Can you restart Mounjaro at 5mg?

You may be able to, but the decision belongs to a prescriber who knows your history. The licensed ladder is clear about where treatment begins: everyone starts Mounjaro on 2.5mg once a week.1 It also says this starting dose is about letting your body adjust, not about weight loss itself.1 That adjustment is the reason a restart can differ from simply carrying on.

What decides your restart dose

Three questions shape the answer. Your prescriber will usually ask about all of them.

03 · How long it stays in your system

How long Mounjaro stays in your system

Clearance is slow, which is why a short gap rarely means starting from scratch. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1

Your prescriber looks at your own history to decide the restart dose.

04 · Missed dose or restart?

Missed dose or restart?

A missed dose and a restart are different situations. For a missed dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1

If you have missed several weekly doses in a row, you are beyond that rule. Do not simply resume where you left off. Contact your prescriber, tell them how many doses you missed, and ask which strength to use. If you are changing your injection day, at least 3 days must pass since your last dose.1

05 · Why 5mg is not a safe default

Why 5mg is not a default

Many people settle at 5mg, and it is an effective dose for some. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 Higher is not automatically better, and your prescriber will guide the right pace.

For context, in the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary.

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

Can I just come off 5mg Mounjaro?
You can stop at any time, but speak to your prescriber first so they can review your progress and discuss what comes next. After your final dose, tirzepatide takes roughly four to five weeks to clear the system, and nothing needs doing while it does. If you later want to restart, a prescriber will advise on the dose.
Does Mounjaro have withdrawal symptoms?
Withdrawal symptoms are not listed as a side effect in the Mounjaro patient leaflet. If you feel unwell after stopping, tell your prescriber. You can also report any suspected side effect through the MHRA Yellow Card scheme. Tell your prescriber about any new or lasting symptom. If you have severe, persistent stomach pain, with or without vomiting, stop using Mounjaro and get medical help straight away; for pain in the upper right abdomen, fever or yellowing of the skin or eyes, or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing, get medical help straight away.
Does restarting Mounjaro work?
Mounjaro works through its dual action on GIP and GLP-1 receptors, helping you feel fuller sooner and less hungry between meals. Whether a restart works for you depends on your dose, diet and activity, and results vary. Progress is gradual, so judge it by trend over months rather than individual weigh-ins.
Can I have a break from Mounjaro?
A short gap is covered by the missed-dose rule: take the dose as soon as you can within 4 days (96 hours) of the scheduled time; if more than 4 days have passed, skip it and take the next on your normal day. For anything longer, speak to your prescriber before your next injection.
Can I take half my 5mg Mounjaro?
No. If 5mg is too much, ask your prescriber about stepping down within the licence.
What happens if you start Mounjaro at 5mg?
Everyone starts Mounjaro on 2.5mg once a week, and that starting dose is about letting your body adjust. Starting higher than 2.5mg is outside the licensed ladder, so speak to a prescriber rather than choosing a dose alone. Mild nausea, altered appetite and tiredness are the commonest early effects and usually settle.
Is it normal to gain weight after stopping Mounjaro?
Weight regain is common: in SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued lost a further 5.5% on average. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.
Is it safe to come off Mounjaro cold turkey?
Stopping is your choice, and the medicine clears over about four to five weeks without anything needing to be done. Even so, speak to your prescriber first so they can review your progress, discuss what to expect and plan any restart. Do not adjust doses yourself in the meantime.
