and expiry date that can be checked. Ours comes through official LillyDirect supply, direct from Eli Lilly's UK distribution.

A genuine UK pen comes from a registered pharmacy with a batch number and expiry date, arrives cold-chain packaged, and the solution inside is clear and colourless. Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, has been above 30°C, or has been out of the fridge for more than 30 days in use. If supply or packaging ever seems wrong, check the pharmacy on the GPhC register and contact the pharmacy before injecting.12

Storage matters for any pen you buy. Unopened pens go in the fridge until first use and keep until the printed expiry date. For any question about an unopened pen that has been out of the fridge, check the patient information leaflet and ask the pharmacy rather than guessing.13

Practical checks before you buy: look the pharmacy up on the register, confirm that a prescriber will review your answers, and be wary of any seller offering to supply without a consultation or prescription. A price far below the rest is a prompt to check harder, not a reason to buy faster.

If you have already used a pen you now doubt, contact your prescriber or GP, and report any side effect or suspect product through the MHRA Yellow Card scheme. Reporting helps the regulator track falsified products, and it protects other people.

Safety is the part of the price you cannot see on a listing. A fake pen at a low price is the most expensive purchase on the market, because the cost may be a hospital stay rather than a pound figure.

14 · Switching and long-term cost

Switching, maintenance and the long-term bill

There is no licensed direct conversion between semaglutide and tirzepatide. Switching is done under prescriber supervision: typically the previous medicine is stopped, a washout interval observed, and Mounjaro started at a dose your prescriber judges appropriate, often the 2.5mg starting dose, sometimes adjusted based on your history. Never run two GLP-1 medicines together.

From a cost view, a switch is a decision to make with your prescriber, not a bargain-hunting exercise. If you are on semaglutide now and wondering about tirzepatide, your prescriber decides when to switch and which dose to start on. Think, too, about the long game. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears. Stopping and restarting is therefore a clinical decision, not a quick cost-saving move.

Maintenance is where many people want to cut cost. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. Stopping should always be planned with your prescriber.14

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. What's better, Mounjaro or semaglutide?
Both are effective, licensed treatments, and the better one depends on your health profile, tolerance and prescriber advice. In the head-to-head SURMOUNT-5 trial of 751 adults, average weight loss over 72 weeks was 20.2% on tirzepatide versus 13.7% on semaglutide 2.4mg (Wegovy). Individual results vary, and both work alongside a reduced-calorie diet and increased physical activity.
What is known as the "poor man's Ozempic"?
The nickname is usually applied to low-cost supplements such as berberine, sometimes marketed as "natural GLP-1" or "nature's Ozempic". These are not medicines, are not regulated for this use and have no comparable trial evidence. They are available cheaply without assessment and with no clinical oversight, and dosing is not standardised. A supplement is not a substitute for a licensed treatment, so speak to a prescriber if you want a proven option.
What is a cheaper alternative to Ozempic?
For weight loss, the licensed alternatives are Mounjaro (tirzepatide) and Wegovy (semaglutide 2.4mg), because Ozempic is licensed for type 2 diabetes rather than weight management. Other licensed options include orlistat, which blocks some dietary fat absorption and gives smaller average losses, and Saxenda (liraglutide), a daily injection. Two GLP-1 tablets are also now licensed.
Which weight loss pill is right for me?
No single pill is best for everyone, and the prescriber decides what suits the person.
