So a half dose is not simply a smaller version of the licensed treatment. It is a different, untested way of using the pen, and nobody can tell you how much medicine you actually received.

How the KwikPen is designed to work

The Mounjaro KwikPen is a pre-filled multi-dose pen. It comes in six strengths, and each pen is made for a single strength.1 You do not mix strengths or draw medicine into a syringe. That design matters for the half-dose question, because nothing about it is built for measuring a fraction.

The pen is set by turning the dose knob until the dose indicator shows the correct dose in the window. Each pen contains four weekly doses.

It also helps to think about what the pen is protecting you from. The design stops you dialling past the medicine left inside. That is a safety feature, and it exists so that you receive the dose the label says you are receiving. Workarounds, such as injecting part of the dose, then stopping, remove that protection.

Tirzepatide acts on two gut-hormone receptors at once, GIP and GLP-1. Together they help regulate appetite, slow how quickly the stomach empties and improve how the body responds to insulin.1 Because the effect depends on a consistent weekly dose, an unpredictable amount in your body makes it harder for you and your prescriber to judge how you are responding.

05 · Why splitting is unsafe

A half dose falls squarely under that description.

The practical problems are easy to see. Altering doses outside the licence means unknown dosing accuracy and no safety data. You cannot tell how much tirzepatide you received. Your prescriber cannot interpret your progress or side effects accurately either, because the dose you report is not the dose you took.

Some people ask whether a half dose would at least be harmless because it is less medicine. That reasoning does not hold. A smaller, uncertain amount is not a safer known amount. The pen was not tested for partial delivery, so nobody can describe what you would receive.

There is also the question of what a half dose would do to your treatment plan. The licensed ladder is built around steady steps. Your prescriber reviews your response against the dose you were given. If you quietly alter it, the review is based on wrong information, and decisions about increasing, holding or stopping become guesswork.

If you have already tried splitting a dose, be honest with your prescriber. They will not be shocked, and they can help you return to a licensed approach. Report any unusual symptoms through the MHRA Yellow Card scheme as well.19

If a dose feels too strong, tell your prescriber, because the licence already allows safer adjustments. That is the correct answer to the real problem behind the half-dose question.

Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1

Notice the wording: a minimum interval, not a required one. There is no obligation to climb quickly. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better. Your prescriber will guide the right pace.1

Before you contact the team, it helps to note a few things in plain words: which day you inject, when the symptoms appear, how long they last and whether they ease before the next injection. Practical tweaks that sit within normal use can also help. Some people choose a weekend morning so that early side effects fall on days off. There is no best time of day, and Mounjaro works the same morning or evening, with or without food. What matters is the same day each week.

07 · Missed doses and doubling up

Missed doses and why you never double up

Some people who ask about half doses are really thinking about how to handle a gap. The rule for a missed dose is clear, and it replaces any urge to compensate. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day.1

If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1 That sentence is the key safeguard, and it applies in both directions. You do not split a dose to cover the gap, and you do not take extra injections to catch up.

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system.

The same logic applies to anyone who has had a rough week and wonders about taking just a little to stay on track. Staying on track in this treatment means keeping the weekly rhythm and staying in touch with your prescriber, not injecting fragments.

If you have had several missed weeks, do not guess your dose on return. Contact your prescriber for advice. That decision should come from a clinician, not from a forum thread.

08 · Changing your injection day

You can change your injection day, and that is the licensed alternative to squeezing doses closer together. You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 The label also describes this as at least 3 days between two doses.

This is useful when your routine changes. Perhaps a new shift pattern makes your usual day awkward, or you are travelling and the day falls on a flight.

There is no best time of day. Mounjaro works the same morning or evening, with or without food. Pick a day and time you will remember and stick to it. Many people choose a weekend morning so that early side effects fall on days off.

Some people consider a half dose because they want to inject twice in a week to spread out the side effects. That is exactly the kind of plan that breaks the minimum-gap rule, and it is not supported. If the effects hit hard after the injection, talk to your prescriber about your dose.

Rotate the injection site with each dose. Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. The licensed ladder already begins at the gentlest strength, which removes much of the reason to cut a dose. The 2.5mg starting weeks are about adjustment, and experiences differ widely. Some people notice appetite change and early loss in week one, others feel little until a higher dose, and both are normal.1

This matters because impatience is a common driver of dose tinkering. Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.

Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend rather than individual weigh-ins. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages and individual results vary. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

Those results come from full licensed doses taken as directed.

Response is also reviewed. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.

Storage and pen life

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away.1

Keep the pen cap on to protect it from light, and never use a pen that has been frozen.

Travelling is a common trigger for storage worries. You can fly with Mounjaro. Carry pens in hand luggage, since hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements. For hot climates, use an insulated pouch, and check destination rules for travelling with prescription injectables before you fly.

Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit. Figures circulating online often come from other countries' labels.

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

Over a longer horizon, a first year costs £2,711.88 if you settle at 5mg, up to £3,599.88 at 15mg, at standard prices before discounts.
