Mounjaro (tirzepatide) is a once-weekly injectable prescription medicine licensed in the UK for weight management in adults and for type 2 diabetes.1 It is used alongside a reduced-calorie diet and increased physical activity, not instead of them.2
That licence matters. A prescriber has to judge whether treatment is appropriate and safe for you, and that judgement is about your health, not just a number on the scales. If your goal is genuinely small, a prescriber may reasonably suggest other approaches first. We think that honesty is better than selling a medicine to someone who does not need it.

How GLP-1 medicines work
GLP-1 medicines work by acting on natural gut-hormone signals that help regulate appetite. Mounjaro is the brand name for tirzepatide, made by Eli Lilly. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1
Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1
This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic). That does not make semaglutide ineffective. Both are licensed treatments, and the choice between them is one for you and your prescriber.
The practical consequence is worth spelling out. These medicines do not burn fat directly, and they are not a shortcut that bypasses eating habits. They change how hungry you feel and how full you feel after eating. That makes it easier for many people to eat less without constant struggle, which is why they are always used alongside a reduced-calorie diet and increased physical activity.
Because the effect depends on a steady level of medicine in the body, the pen is used once weekly. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1
This slow build and slow clearance have two consequences for someone chasing a quick, small loss. You will not feel the full effect in the first days, and you cannot use the medicine like a crash diet and switch it off cleanly afterwards. The medicine is built for a gradual, supervised course, which is why a prescriber reviews you before supply and again at repeat orders.

Early results are usually modest, and that is normal. Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month.2
This matters for anyone with a short-term goal. The starting dose is not meant to deliver the full effect. It lets your body adjust, and after at least 4 weeks the dose can step up, depending on how you respond. Your own response, not a calendar, guides that decision with your prescriber.
The first few weeks differ widely between people. The 2.5mg starting weeks are about adjustment, and experiences differ widely: some people notice appetite change and early loss in week one, others feel little until a higher dose — both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.2
That point about water weight is worth taking seriously. If you weigh yourself daily, you will see numbers jump around from hydration, meals and timing. Judging progress by trend rather than individual weigh-ins keeps you from feeling that nothing is working, or from over-celebrating a lucky morning.
Three months in, most people are still on the lower rungs of the dose ladder, so trial-average losses at this stage are modest compared with the headline figures. In SURMOUNT-1, average weight loss built month by month, with the full 15–21% averages measured at 72 weeks, not 12. A sensible 3-month expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation.2
Slower early progress does not predict failure, because the dose you respond to may still be ahead of you. Common early effects are mild nausea, altered appetite and tiredness in the first days after each injection, and they usually settle.
So can you lose a few pounds? Many people do, within the early part of treatment. But it is not a fixed promise, and it is not the point of the licence.
06 · What the trials measured
What the trials measured
The headline evidence for tirzepatide comes from SURMOUNT-1, and it measured far more than a few pounds. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2
These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them. Trial averages do not predict your own result.
The trial also reported how many participants reached particular thresholds. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2
What does this tell someone who wants a modest loss? Mainly that the medicine is designed and tested for substantial, sustained weight management. A small goal sits well within what many people achieve early, but the medicine is judged on much bigger outcomes. That is why prescribers think carefully about whether it is proportionate.
It also helps to be wary of anything that implies a precise timetable. The trial measured results at 72 weeks, and individual response varies widely. We therefore do not give week-by-week loss predictions.
The comparison with lifestyle changes alone is also informative. In the same trial, the group receiving only diet and exercise support lost far less on average. That is useful context: lifestyle change works, but for some people it is not enough on its own, and that is the group for whom medication was designed.

People often ask whether you can take a GLP-1 medicine without being overweight, or how to get one if you are not overweight. The honest answer is that the licence is for weight management, so a prescriber will weigh up whether treatment is appropriate for you, and may say no. Do not be tempted by sellers offering to supply without a consultation or prescription. That is a red flag for unsafe or counterfeit products.
Mounjaro is not suitable for everyone. You should not take it if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems. Always share your full medical history and medicine list during consultation.13 Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.
On the NHS, the picture is different and honestly phased. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46
Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting. If your goal is small, you are unlikely to be in the highest-need NHS groups, so the practical question is whether private treatment is proportionate for you, and a prescriber will help you answer it.

08 · Plateaus and staying on track
Plateaus and staying on track
Plateaus are normal on a GLP-1 medicine, and they are not a sign that treatment has failed. Weight loss on Mounjaro is not linear — plateaus of a few weeks are normal and expected as the body adapts to a lower weight.2
If you are near a small target and the scales stall, it is tempting to panic. Within the licence, the responses are sensible and unglamorous. Review food intake honestly, because portions drift back up. Protect protein and activity. Check the basics, such as sleep and alcohol. Discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum.2
A plateau is not a reason to double doses or add unlicensed products. Take any worries to your prescriber.

How to lose weight on GLP-1 is, in practice, the same advice as for any weight loss, made easier by lower appetite. Eat regular balanced meals, prioritise protein, keep moving, sleep well and weigh yourself in a consistent way. 09 · Beyond the scales
Health changes beyond the scales
Weight is not the only thing that changes on treatment. In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.2
These are trial-average secondary outcomes, not promises. But they explain why prescribers talk about weight management as health management rather than appearance. It also explains why a medicine of this kind sits uneasily with a purely cosmetic goal: the benefits it was developed to deliver are medical.
If you are weighing a small goal, that framing can help. Ask yourself what you actually want. If it is to look or feel different for an event, a structured lifestyle plan may serve you better and carry no medical risk. If it is to improve long-term health because of a weight-related concern, a conversation with a prescriber is the right next step.
Consider too what happens when treatment ends. Stopping is a decision to make with your prescriber, not on impulse. After a final dose, tirzepatide takes roughly four to five weeks to clear the system, and nothing needs doing while it clears.1 Appetite changes as the medicine clears, so plan your habits in advance. The same applies to the question of how to get off a GLP-1 medicine without regaining weight: make sustainable eating and activity habits part of treatment from the start, and talk through a stopping plan with your prescriber.
Side effects deserve a mention too. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.2 You can report suspected side effects through the MHRA Yellow Card scheme.19
If a symptom is new and you are unsure whether it is related, tell your prescriber rather than guessing. That is the safest habit for anyone starting a prescription medicine.
10 · Pens, pills and ordering
Pen or pill: which is right for you?
Both injectable and tablet GLP-1 options exist in the UK, and the right one depends on your health, preferences and prescriber's advice. Mounjaro and Wegovy are once-weekly injections. Wegovy tablets (oral semaglutide) and Foundayo (orforglipron) are licensed tablet options, described in their product information.2021
Trial results for tablets and Mounjaro come from different trials and cannot be set side by side. The question of whether GLP-1 pills are effective is best answered by your prescriber, looking at your circumstances, rather than by headline numbers.
Liraglutide is a different case again. Liraglutide (Saxenda, and products such as Erly) is supplied at a concentration of 6mg per millilitre; it is a daily injection, unlike once-weekly Mounjaro and Wegovy. Some people prefer a once-weekly schedule for convenience.
Wegovy's dose ladder is 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once weekly, and titration steps up after at least four weeks, reaching the 2.4mg maintenance dose from week 17.18 Mounjaro has its own dose ladder from 2.5mg to 15mg.

11 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
