Can you get weight loss injections on a medical card?

The short answer is that it depends on where you live, and a medical card is not a UK concept. The term belongs to the Irish health system. For that, ask your GP or the HSE directly.

If you live in the UK, the equivalent question is whether you can get a weight loss injection on the NHS. There the answer is yes, for some people. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition.4 The primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.6

The Mounjaro pen costs £169.99 per month at the 2.5mg starting dose, and all six doses are in stock. New customers save £40 on their first order with code FIRST40, at any dose. An automatic £25 reorder discount then applies to every future order (code DOSE25).

For planning, if you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96 at standard prices. A first year costs £2,711.88 if you stay on 5mg after your 2.5mg starting pen, up to £3,599.88 if you step up through every dose to 15mg. 05 · Medical cards explained

What a medical card is, and why UK readers get confused

A medical card is an entitlement issued under the Irish public health system. It is not issued in the UK, and no UK card or certificate gives automatic access to weight loss injections.

If you hold an Irish medical card, your GP or the HSE can tell you exactly what is covered and under what conditions.

If you are in the UK, there are three practical questions to ask. First, do you meet the NHS criteria as they currently stand in your area? Second, if not, do you meet the licensed criteria for private prescribing? Third, what would ongoing treatment cost you each month?

An NHS prescription charge exemption affects what you pay at the pharmacy for an NHS prescription. It does not decide whether you are eligible for a weight loss injection in the first place. Eligibility is a clinical decision by a prescriber.

06 · The NHS route

Who is eligible for NHS weight loss injections?

NICE recommends Mounjaro for adults with a BMI of at least 35 plus at least one weight-related condition.4 That is the recommendation, not the date your local service will offer it. Access is being phased in over several years.6

The primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.6 If you sit outside that first group, you may not be offered it yet, even if you meet the NICE recommendation. Full rollout is expected to take years.

Your GP or local NHS weight-management service will confirm how the criteria apply to you. The NHS guide to the medicine is a useful starting point.5

Which injections are NHS approved? Mounjaro is available on the NHS, with access phased in over several years. NHS access to Wegovy runs through specialist weight-management services, so ask your GP. Liraglutide (Saxenda) is a daily injection supplied at 6mg per millilitre, unlike once-weekly Mounjaro and Wegovy. For other details, your prescriber can advise.

Free weight loss jabs are therefore not a matter of applying for a card. You qualify by meeting clinical criteria and reaching the front of a phased rollout. Ask your GP what applies in your area, and keep records of your BMI and any weight-related conditions to speed that conversation up.

07 · Private eligibility

Private eligibility: are you licensed for Mounjaro?

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Examples include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.1 It must be used alongside a reduced-calorie diet and increased physical activity.1

This licence is wider than the current NHS rollout stage, which is why the two routes differ. A prescriber assesses your BMI, medical history and current medicines before deciding whether it suits you.11 Our consultation is free and reviewed by a GPhC-registered prescriber, so you can find out without cost or obligation.

Everyone starts on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg. It can then step up by 2.5mg at a minimum of 4-week intervals, up to a maximum of 15mg weekly, depending on response and tolerance.1 Many people settle at 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the pace.

08 · What to expect

What results and side effects to expect

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages and individual results vary. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

For someone starting at 100 kg, 20.9% is about 21 kg. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.2

Weight loss is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month. Think in months, not days. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1

What are the common side effects of Mounjaro? Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects, and they usually settle.13 The most common side effects, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort. Side effects are most likely when starting treatment or moving up a dose, and for most people they ease within days to a few weeks. Report any suspected side effect through the Yellow Card scheme.19

Early loss includes water weight, so the trend over weeks matters more than any single weigh-in. Some people notice appetite change in week one, while others feel little until a higher dose. Both are normal.

09 · Weight loss surgery

Weight loss surgery and medical cards

Your GP or a specialist weight-management service can explain what applies to you and how referral works.

Surgery and injections are not always either/or, though. Some people regain weight years after a gastric sleeve or bypass and ask about Mounjaro. It can be clinically appropriate, but the decision sits with an individual prescriber. Surgery changes gut anatomy and nutrition needs, and trial data in post-surgical patients is limited. Declare any bariatric history in your consultation.

If you take Mounjaro and are having an operation or procedure under sedation or general anaesthetic, tell the surgical team. Because Mounjaro slows stomach emptying, they may give specific fasting or medication-pause instructions before the procedure.1

10 · Buying safely

How to buy safely and spot counterfeits

Buy weight loss injections only from a pharmacy you can verify on the GPhC register.12 Ours is registration 9012878. A prescription-only medicine should only be supplied after a consultation reviewed by a prescriber. Anyone offering to supply without a consultation or prescription is a warning sign.

The MHRA publishes counterfeit medicine alerts and runs the Yellow Card scheme for reporting suspected problems.7 Social media sellers and unregulated websites are where counterfeit pens turn up. Dramatic weight-loss claims are another red flag.

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
