Eligibility is set by the licence. Mounjaro is licensed for adults with a BMI of 30 or above, or 27 to 30 with at least one weight-related condition, alongside a reduced-calorie diet and more activity.111 Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. 04 · Where 7.5mg fits

Where the 7.5mg pen sits on the dose ladder

The 7.5mg pen is the third step on the Mounjaro ladder, not a starting dose. Everyone begins on 2.5mg once a week. That starting dose is about letting your body adjust, not about weight loss itself.1 After at least four weeks the dose rises to 5mg. It can then step up by 2.5mg at a minimum of four-week intervals, to 7.5mg, 10mg and 12.5mg, up to a maximum of 15mg weekly, depending on how you respond and tolerate it.111

That means you normally reach 7.5mg after at least eight weeks of treatment, and only if your prescriber agrees you are ready. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, so for some people 7.5mg is a stepping stone rather than a destination. Higher is not automatically better, and your prescriber will guide the right pace.1

How tirzepatide works at this dose

Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1. Together they help regulate appetite, slow how quickly the stomach empties and improve how the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide.

Tirzepatide has a half-life of about five days. After a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1

Why the same dose can cost so differently

Prices for the same 7.5mg pen differ between providers. Pharmacy sell prices vary because of confidential discounts. That is why a gap exists even between genuine, regulated suppliers.

Across the UK pharmacies we track, the smallest gap between providers for the same dose was £27 and the largest was £41, checked 11 October 2026. The cheapest and dearest pens across all doses were £143.96 and £339.00. Those are all-dose figures, so they say nothing on their own about 7.5mg. Use them as context, then look at the specific 7.5mg price each provider shows.

Boots' Mounjaro dose prices ranged from £179.00 to £339.00 on our last check. Starting-pen prices (2.5mg) across the providers we track ran from £143.96 to £179.99. Because many providers advertise a low first-order price for the starter pen, it is worth asking what the 7.5mg pen will cost when you reach it, not only what you pay in week one.

Pricing traps to watch for

  • Introductory prices that apply once. A listing that says "from" may only apply to a first order. Our own first-order price is the standard price less £40; every later order is £25 off the standard price.
  • Extras at checkout. Consultation, delivery and review fees can be added late. For us the consultation and next-day delivery are free.
  • Bundles that lock you into a dose. Our single-pen option keeps you flexible.

Planning the cost of a whole course

The pen price matters less than the total you will spend, because treatment runs for many months. At standard prices, before discounts, if you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96. A new customer's first order is £40 less, and every later order is £25 less.

A first year is 52 weeks, or 13 pens. If you settle at 5mg, a first year costs £2,711.88 at standard prices. If you settle at 15mg, it comes to £3,599.88. Those two figures are the bookends. Anywhere between them depends on where your prescriber and your results land. If you settle at 7.5mg, the monthly cost is £279.99 per pen at the standard price.

That table assumes the quickest step-ups the licence allows. Your prescriber may hold you on a dose for longer if side effects are the concern, which changes the order in which you buy pens and the total. The licence permits increases at a minimum of four-week intervals, not a fixed schedule.1

07 · Is 7.5mg for you

Who the 7.5mg pen is for

The 7.5mg pen is for adults who already tolerate 5mg and whose prescriber has agreed to a further step. It is not a dose to start on, and you cannot choose it at a first order unless you are already on it. Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.111

A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable. That is why we ask for a consultation even if you have used the medicine before elsewhere. If you are moving from another provider, bring your current dose and recent history.

How your prescriber judges progress at each step

Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend, not individual weigh-ins.2

Treatment response is reviewed by your prescriber. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1 That review is part of why reaching a higher dose is a clinical decision rather than a purchase decision.

The 2.5mg starting weeks are about adjustment, and experiences differ widely. Some people notice appetite change and early loss in week one; others feel little until a higher dose, and both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.13

When you step up to 7.5mg, expect a similar pattern in the days after the first injection at the new dose. If side effects feel more than mild, tell your prescriber before the next injection, and you can report suspected side effects through the Yellow Card scheme.19

Higher is not automatically better.

08 · What results to expect

What the trials show, and what they do not

The best-known trial of tirzepatide for weight loss is SURMOUNT-1, which included 2,539 adults. People taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2

As a worked example only, for someone starting at 100 kg, 20.9% is about 21 kg and 15% is about 15 kg. Trial averages do not predict your own result, and the examples say nothing about how long it will take you.

Where 7.5mg fits in the trial results

The honest summary is that 7.5mg sits between two doses with published averages. It is a step on the way, and your own trend matters more than a number.

The pen price is the cost of the dose, not a promise of outcome.

A realistic timeline

Think in stages rather than weeks lost. The adjustment phase on 2.5mg is followed by gradual progress from 5mg. By the time you reach 7.5mg you have typically completed at least eight weeks of treatment. The formal checkpoint is a review after 6 months on the highest dose you can tolerate: if you have lost less than 5% of your starting weight, your prescriber will discuss whether continuing or stopping is right for you.

09 · NHS and private

NHS or private: the honest picture

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40 and above with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

NHS prescriptions in England carry the standard prescription charge of £9.90 per item, where charges apply. Private treatment costs more up front but is not tied to the phased NHS criteria. It is the licensed medicine, supplied after a prescriber review, whichever route you use.

Can my GP prescribe it, and why might they say no?

Your GP can prescribe tirzepatide on the NHS only if you fall into the groups the rollout has reached and local services are in place. If you are not yet in an eligible group, a GP may say they cannot prescribe it yet. That is a statement about phased access, not necessarily a judgement on whether the medicine would suit you. Ask your GP which stage applies to you, and whether you are on a list for a later stage.

A prescriber can also decline for clinical reasons: your BMI, your medical history or your current medicines may make the medicine unsuitable. That applies on the NHS and privately. Moving between NHS and private supply

If you are already on an NHS prescription and want to switch, or the reverse, talk to both teams so doses and monitoring stay consistent. Tell the new prescriber your current dose and when your last injection was. If you stopped for a while, the dose you restart on is a clinical decision.

10 · Fake pens

Counterfeit pens and why the cheapest price is not the whole story

The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified. Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.7 Only buy from a pharmacy you can verify on the GPhC register, never from social media, beauty salons or unverified websites. Genuine treatment always involves a prescription and a clinical consultation.

Genuine UK stock is supplied through Lilly's official distribution to licensed pharmacies, and every pen carries a batch number and expiry date that can be checked. How to check your pen is genuine and still good

A genuine UK Mounjaro pen comes from a registered pharmacy with a batch number and expiry date, arrives cold-chain packaged, and the solution inside is clear and colourless.13 Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, or has been out of the fridge for more than 30 days, or above 30°C in use. If supply or packaging ever seems wrong, check the pharmacy on the GPhC register and contact the pharmacy before injecting.

  • Packaging. Look for the batch number and expiry date on the box and pen.
  • Cold chain. Our parcels arrive in discreet, temperature-controlled packaging by next-day tracked delivery.
  • Appearance. The liquid should be clear and colourless, with nothing floating in it.
  • Paperwork. A pharmacy label and the patient information leaflet should be present.

Red flags when hunting for a low price

Be cautious of any seller who offers to supply without a consultation or prescription, who cannot give a registration number, who sells through social media messages or who asks you to pay by bank transfer to a personal account. A price that is far below every regulated provider should raise questions rather than excitement. A saving of a few pounds is never worth the risk of an injection of unknown content.

11 · Using your pen

How to inject a 7.5mg pen

Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food.1 Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose. Each KwikPen contains four weekly doses.

Timing, missed doses and changing day

There is no best time of day. Mounjaro works the same morning or evening, with or without food. What matters is the same day each week. Pick a day and time you will remember. Many people choose a weekend morning so early side effects fall on days off.

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other. You can change your weekly injection day if at least 3 days (72 hours) have passed since your last dose.1

Storage and travel

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, which is useful for short trips, after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.113

You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. Most holidays need no special arrangements, and for hot climates use an insulated pouch. Check destination rules for travelling with prescription injectables before you fly.

Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit, because figures circulating online often come from other countries' labels.

12 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40 and above with four or more qualifying conditions. Why won't my doctor prescribe Mounjaro?

A doctor can also decline on clinical grounds, such as your BMI, medical history or current medicines. Ask which of the two applies and whether you can be reconsidered at a later stage. Why have I been refused Mounjaro?
A refusal usually means the prescriber judged it unsuitable, or that you did not meet the criteria at that point. The licence covers adults with a BMI of 30 or above, or 27 to 30 with at least one weight-related condition, and the prescriber also reviews your history and current medicines. If one of our orders is declined it is refunded automatically in full. Ask for the reason, and discuss alternatives with your GP or pharmacist.
When is the best time of day to take tirzepatide?
There is no best time of day. Mounjaro works the same in the morning or evening, with or without food. What matters is injecting on the same day each week. Choose a day and time you will remember, and many people pick a weekend morning so that early side effects, such as mild nausea or tiredness, fall on days off. If you need to change your injection day, leave at least 3 days (72 hours) since your last dose.
What's better, Mounjaro or semaglutide?
Both are licensed, effective once-weekly treatments, and the right one depends on your history, tolerance and what your prescriber recommends. Tirzepatide activates two gut-hormone receptors, GIP and GLP-1, while semaglutide (Wegovy) acts on one. Ozempic is semaglutide licensed for type 2 diabetes only. It is not a UK weight-loss medicine.
What alternatives are there to Mounjaro?
Other licensed options include Wegovy, which is semaglutide taken once weekly and stepped up after at least 4 weeks to the 2.4mg maintenance dose from week 17, and liraglutide products, which are a daily injection supplied at 6mg per millilitre.
